연령과 건강 행위에 따른 남성 호르몬의 변화

dc.description.abstract[한글] 중노년 이후 평균 여명 증가로 삶의 질에 대한 관심이 증대되면서 사회적, 의학적으로 건강 증진 및 노화 방지에 대한 관심과 인식의 변화가 빠르게 진행되고 있으며 임상 의학적 측면에서는 현재 다양한 노화 방지 요법들이 시행되고 있다. 그 가운데 50대 이후 중노년 남성들에서 여성의 갱년기와 유사한 증상을 호소하며 이것은 연령 증가에 따른 부분적 남성 호르몬 결핍에 기인한다는 사실들이 밝혀짐에 따라 남성들의 노화 방지를 위하여 남성 호르몬 치료의 필요성이 제기되고 있다. 이처럼 남성들의 적극적인 노화 방지 치료의 필요성이 제기되는 시점에서 본 연구는 첫째, 연령별 평균 남성 호르몬 농도를 알아보고 둘째, 연구 대상자들의 일반적 특성 및 건강 행위가 남성 호르몬 변동을 어느 정도 설명하는지 알아보고자 하였다. 본 연구는 2000년 1월부터 2001년 12월까지 성남 소재 모 대학병원 양한방 종합 건강 증진 센터에서 남성 호르몬 검사를 시행한 803명 가운데 검사 당시 남성 호르몬 치료를 받고 있는 환자, 예전에 고환 절제술을 시행한 환자, 과거 및 현재 만성 질환이 있는 환자를 제외한 629명을 대상으로 단면 연구를 시행하였다. 연구 대상자들의 신체 계측으로 신장, 체중 및 혈압을 측정하였고 자가 기입 설문지를 통하여 음주, 흡연, 운동 유무를 조사하였다. 전체 연구 대상자의 평균( 표준편차) 연령은 54.60( 9.83)세였고 연령별 평균 총 테스토스테론(total testosterone)은 40세 미만군, 40-49세, 50-59세, 60-69세, 70세 이상군에서 각각 5.85( 1.62), 5.39( 2.04), 5.21( 1.88), 5.19( 1.58), 5.53( 2.79) ng/mL 으로 조사되었다. 다중 회귀 분석 결과 연령, 체질량 지수, 성 호르몬 결합 단백질(sex hormone binding globulin), 음주 및 흡연은 총 테스토스테론의 변동을 23.7% 설명하였다. 연령, 체질량 지수, 성 호르몬 결합 단백질, 음주 및 흡연을 각각 통제한 상태에서 연령이 1년 증가시 총 테스토스테론은 0.028 ng/mL 감소(p < 0.01)하고 체질량 지수 1 kg/m2 증가할 때마다 총 테스토스테론은 0.060 ng/mL 감소(p < 0.05)하였다. 성 호르몬 결합 단백질이 1 nmol/L 증가함에 따라 총 테스토스테론은 0.049 ng/mL 증가(p < 0.01)하였다. 그 외 음주군과 흡연군에서 비음주군과 비흡연군에 비해 각각 총 테스토스테론이 0.257, 0.140 높았으나 각각의 변수들을 통제한 상태에서 통계학적으로 유의한 차이는 없었다. 본 연구 결과 총 테스토스테론의 변동은 연령, 체질량 지수, 성 호르몬 결합 단백질, 음주 및 흡연 이외에도 다양한 생물학적, 사회 환경 요인이 복합적으로 작용함을 시사한다. 따라서 중노년 남성이 부분적 남성 호르몬 결핍(partial androgen deficiency for aging male) 증상을 호소할 경우 이를 유발할 수 있는 다양한 사회 환경적 요인을 고려할 필요가 있으며, 향후 남성들의 노화 방지 및 건강 증진을 위한 다양하고 체계적인 연구와 의학적 접근이 필요할 것으로 생각된다. [영문] As the average life span was increased after the middle aged, the interest to the quality of life after the middle aged have been increased. There have been much interested and changed the concept of health promotion and anti-aging therapy in social and medical aspects with the increasement of life expectancy and much interest to the quality of life. It is evident that function of the testes decreases gradually with age. This decline in testicular function after middle aged contributes to a series of signs and symptoms of aging such as menopausal symptoms in women. And some study have been suggested the need for testosterone replacement therapy after middle aged men who have experienced and been suffered the symptoms of partial androgen hormone deficiency. At the time of needed testosterone replacement therapy, we had evaluated that the relationship between health behavior such as drinking, smoking and exercise, and androgen hormone. Also, we had evaluated the change of serum concentration of testosterone according to the change of the aging. We had enrolled the subjects from January in 2000 to December in 2001. We have cross-sectioned evaluated the subjects that 629 men excluding current testosterone replacement therapy, previous orchiectomy and chronic disorders in past and current medical history among 803 men had been participated in the study. We had investigated anthropometric parameters such as height, and weight, blood pressure, androgen hormone, and surveyed health behavior such as drinking, smoking, and exercise by self-questionnaires. The mean( SD) age was 54.60( 9.83) years in the subjects. The mean total testosterone and sex hormone binding globulin were 5.31( 1.88) ng/mL and 36.23( 16.84) nmol/L. The calculated mean( SD) free androgen index and bioavailable toestosterone by the formula were 16.60( 7.36) and 25.62( 10.17) nmol/L. The age, body mass index, sex hormone binding globulin, drinking and smoking were correlated 23.7% of the total testosterone in mutiple regression analysis(R2=0.237). According to the decreasing age and increasing body mass index, total testosterone were decreased 0.028 ng/mL/year(p < 0.01) and 0.060 ng/mL/year(p < 0.05). Also, total testosterone was increased 0.049 ng/mL/year(p < 0.01) according to the increasing sex hormone binding globulin. The total testosterone in drinking and smoking group were 0.257 and 0.140 as much as in non-drinking and non-smoking group but were not significant after adjusted each variables. / The results have shown that the changes of testosterone have been contributed many factors such as age, drinking smoking and others. At the time of increasing interest of aging male in the social and medical aspects, we have approached to the subjects variously that have been complained the symptoms of partial androgen hormone deficiency by the varied possible causes of partial androgen hormone deficiency after middle aged men. Also, the results have been suggested the need of further study for aging male.-
