주의력결핍 과잉행동장애의 한국형 진단도구 개발 및 도파민 D4 수용체 유전자와의 연관성

dc.description.abstract[한글] 대표적인 소아청소년 정신과적 장애인 주의력결핍 과잉행동장애 (Attention Deficit Hyperactivity Disorder : 이하 ADHD)의 원인은 아직까지 잘 알려져 있지 않지만 도파민 (dopamine) 관련 유전자와 연관이 있다고 제시되어왔다. 특히, 도파민 D4 수용체 유전자 (Dopamine D4 Receptor gene : 이하 DRD4)가 ADHD와 연관이 있다는 보고가 많이 있어 왔으나 모두가 백인을 대상으로 한 연구들이었다. 따라서, 본 연구에서는 전 세계적으로 널리 쓰이고 있는 소아청소년 정신장애 진단 도구인 Kiddie-Schedule for Affective Disorders and Schizophrenia - Present and Lifetime Version (K-SADS-PL)을 한국어로 번역한 한국판 K-SADS-PL (K-SADS-PL-Korean version : 이하 K-SADS-PL-K)의 표준화 작업을 거쳐 국내 소아청소년 정신 장애의 정확한 진단을 도모하는 데 사용하고자 하였다. 그 후 K-SADS-PL-K로 선별된 ADHD 아동들과 부모를 대상으로 하여 가족유전상관연구 방법을 이용하여 ADHD와 DRD4 다형성 (polymorphism)과의 연관성을 알아보고자 하였다. 1차적으로 K-SADS-PL-K 표준화 연구에서의 연구 대상은 국내 4개 대학 병원 소아청소년 정신과 외래를 방문한 80명의 환아들과 정상 아동군 11명이었다. K-SADS-PL-K 진단의 타당도 및 면담자간 신뢰도, 검사-재검사 신뢰도를 평가하였다. 이후 2차적으로 K-SADS-PL-K에 의해 ADHD로 선별 진단된 73명의 아동과 그 부,모를 대상으로 분자유전학적 검사를 시행하였다. 가족유전 상관분석인 transmission disequilibrium test (TDT)는 질환군-대조군 연구 (case-control study)에 비하여 서로 다른 집단간의 이질성으로 인한 통계적 오류를 최소화할 수 있다는 장점이 있는 바, 본 연구에서 한국인 ADHD 아동들과 그 부모를 대상으로 TDT를 시행했을 때 DRD4의 다형성과 ADHD 사이의 연관성이 있을 것이라고 가정하였다. 본 연구의 결과는 다음과 같다. K-SADS-PL-K의 표준화 연구 결과, ADHD는 높은 신뢰도를 (Kappa = 0.756) 보였고 틱 장애와 반항성 도전장애는 비교적 높은 신뢰도 (Kappa = 0.40, 0.46)를 보였다. 면담자간 신뢰도는 ADHD와 틱 장애에서 비교적 높게 나타났고 (Kappa = 0.421, 0.421), 검사-재검사 신뢰도는 두 질환 모두에서 매우 높았다 (Kappa = 0.755, 1.000). 이러한 결과는 K-SADS-PL-K이 한국 아동들의 ADHD를 포함한 소아정신장애를 진단하는데 유용하게 사용될 수 있는 진단 도구임을 알 수 있었다. 2차적으로 행해진 유전자 상관분석연구에서는 한국 ADHD 아동과 그 부,모로 구성된 73 트리오가 유전자 연구 분석에 포함되었다. 총 73 트리오의 DRD4 exon III 48bp에서 318, 364, 410, 458, 502 의 5 종류의 allele이 발견되었다. 이것은 각각 2,3,4,5,6 repeats allele로서 백인을 대상으로 했을 때 DRD4의 다형성에서 흔히 관찰되는 7-repeats allele는 전혀 발견되지 않았다. TDT 분석을 이용하여 5가지의 allele들 각각이 부모로부터 ADHD 아동에게 전달 (transmitted) 또는 비전달되는 (not transmitted) 빈도를 본 결과, 458 bp (5-repeat allele)에서 비전달 (non transmission)의 빈도가 유의하게 상승된 것 이외에는 유의한 연관성이 존재하지 않는 것으로 나타났다. 본 연구의 DRD4와 ADHD의 가족유전상관연구 결과는 ADHD의 표현형 (phenotype)이 백인과 한국인에서 유사하다 하더라도 유전형 (genotype)과 다형성의 양상이 다를 수 있다는 점을 시사한다. 향후 한국 정상 대조군을 포함한 이와 같은 ADHD 가족유전상관연구가 시행될 필요가 있다. [영문] The causes of attention deficit hyperactivity disorder (ADHD), a common child and adolescent mental disorder, have not been defined clearly, but dopamine-related genes are considered to be associated. Many studies have reported that in particular dopamine D4 receptor gene (DRD4) is related to ADHD, but all of them investigated European-Caucasians as the subjects. We standardized the Korean version of Kiddie-Schedule for Affective Disorders and Schizophrenia -Present and Lifetime Version (K-SADS-PL), a worldwide common diagnostic tool for child and adolescent mental disorder, for accurate diagnosis of child and adolescent mental disorder in Korea, and investigated the association of ADAH and DRD4 polymorphism by using the methods for family-based association study on ADHD children and their parents who were selected by the K-SADS-PL-Korean version (K-SADS-PL-K). The subjects of the K-SADS-PL-K standardization study consisted of 80 child outpatients who were admitted to the child and adolescent psychiatric department in four university hospitals in Korea and 11 normal children. We examined the validity and inter-rater reliability and test-retest reliability of the K-SADS-PL-K. Then, molecular genetic tests were performed on 73 children who were diagnosed with ADHD by K-SADS-PL-K and their parents. Considering that the transmission disequilibrium test (TDT) as family based genetic association analysis can minimize the statistical errors induced by the heterogeneity among different groups better than case-control study, we assumed that there was an association between the polymorphism of DRD4 and ADHD in the TDT analysis on Korean ADHD children and their parents. According to the results of the K-SADS-PL-K standardization study, the reliability of ADHD was a good to excellent range (Kappa = 0.756) and that of tic disorder and oppositional defiant disorder was fair (Kappa = 0.40, 0.46). Inter-rater reliability was fair in both ADHD and tic disorders (Kappa = 0.421, 0.421), while test-retest reliability was excellent in both disorders (Kappa = 0.755, 1.000). Such results indicate that the K-SADS-PL-K may serve as a useful tool for diagnosing Korean child and adolescent psychiatric disorders including ADHD. From 73 trios of Korean ADHD children and their parents investigated in the molecular genetic study, we defined five kinds of allele (318, 364, 410, 458, 502) which were respectively 2,3,4,5,6 repeats allele, while failing to find 7-repeats allele that was common in the polymorphism of European-Caucasians. Also, in contrast to the findings in European-Caucasian children with ADHD, transmission of any these alleles to the Korean ADHD children from their biological parents were found not to be associated. These negative results of the family based association study on DRD4 and ADHD suggest that the patterns of the genotypes and polymorphisms of ADHD can be different between European-Caucasians and Koreans though the phenotypes of ADHD are similar. Further studies are required on the family-based genetic association of ADHD with Korean normal control group.-
