소규모사업장의 일반질병자 사후관리에 관한 연구

dc.description.abstract[한글] 건강진단제도의 목적을 달성하기 위해서는 사후관리가 매우 중요함에도 불구하고 일반질병자의 경우 인식부족과 사업주의 법적 의무 소홀 등에 의해 관리가 미비한 상태이고, 특히, 보건관리자 선임 의무가 없는 50인 미만 소규모사업장의 경우는 더욱 심각한 실정이 다. 따라서 본 연구는 50인 미만 소규모사업장에서의 건강진단후 사후관리 실행에 따른 문제점을 파악하고 대안을 제시하기 위해서 수행되었다. 인천지역 근로자중에서 전년도 건강진단결과 일반질병자로 판정 받은 50인 미만 소규모사업장 근로자 83명과 보건관리대행 및 보건관리자선임사업장 근로자 100명을 무작위로 선정하여 설문 조사하였으며 분석한 결과는 다음과 같다. 1. 사후관리 실행율은 소규모사업장 61%, 보건관리대행 및 보건관리자선임사업장 85%로 소규모사업장에서 낮은 사후관리 실행율을 보였다. 2. 보건관리자가 있는 경우와 사업장내 건강관리 활동이 있는 경우, 그리고 개인적으로 운동을 하는 경우가 사후관리 실행율이 높게 나타났다. 3. 건강진단에 대한 만족도가 높고 건강진단결과에 대해 잘 알고 있을수록 그리고 통보된 질병에 대해 잘 알고 있을수록 사후관리 실행율이 높게 나타났다. 4. 사후관리를 위해 회사에서 지원해 주기를 원하는 것은 상담 지도, 경제적 도움, 유급휴가 및 직장동료간의 격려를 꼽았고, 사회나 정부로부터는 일반질병 전문병원 지정 및 진료기회 제공, 치료비 지원, 건강증진프로그램 개발 및 참여기회 제공 등을 가장 필요로 하는 것으로 나타났다. 5. 소규모사업장 중 과거 국고지원 경험유무에 따른 비교에서 건강진단결과 통보율과 검진결과의 설명 정도에 있어서 과거 국고지원 경험이 있는 사업장이 의미있게 높았다. 이상의 결과에서 소규모사업장의 경우 사후관리실행이 낮았으며, 이는 주로 건강진단통보 정도와 검진결과 설명여부와 관련되었다. 따라서 향후 소규모사업장의 근로자의 건강진단 사후관리를 보다 활성화하기 위해서는 건강진단결과에 대한 통보 및 상담이 잘 이루 어질 수 있도록 하기 위한 노력이 필요하다. [영문] Post management is very important in order to achieve the goal of workers' health examinations. This is insufficient with lack of understanding of employees with regard to general diseases and the unlawful negligence of employers to provide adequate health services to the their employees. Especially, small scale enterprises with fewer than 50 employees without a health service manager, have very serious problems to address. Therefore, This study was conducted to identify the problems of post management and to propose an alternative plan in small scale enterprises with fewer than 50 employees. This study was taken on 83 employees of small scale enterprises with fewer than 50employees, 100 employees out of the enterprises with a group health service agency and enterprises with a health service manager, diagnosed with general diseases at last year`s health examination results of the employees who work in the Inchon area. The subjects of study are a random sample. The study used questionnaires completed by employees with general diseases. The result of this study were as follows: 1. The percentage of post management is lower at small scale enterprises (61%) than enterprises with a group health service agency and enterprises with a health service manager (85%). 2. The percentage of post management is higher with employees with a health service manager, when the health management activity is at their worksite, those employees who exercise personally, opposed to employees without a health service manager, without a health management activity at their worksite, those employees who do not exercise personally. 3. The more satisfaction with the health examination, the more aware the employeesare of their test results, the more understanding the employees have of their diseases, thus the percentage of post management is higher. 4. The employees with general diseases want health counselling, economic help, time off for appropriate post management from their enterprises and they also feel that they need a designated hospital for those employees with general diseases for the opportunity for treatment, support of the treatment, development and participation of a health promotion program from the Korean Government and society. 5. In the comparison between the two groups of small scale enterprises, the group with past experience with health management funded by the national treasury has a higher percentage (64%), the other group with no experience has a lower percentage of post management (59%). Variables that influence the post management showed a significant association with the reports of the health examination results and the explanation of the health examination results. From these results, The percentage of post management is lower (61%) at small scale enterprises.It wasassociated with the reports of the health examination results and the explanation of the health examination results. Accordingly, in order to activate the post management of employees at small scale enterprises, efforts need to be made to achieve the reports of the health examination results and health counselling.-
