N-Butyl-N-(4-hydroxybutyl)nitrosamine 유도 백서 방광암 유발과정에 미치는 cyclooxygenase-2 억제제의 역할

dc.description.abstract[한글] 인체 방광암에서 cyclooxygenase-2 (COX-2)는 암세포 증식 및 혈관신생과정 촉진에 중요한 역할을 한다. 따라서 방광암 억제에 있어서 COX-2 억제가 중요하게 인식되고 있다. COX-2 선택적 억제제는 in vivo 및 in vitro 연구결과 다양한 종양 형태에서 항암효과를 가진다. 이에 본 연구는 BBN 유도 백서 방광암의 발암과정에서 COX-2 선택적 억제제의 항암 또는 항혈관신생 효과를 연구하였다. 90마리의 수컷 Fischer 344 백서를 구입하여 40마리 (대조군)는 0.05% BBN을 24주 동안 자유롭게 음용시켰고, 40마리 (실험군)는 0.05% BBN 투여 1주전부터 1500mg/kg celecoxib을 1회/1일 24주 동안 경구투여함과 함께 0.05% BBN을 자유롭게 음용시켰다. 나머지 10마리는 정상대조군으로 사용하였다. 발암단계가 변화하는 4주, 12주, 16주, 24주에 대조군과 실험군에서 각각 10마리 백서를 희생시켜 방광을 절개한 후 육안적 및 현미경적으로 관찰하였다. 또한 COX-2 선택적 억제제의 COX-2 발현, 혈관신생인자 (미세혈관 밀도, vascular endothelial growth factor)에 대한 영향을 측정하였다. 그리고 조직학적 판정에 따라 발암 직전 단계인 12주 대조군과 12주 실험군을 대상으로 COX-2 선택적 억제제의 유전자 발현에의 영향을 cDNA microarray법으로 측정하였다. 육안적 및 현미경적 소견은 12주 실험군 백서 방광조직에서부터 총 종괴의 크기와 수의 감소와 분열기 세포 출현의 지연을 보여주었다. 그리고 COX-2 선택적 억제제에 의해 COX-2 발현 및 미세혈관 밀도, VEGF 발현 같은 혈관신생인자는 12주 실험군부터 각 대조군보다 의미있게 감소하였다. 발암 이전 단계인 전암병변이 우세한 12주 실험군을 12주 대조군에 대하여 방광조직에서의 유전자 발현을 비교한 결과, COX-2 선택적 억제제는 15개의 유전자를 조절시켰고 그 중 transgelin, membrane metallo endopeptidase, apolipoprotein E를 발암 억제 방향으로 조절시켰다. 결론적으로, COX-2 선택적 억제제는 항혈관신생 효과와 COX-2 발현 억제를 통하여 BBN 유도 백서 방광암의 발암과정을 억제시켰다. 특히 발암 이전 단계에서 COX-2 선택적 억제제는 transgelin, membrane metallo endopeptidase, apolipoprotein E 유전자를 발암 억제 방향으로 조절시켰다. 향후 COX-2 선택적 억제제에 의해 조절된 transgelin, membrane metallo endopeptidase, apolipoprotein E의 작용기전 규명과 함께 COX-2 선택적 억제제의 항혈관신생 효과와 COX-2 발현 억제를 통한 방광암 억제 효과가 임상적으로 재현된다면 COX-2 선택적 억제제의 방광암 예방 및 치료제로서의 역할을 기대할 수 있을 것이다. [영문]Cyclooxygenase-2 (COX-2) plays an important role in promoting cancer cell proliferation and angiogenesis in human bladder cancer. Selective COX-2 inhibitor has an antitumor activity in vivo and in vitro for a variety of tumor types. In this study, the antitumor or antiangiogenic effects of selective COX-2 inhibitor on BBN-induced rat bladder tumorigenesis were investigated. Forty male Fischer 344 rats (the control groups) were given only 0.05% BBN in water ad libitum, while 40 rats (the experimental groups) were administered 1500mg/kg celecoxib once daily by gavage tube, and this treatment started from 1 week before the BBN treatment. Ten rats were used as normal control. Ten rats from the control and experimental groups were sacrificed at the time of 4, 12, 16 and 24 weeks after BBN treatment. All bladders were evaluated macroscopically and microscopically. We also measured COX-2 expression, microvessel density (MVD) and vascular endothelial growth factor (VEGF) protein concentrations in bladder tissues. Utilizing a cDNA microarray, we analyzed the significant differences of gene expression between the 12 week-control group and the 12 week-experimental group. Under macroscopic and microscopic observations, the incidence of tumor was lower in the experimental group than in the control group from week 12 to week 24. The incidence of tumor at week 12, week 16, and week 24 was 20%, 50%, and 80% in the control group, and 0%, 20% and 40% in the experimental group, respectively. In the control and experimental groups, COX-2 expression mainly tends to concentrate in the cytoplasm of the epithelial cells in papillary tumor, and in the endothelial cells that are adjacent to the vessel of the basal layer of the bladder. COX-2 and VEGF expression were more significantly decreased from BBN induction (p<0.05), in the experimental groups than in the control groups after 4 weeks. MVD was decreased significantly in the 16 week-experimental group rats (p<0.05). At 12 week-experimental group, there were 15 genes altered by the administration of selective COX-2 inhibitor, and selective COX-2 inhibitor especially regulated transgelin, membrane metallo endopeptidase and apolipoprotein E of these 15 genes to prevent the incidence of bladder tumor. In conclusion, selective COX-2 inhibitor has an inhibitory effect because of its antiangiogenic effects on BBN-induced rat bladder tumorigenesis. In preneoplastic phase, selective COX-2 inhibitor regulates transgelin, membrane metallo endopeptidase and apolipoprotein E to prevent the incidence of bladder tumor. If these inhibitory effects are reproducible in human bladder tumorigenesis, then in the future, selective COX-2 inhibitor could be expected to play an important role as a chemopreventive agent and as a therapeutic aid for bladder cancer.-
