Clinicopathologic and molecular analysis of prognostic biomarkers in colorectal carcinomas

dc.description.abstract[한글] 대장암은 선진국형 질환으로 우리 나라에서도 그 발생률이 증가하고 있다. 수술 후 항암 치료는 일부 환자에서 생존 기간을 늘일 수 있지만 상당한 morbidity를 초래한다. 따라서 수술 후 치료 방향 결정에 있어 전이를 포함한 보다 정확한 예후 예측의 척도가 필요하다. 대장암의 경우, 현재 방사선 영상이나 조직학적 소견만으로 예후, 특히 재발이나 전이 가능성 여부를 정확히 예측하기는 어렵다. 대부분의 분자 표지자 (molecular marker) 들은 예후 예측에 있어 아직 검증 단계이며, 그 검증을 위해서는 대량의 증례에 대해 체계적이고 통계적으로 유의한 결과를 필요로 한다. 따라서 본 연구에서는 301명의 대장 및 직장암 절제술을 한 증례를 대상으로 대량의 증례를 비교적 짧은 시간에 평가할 수 있는 한 방법인 tissue microarray를 사용하여 파라핀 포매된 대장암 조직에 E-cadherin, MUC-2, p53, beta-catenin, Rb, bcl-2, hMLH1, hMSH2, 등 예후 관련 분자 표지자를 면역조직화학 염색하였고, 각 예에 대하여 현미부수체 불안정성과 18번 염색체 장완의 이형접합성 소실을 검색하였다. 이상의 결과들을 임상 및 조직학적 인자와 함께 분자 표지자 발현이 생존에 미치는 관계를 분석하였다. 수술 후 치료 방향 결정이 중요한 제2기와 제3기 환자에서 univariate analysis 결과 분자유전학적 표지자 중 현미부수체 불안정성, E-cadherin, 및 MUC-2의 발현이 예후와 관련이 있었으며, multivariate analysis 결과 임상 및 조직병리학적 인자는 이전의 보고된 생존 관련 결과와 유사하였고, 분자 표지자 중 E-cadherin의 발현이 소실된 경우에서 독립적으로 나쁜 예후와의 관련성을 보였다. 본 연구는 기존의 임상적, 조직학적 예후 인자에 더하여 분자유전학적 예후 인자를 비교하여 예후 예측도에 있어 보다 유용한 인자를 가려내고자 하였으며, 현미부수체 불안정성, E-cadherin, MUC-2 등의 분자유전학적 인자들은 향후 수술 후 치료 방향 결정 및 예후 예측에 유용할 것으로 생각된다. [영문]Clinical and histological staging after surgery have been the gold standard in predicting prognosis and planning for adjuvant therapy in colorectal cancer. In stages II and III colorectal carcinomas, it is important to identify the subset of patients who will experience recurrence or metastasis, and those for whom the adjuvant therapy is most beneficial. With recent development in molecular pathology, it became possible to characterize the colorectal carcinomas beyond the clinical and histologic aspect. In this study, clinical, histologic, and molecular factors for 301 colorectal carcinoma patients were analyzed for prognostic implication. Expressions of molecular markers including p53, bcl-2, Rb, hMLH1, hMSH2, beta-catenin, E-cadherin, and MUC-2 were analyzed for correlation with prognosis using tissue microarray. With fresh tissue samples, status of microsatellite instability (MSI) and 18q loss of heterozygosity (LOH) were analyzed. The univariate analysis of markers in stages II and III showed the clinical factors such as the left-sided colonic location in stage II, age over 70 in stage III, higher preoperative serum carcinoembryonic antigen (CEA) level (greater than or equal to 5 ng/mL) in all stages correlated with poor overall survival. Histologic factors including the irregular growth pattern and perineural invasion were significantly correlated with poor overall survival in stages II and III. The molecular factors such as loss of expression of E-cadherin and MUC-2 showed significant correlation with poor overall survival in all stages. Patients with MSI-H tumors showed significant correlation with better overall survival in combined stages II and III. Multivariate analysis showed those with higher stage, higher preoperative serum CEA level (greater than or equal to 5 ng/mL), perineural invasion, loss of E-cadherin and MUC-2 expression independently correlated with poor overall survival. These results indicate that among the analyzed molecular markers in this study, MSI, MUC-2 and E-cadherin appear to be useful in predicting prognosis and planning for adjuvant therapy in stages II and III colorectal carcinomas, in addition to clinicopathologic factors.-
