[한글]

동맥경화 혹은 기타의 원인에 의한 혈관폐쇄로 인한 허혈성 심혈관 질환에서 혈관 침범의 정도가 광범위하거나 비교적 말초 혈관에 침범된 경우, 혈관성형술이나 우회로술 등의 치료가 적합하지 않으며 불량한 예후를 가지는데, 최근 혈관신생 유도를 통한 치료가 많은 관심을 끌고 있다. 혈관 내피 세포 성장인자 (vascular endothelial growth factor, VEGF)를 비롯한 몇 가지 성장 인자들이 혈관 내피 세포에 대한 특이적인 세포 증식 촉진 물질로 허혈성 사지 질환이나 심근 허혈 동물 모델에서 신생 혈관을 증가시키는 것으로 보고 되었으나 임상적인 효과에 대해서는 아직 검증이 더 요구되고 있다. 한편 VEGF와 더불어 최근 연구가 활발히 이루어지고 있는 섬유아세포 성장인자 (fibroblast growth factor, FGF)는 섬유아세포뿐 만 아니라 내피 세포의 성장을 강력히 촉진하는 단백질로 알려져 허혈성 질환에 대해 치료 효과가 있는 것으로 밝혀지면서 이를 응용한 전 임상 및 임상 연구가 이루어지고 있지만 이의 치료 효과에 대한 기전 연구는 미미한 형편이다. 본 연구에서는 토끼의 하지 동맥 결찰로 허혈 상태를 만든 허혈성 조직에 대해 염기성 섬유아세포 성장인자 (basic FGF, bFGF) 유전자를 전달하여 혈관신생 효과를 정량적, 기능적인 분석을 하고, bFGF에 의한 혈관신생 과정의 신호 전달기전을 분자생물학적 방법으로 규명하고자 하였으며, bFGF 유전자 전달 과정에서 관찰된 근육 세포의 비후 기전을 밝히고자 하였다.

하지 허혈 모델은 토끼를 이용하여 대퇴 동맥 전부를 절제하는 방법으로 만들었으며, 2주 후 혈관조영술을 시행하여 허혈 모델을 확인하고, 아데노바이러스 전달체를 이용하여 제조한 bFGF 유전자 전달체 (Ad/bFGF)를 근육 주사하였다. Ad/bFGF 근육 주사 4주 후에 거시적 혈관조영술 및 미시적 조직 염색 결과 측부 혈관 및 신생 혈관의 생성 빈도가 대조군에 비하여 월등히 증가됨을 확인하였다. 또한 신생혈관의 기능적 변화를 관찰하기 위해 혈관 도플러 초음파를 혈관조영술과 동시에 시행하여 본 결과 bFGF군에서 혈류 속도가 유의하게 증가됨을 검증하였다.

이러한 bFGF 유전자 치료의 혈관신생 기전을 밝히기 위해 근육 세포에 유전자 전달 후 분비되는 인자들을 조사한 결과 bFGF 외에도 VEGF가 함께 분비되는 것을 확인하였으며, bFGF 유전자가 전달 된 골격근 세포로부터 유리된 조건 배지 (conditioned media)를 이용한 혈관 내피 세포에 대한 작용 및 신호 전달에 대한 분자생물학적 조사 결과 내피 세포의 효과적인 증식을 입증하였고, 이는 bFGF 유전자 전달 후 근육 세포에서 분비되는 bFGF 및 VEGF에 의한 이중 작용 기전을 통해 Raf-MEK1/2-ERK1/2의 신호 전달 경로를 활성화시킴으로써 내피 세포의 생존과 증식에 영향을 주는 것으로 생각된다. 또한 bFGF에 의해 혈관신생 뿐만이 아니라 근육세포의 비후도 관찰되었는데, 이는 bFGF에 의해 분비되는 IL-6에 의해 매개되는 JAK-STAT의 신호 전달 경로를 통해 이루어짐을 규명하였다.

결론적으로 bFGF 유전자 치료는 토끼의 하지 허혈 모델에서 효과적인 혈관신생 효과가 있는 것을 확인하였으며, 이러한 혈관신생의 효과는 bFGF 유전자에 의해 bFGF와 함께 VEGF가 분비되고 이 둘의 이중 작용 기전을 통해 Raf-MEK1/2-ERK1/2의 신호전달경로를 거쳐 이루어진다는 것을 규명하였다. 또한 혈관신생 효과를 확인하는 과정에서 발견된 근육 세포의 비후는 IL-6로 인한 JAK-STAT의 신호 전달 경로를 통해 이루어짐을 규명하였다. 본 연구에서는 세포내의 복잡하고 다양한 신호 전달 경로를 완전하게 밝혀낸 것은 아니지만, 강력하게 영향을 줄 수 있는 주요한 신호 전달 경로를 규명함으로 인해 허혈 조직에 대한 혈관신생 과정을 조절할 수 있을 것이라 생각되며, 이는 향후 혈관신생 유전자 치료를 하지 허혈 뿐 만 아니라, 관동맥, 뇌동맥 질환에 의한 전신의 허혈 조직에 임상적으로 응용할 수 있을 것으로 기대된다.





[영문]The purpose of this study is to confirm the angiogenic effect of bFGF gene therapy in rabbit ischemic hind limb model and to disclose the mechanism of basic fibroblast growth factor (bFGF) angiogenic effect.

Recombinant gene (Ad/bFGF) was constructed by using human bFGF gene and adenovirus vector by homologous recombination technique. The efficiency of transfection and the effect on the endothelial proliferation and migration was confirmed in vitro. We made ischemic hind limb model in the rabbit. The bFGF gene was delivered intramuscularly into the ischemic hind limb (5×109 pfu). LacZ was used as control gene. Four weeks after bFGF gene transfer, increased new vessel formation and strengthened collateral vessels were observed. Interestingly, skeletal muscle cell hypertrophy was also found in bFGF gene delivery group.

In vitro culture showed marked increase of VEGF, IL-6, as well as bFGF. So, we hypothesized that these two additional factors might affect the angiogenesis and skeletal muscle cell hypertrophy.

Phosphorylation inhibition study demonstrated that the angiogenesis after bFGF gene transfer was significantly affected by the Raf-MEK1/2-ERK1/2 pathway under dual activation mechanism of Raf by both bFGF and VEGF. This finding was confirmed by Western blot immunostaining, MTT assay, and endothelial cell proliferation study. Also it was demonstrated that the secreted IL-6 after bFGF gene transfer influenced to skeletal muscle cell hypertrophy by significant activation of JAK2-STAT3 signal transduction pathway. These findings were demonstrated using Western blot immunostaining with glycoprotein 130 neutralizing antibody and AG490, as JAC2 inhibitor. And also, [3H]-leucine incorporation assay revealed these inhibitors significantly reduced skeletal muscle cell hypertrophy induced by bFGF gene transfer. The clinical significance of skeletal muscle cell hypertrophy after bFGF gene therapy needs to be cleared.

In conclusion, the angiogenic effect of bFGF intramuscular gene therapy in rabbit ischemic hind limb model was confirmed. And the phosphorylation inhibition study showed that Raf-MEK1/2-ERK1/2 signal transduction pathway directed by dual action mechanism of bFGF and VEGF might be involved in the angiogenic process of bFGF gene therapy.
