개질 고분자 이식편을 이용한 관상동맥용 스텐트 그라프트의 개발

dc.description.abstract[한글] 본 연구에서는 기존의 고분자 인조혈관을 표면처리 방법을 통하여 그 물성을 개선하여 혈전형성을 최소화하고, 이를 튜브형태로 제조하여 관동맥질환 치료에 적합한 스텐트 그라프트를 개발하고자 하였다. 실험에 사용한 고분자는 Dacron, GoreTex 및 Teflon으로 이 세 가지 인조혈관의 표면에 플라즈마 및 UV ozone 처리로 반응기를 형성시킨 후, 혈액적합성이 우수한 인지질 유도체인 hydroxybutyl acrylate (HBA) choline 단량체를 표면에 고정화시켜 안정적이고 효과적인 항혈전능을 가지도록 하였다. 플라즈마 및 UV ozone 처리 조건을 다양하게 변화시켜 HBA choline이 고분자 표면에 가장 많이 부착되는 최적의 조건으로 체외에서의 혈액적합성 평가실험을 실시하였다. 그 결과 HBA choline의 중합량이 증가할수록 우수한 혈액적합성을 나타내는 것을 확인하였다. 세 가지 표면개질 한 고분자 이식편을 사용하여 관상동맥용 스텐트 그라프트를 제작하고 이를 돼지 관상동맥에 삽입하여 생체내 실험을 하였다. Dacron과 GoreTex를 이용해서 제작한 스텐트 그라프트를 삽입한 돼지의 경우, 모두 48시간 이내 사망하였다. 반면에 Teflon을 이용한 스텐트 그라프트를 삽입한 돼지는 6마리 중 5마리가 4주까지 생존하였고, 추적 관상동맥 조영술 상 4마리의 돼지의 관상동맥은 개방된 소견을 보였다. 또한, 혈관내 초음파 및 조직 검사상 전체 스텐트 그라프트가 완전히 신생내막에 의해서 둘러싸여 혈관내에 잘 고정되어 있었고, 관상동맥내 혈전 및 만성 염증반응 세포는 발견되지 않았다. 결론적으로 개질 고분자 Teflon 이식편을 이용한 관상동맥용 스텐트 그라프트를 개발하여 돼지관상동맥에 성공적으로 시술하였고, 향후 더 많은 연구와 임상에서의 효과를 입증하여야 할 것이다. 핵심 되는 말: 고분자, HBA choline, 혈액 적합성, 스텐트 그라프트, 관상동맥 [영문] The purpose of this study is to develop a new type of coronary stent-graft by surface modification of established polymeric synthetic graft materials and modifying it into tubelike structures for possible application in coronary intervention. The purpose of the modification process is to limit the thrombogenecity of the graft and to improve biocompatibility. Three different polymers, Dacron, GoreTex, and Teflon were used in this study. During the suface-modification process, peroxides were produced by treatment of polymers with plasma or UV ozone that enabled hydroxybutyl acrylate (HBA) choline, an excellent blood-compatible phospholipid derivate, to be stably grafted on the polymeric surface of the grafts. The optimal conditions for variables such as the amount of electrical power to be used and the duration of treatment with plasma that are needed to maximize the amount of HBA choline that can be grafted onto the polymeric surface was determined by quantitative analysis. Under these optimal conditions, in vitro analysis of biocompatibility was performed. The results of the in vitro platelet adhesion test revealed that all HBA choline-grafted polymers showed better biocompatibility compared to the none-modified control polymers. Also, the biocompatibility showed a positive correlation with increasing quantity of the grafted HBA choline. Under optimal conditions as determined in previous experiments, we constructed stent-grafts using three different types of polymers modified with HBA choline-grafting. The stent-grafts were then implanted in porcine coronary arteries to compare their biocompatibility in vivo. All pigs implanted with modified Dacron or GoreTex stent-grafts died within 48 hours after implantation. However, five out of the six pigs implanted with Teflon stent-grafts were alive at the 4th week of follow-up. The follow-up angiography at the 4th week showed patent coronary artery in four out of the five surviving pigs and total occlusion in one pigs. IVUS and histologic evaluation demonstrated uniform neointima formation covering the entire stent-graft without any evidence of stent thrombosis or chronic inflammatory cells. In conclusion, the development of a biocompatible coronary stent-graft using surface-modified polymers was successful. However further animal studies and clinical studies are required to validate the efficacy of surface-modified polymer stent-grafts.
