국소 뇌허혈에서 뇌저온 요법의 허혈 손상 및 matrix metalloproteinase에 대한 영향

dc.description.abstract[한글] 뇌졸중은 크게 뇌출혈과 뇌경색으로 나누어지는데 단일 질환으로서는 가장 흔한 사망 원인이며 생존하는 경우에도 많은 후유 장애를 남긴다. 뇌경색의 치료와 관계된 연구는 뇌허혈시에 발생하는 뇌손상의 기전을 밝혀 이를 억제하여 치료에 응용하고자 하는 시도와 함께 막힌 뇌혈관을 빠른 시간 내에 재관류 시킴으로써 허혈에 빠진 뇌조직을 회복시키고자 하는 방향으로 이루어지고 있다. 그러나 아직까지는 급성기의 혈전 용해술 이외에는 효과적인 치료법은 거의 없는 상태이다. 뇌저온 요법은 뇌경색에서 뇌손상을 감소시키는 목적으로 시도되고 있는데 동물 실험에서는 뇌경색 및 뇌부종의 크기를 감소시키고 신경학적 장애를 호전시키는 것으로 알려졌고 임상적으로는 일부에서 심한 뇌경색의 환자에서의 유용성이 보고되었다. 뇌저온 요법은 여러 가지 다양한 기전에 의하여 뇌 보호 효과를 나타내는 것으로 생각되는데 그 기전 중의 하나가 혈액-뇌 관문의 보호작용이다. 한편 뇌경색에서 세포외 기질을 파괴하는데 관여하는 MMP-2 및 MMP-9 활성도가 증가하며 또한 혈액-뇌 관문을 손상시키는데 관계하는 것으로 알려져 있다. 그러나 아직까지는 뇌경색에서 MMP-2 및 MMP-9와 뇌저온 요법과의 관계는 밝혀진 바가 없다. 이번 연구에서는 실험동물에서 중뇌동맥 폐색 및 재관류 방법으로 뇌경색을 유발하여 MMP-2 및 MMP-9의 활성화와 뇌경색의 크기와의 상관 관계를 알아보고 뇌저온 요법이 뇌경색의 크기, 혈액-뇌 관문의 손상 및 MMP-2 및 MMP-9의 활성도에 미치는 영향을 규명하고자 하였다.실험동물은 자발성 고혈압 쥐를 이용하였는데 2시간 동안 중뇌동맥을 폐색한 후 18시간 재관류한 후에 희생하였다. 실험대상은 대조군, 정상 체온군, 허혈중뇌저온 요법군, 허혈후뇌저온 요법군으로 나누었으며 뇌저온 요법군의 경우에는 체온을 섭씨 32도까지 낮추어서 2시간 동안 유지하였다. 각 군에서 적출된 뇌를 triphenyltetrazolium chloride으로 염색하여 뇌경색의 크기를 측정하고, gelatin zymography를 이용하여 뇌조직에서의 MMP-2 및 MMP-9의 활성도를 측정하였다. 혈액-뇌 관문의 손상 정도는 허혈중뇌저온 요법군과 정상 체온군에서 에반스 블루의 혈관외 유출 정도를 측정하였다. 허혈중 뇌저온 요법은 정상 체온을 유지한 경우에 비하여 뇌경색의 크기를 55.1% 통계적으로 유의하게 감소시키는 효과를 보였다. 허혈후 뇌저온 요법은 정상 체온에 비하여 뇌경색의 크기를 11.9% 감소시켰으나 통계적인 유의성은 없었다. 허혈중 뇌저온 요법은 정상 체온 및 허혈후 뇌저온 요법에 비하여 허혈 대뇌반구에서의 MMP-2 및 MMP-9 활성도를 통계적으로 유의하게 감소시켰다. 허혈 대뇌반구에서의 MMP-2 및 MMP-9 활성도는 뇌경색의 크기와 통계적으로 유의한 양의 상관관계를 보였다. 허혈중 뇌저온 요법은 정상 체온을 유지한 경우에 비하여 혈액-뇌 관문의 손상을 통계적으로 유의하게 감소시켰다. 이상의 결과를 종합할 때 허혈중 뇌저온 요법은 뇌허혈 및 재관류시에 발생하는 뇌경색의 크기를 감소시키며 혈액-뇌 관문의 손상도 억제하는 효과를 보였는데 이러한 보호 효과에는 MMP-2 및 MMP-9의 억제가 중요한 기전으로서 작용한다고 생각한다. 허혈후 뇌저온 요법의 경우에는 통계적으로 유의한 뇌경색 억제 효과를 나타내지는 못하였으나 임상적인 적용에 있어서의 중요성을 고려할 때 향후 추가적인 연구가 필요할 것으로 생각한다. [영문] Stroke is the leading cause of death and leaves much physical and mental disability. The principal goal of treatment is a rapid recanalization of the occluded artery and protection of brain tissues from the ischemic injury. The brain hypothermia has been effective in reducing infarction size and has improved neurologic deficits in experimental cerebral ischemia. The protection of blood-brain barrier (BBB) disruption is one of the proposed mechanisms of brain hypothermic effect. Matrix metalloproteinase (MMP)-2 and MMP-9, which are increased in ischemic brains, are known to be related to the destruction of the BBB by digesting extracellular matrix proteins. In this study, we investigated the effect of brain hypothermia on focal cerebral ischemia in relation to the infarction size, the activity of MMP-2 and MMP-9, and BBB disruption. Using an intraluminal nylon thread, spontaneous hypertensive rats were subjected to 2 hours of middle cerebral artery occlusion (MCAO) and 18 hours of reperfusion. The normothermia group was maintained at 36.5°C during the procedure. Moderate hypothermia was induced by maintaining the temperature at 32°C during MCAO in the intra-ischemic hypothermia group, and for 2 hours immediately after reperfusion in the postischemic hypothermia group. Two mm-thick coronal slices of brains were stained with 2,3,5-triphenyltetrazolium chloride solution to define the area of ischemic damage. By gelatin zymography, the activity of MMP-2 and MMP-9 was measured in frozen sections of ischemic and non-ischemic hemispheres. Evans blue extravasation methods were used to determine the BBB disruption. The percent areas of infarctions were significantly reduced in the intra-ischemic hypothermia group by 55.1% compared to those in the normothermia group. Although the reduction of the percent areas was also observed in the postischemic hypothermia group (11.9% smaller than the normothermia group), it did not reach the statistical significance. The integrated densities of MMP-2 and MMP-9 activities in ischemic hemispheres were significantly lower in the intra-ischemic hypothermia group than the normothermia or the postischemic hypothermia group. The size of infarctions was correlated with MMP-2 and MMP-9 activities. The disruption of BBB was significantly reduced in the intra-ischemic hypothermia group compared to the normothermia group. The intra-ischemic hypothermia effectively attenuated the ischemic injury and BBB disruption in focal cerebral ischemia. Hypothermia-mediated inhibition of MMP-2 and MMP-9 activities might contribute to this protective effect.-
