고도난청환자에서 인공와우에 의한 전기자극이 이명에 미치는 영향

Abstract

[한글]이명에 대한 인공와우의 효과는 여러 저자들에 의해 보고되어 왔다. 본 실험은 신촌 세브란스병원 이비인후과에 내원한 고도난청 환자 중 인공와우를 시술하고 이명을 가지고 있는 환자 20례를 대상으로 연구하였다. 인공와우이식을 통한 전기자극이 이명억제에 미치는 영향을 조사하였고 관련되는 인자와 유형을 조사하였다. 검사방법은 수술 전후 이명의 증감정도를 표시하게 하였고 측정은 다양한 조건하에서 시행하였으며, 변형된 McKerrow 방법을 사용하였다. 실험 결과는 인공와우를 일상생활에서 사용할 때 장착한 귀에서의 이명의 억제효과는 67%, 인공와우를 장착하지 않은 귀에서도 63%에서 이명의 억제를 보여 한쪽의 전기자극은 양쪽의 이명감소 효과를 보였다. 또한 인공와우를 통하여 차폐음을 사용한 경우에는 인공와우를 이식한 귀에서는 약 89%에서 이명의 호전을 보였고, 이식하지 않은 귀에서는 63%의 이명의 감소 효과를 보였다. 인공와우이식 전후의 이명이 일상생활에 미치는 효과는 이식한 귀에서는 거의 대부분 일상생활의 향상을 가지고 왔다고 하였으며, 이식하지 않은 귀에서는 79%에서 일상생활의 향상을 보았다고 하였다. 전기적 자극이 이명억제를 일으키기 위해서는 와우에 가까워야 하고, 안전하게 시행될 수 있어야 하고, 청력을 보존할 수 있어야 한다. 인공와우의 장착으로 이명의 감소가 있는 것은 다른 전기장치가 이명을 치료할 수 있는 하나의 지표로 자리잡을 것으로 기대할 수 있다.

[영문]Tinnitus is one of the most obscure pathologies. There is no universally proved treatment modality for tinnitus. The beneficial effects of cochlear implant on tinnitus have been noted in several studies. Electrical stimulation, by means of the cochlear implant, has various effects on tinnitus in the profound deaf. Twenty patients fitted with cochlear implant device were studied. The effects of use of the implant device on tinnitus were monitored, and suppression of tinnitus was tracked throughout periods of stimulation and poststimulation. Tinnitus was effectively suppressed in 67% on implanted ear, also in 63% non-implanted ear. Under electrical stimulation, tinnitus was effectively suppressed in 89% on implanted ear, also in 63% non-implanted ear. Overall, the majority of the patients thought that their cochlear implant was helpful in tinnitus suppression, especially in the ear with the implant. In order to apply the electrical stimulation to control the tinnitus, the electrical stimulation should be safe and preserved the hearing. The author thought that the other electrical stimulation may be helpful on tinnitus.
