척수손상 백서에서 Nitric Oxide변화가 신경전달에 미치는 영향

dc.description.abstract[한글] 척수손상을 치료하는데서 발생하는 많은 어려움중 하나는 직접적인 물리적 손상뿐만 아니라 손상후 2차적인 척수변화에 의해 조직학적 이상과 기능적인 결함이 진행된다는 것이다. 2차적인 신경손상을 설명하기 위해 많은 실험과 약물투여가 시도되어 왔지만 명확한 기전을 밝혀내지는 못했으며 이에 따른 치료법도 완전히 정립되지 않았다. 2차적 신경손상의 여러 가설 중 최근 관심이 집중되고 있는 것은 NO (nitric oxide)에 의한 신경독성이다. NO는 L-arginine이 NOS (nitric oxide synthase)에 의해 생성되며 NOS는 isoform으로서 cNOS (contitutive NOS)와 iNOS (inducible NOS)가 있다. cNOS는 생체내에 정상적으로 혈관(eNOS)이나 신경(nNOS)에 존재하며 정상적인 신체반응을 조절한다. iNOS는 외상이나 자극에 의해 염증세포나 신경세포내에서 생성되며 대량의NO를 만들어 내는 것으로 알려져 있다. NO가 신경독성 작용을 나타내는 것은 비교적 공통된 의견으로 알려져 있다. 그러나 NO를 생성하는 cNOS나 iNOS의 기능과 신경 독성에 대해서는 다른 의견이 많으며 특히 cNOS에 의한 초기 신경보호 혹은독성작총에 대해서는 완전한 이론이 정립되지 않았다. 본 연구에서는 비특이성 NOS길항제인 L-NAME(NG-nitro-L-arginine methyl ester)를실험동물인 백서에 투여하였고 백서는 척수손상 직후 saline (iv)만 투여한 군을 대조군으로 하였고 약물효과를 보기위해 척수손상 15분전에 전처치로 L-NAME (iv, 60mg/kg)을 투여한 군, 척수손상 2시간(iv,40 mg/kg)과 5시간(ip,40 mg/kg)째에 L-NAME를 투여한 군, 척수손상 24시간(iv, 40 mg/kg)과 27시간(ip, 40 mg/kg)째에 L-NAME를 투여한 군으로 나누고 급성기의 신경회복을 보기위해 척수손상 5일후(각 군당 5마리),그리고 만성기의 신경회복을 보기위해 척수손상 2주후(각 군당 10마리) 체성감각 유발전위와 운동검사를 시행하여 다음과 같은 결론을 내렸다 1. 척수손상 5일후 시행한 체성 감각 유발전위 검사중 saline만을 투여한 군은 약물투여군과 현저한 차이를 보였다. Saline군은 파형이 전혀 관찰되지 않았으나 L-NAME를 15분전에 전처치한 군, 2시간과 5시간째에 투여한 군 및 24시간과 27시간째에 투여한 군에서는 파형이 나타났다. 2. 척수손상 2주후 시행한 체성 감각 유발전위 검사중 saline만을 투여한 군을 포함하여 각 군당 파형이 나타났으며 대조군인 saline만을 투여한 군에 비해 L-NAME를 척수손상 15분전에 전처치한 군에서 절정진폭이 통계학적으로 의미있는 상승을 보였다(p< 0.05). 3. 운동 검사는 척수손상후 1일, 4일, 7일, 10일, 14일째 시행하였으며 7일째부터 L-NAME를 전처치한 군이 saline만을 투여한 군에 비해 운동기능이 통계학적으로 의 미있게 향상되었으며(p<0.05)다른 약물투여군도 saline군에 비해 높은 점수를 보였다. 이상의 연구결과로 척수손상 초기에 NO (nitric oxide)를 생성시키는 cNOS가 급성기척수손상 기전에 상당한 기여를 한다는 것을 보여 주며 NO의 신경독성 작용은 NOS(nitric oxide synthase)길항제인 L-NAME에 의해 억압될 수 있고 척수손상 전에 투여하는 것을 포함하여 가급적 빠른 시간 내에 투여하는 것이 효과적이라는 것을 알 수 있다. [영문] One of the main difficultis in the treatment of spinal cord injury is that neural deficit is originated not only from direct physical damage, but also from secondary biochemical change. The exact mechanisms of secondary neuronal damage are still unknown and their treatment is still obscure even though many researchs have been performed about them. Recently, NO (nitric oxide) is known as one of the important neural toxic materials. NO is converted from L-arginine by NOS (nitric oxide synthase). There are two isoforms of NOS, cNOS (contitutive NOS) and iNOS (inducible NOS). cNOS exist normally in vessels(eNOS) or neural tissues (nNOS). iNOS is produced in inflammatory cells or neural cells by trauma or extemal toxic stimulation. NO has been known to express neural toxicity. However, there are many different opinions about the functions and the neural toxicity of cNOS and iNOS, especially early neural protection or neural toxicity of cNOS. In this research, the effects of non-specific NOS inhibitor, L-NAME (N**G_nitro-L-arginine methyl ester), on neural conduction in spinal cord injured rat were evaluated. Rats were divided into the control group (saline injection only) and the other 3 groups (injection of L-NAME 15 minutes before injury (iv, 60 mg/kg), injection of L-NAME 2 (iv, 40 mg/kg)and 5 (ip, 40 mg/kg) hours after injury, injection of L-NAME 24 (iv, 40 mg/kg) and 27(ip,40 mg/kg) hours after injury), SSEP (somatosensory evoked potential) and behavior test were performed 5 days after spinal cord injury for the acute effects and 2 weeks after injury for the chronic effect. 1. In the measurement of SSEP 5 days after spinal cord injury, the group treated with saline only did not appear wave. The groups of pretreatment, 2 & 5 hours and 24 ? 27hours injection of L-NAME showed waves. 2. In the measurement of SSEP 2 weeks after spinal cord injury, the group of saline only, pretreatment, 2 & 5 hours and 24 & 27 hours injection of L-NAME showed waves. There was a increased amplitude of SSEP wave between saline group and pretreatment of L-NAME with statistical significance. 3. The behavior tests were performed 1 day, 4 days, 7 days, 10 days and 14 days after spinal cord injury, There was statistical significance in the group of pretreatment of L-NAME comparing to saline injection group. From the above results it is speculated that neural toxicity of NO, which is derived by NOS after spinal cord injury, is inhibited by NOS inhibitor (L-NAME) that is efficient to be injected before the cord injury, The results suggest that cNOS may contribute to the mechanism of early secondary spinal cord injury, too.-
