Adriamycin으로 심근증이 유발된 백서에서 심근 교감신경 손상과 I-131-metaiodobenzylguanidine(MIBG)섭취와의 상관관계

Adriamycin은 광범위한 암종에서 효과가 입증된 우수한 항암제이나 장기간 사용할 경우 심부전등의 부작용이 유발될 수 있다. 이러한 부작용을 조기에 진단하는 방법이 많이 연구되어있고 좌심실 박출계수의 측정은 임상 진단 방법 중의 하나이다. 그리고 일부에서 adriamycin에 의한 심근독성이 심근에 분포된 교감신경 손상과 연관되어 있고 I-131-metaiodobenzylguanidine(MIBG) scan은 임상적으로 교감신경 손상을 조기에 발견할 수 있는 방법이라고 보고되었다. 그러므로 본 연구의 목적은 adriamycin으로 심근증이 유발된 백서에서 심장 교감신경의 변화를 이미 보고된 방법과 다른 방법, 즉 특수염색과의 비교를 통하여 I-131-MIBG의 유용성을 평가하고자 하는 것이다. Wistar rat(평균 무게 200g)에 주당 1회씩 2mg/kg의 adriamycin을 각각 4주(n=10), 6주(n=11) 및 8주(n=11) 동안 미정맥으로 투여한 후 각 기간 마다 high performance liquid chromatography법으로 혈중 norepinephrine치를 측정하였다. 좌심실박출계수는 소형 동물용12MHz 탐촉자를 사용하여 M-mode영상에서 좌심실의 수축기 및 이완기말 내경을 바탕으로 측정하여 심기능의 지표로 삼았다. 실험동물에서 autoradiography 및 PGP9.5 면역염색과 일반염색을 위하여 74MBq/kg의 I-131-MIBG과 thallium-201을 미정맥으로 주사하고 4시간 후 조직 슬라이드를 얻었다. 좌심실의 중간부위에서 autoradiography의 단축영상을 얻은 뒤 좌심실벽을 8구역으로 나누고 각각의 구역을 다시 subepicardium, midmyocardium 및 subendocardium의 3층으로 나누어 한 개체 당 총 24개의 관심영역을 설정하였다. 이들 각 구역에서 통합강도(PSL/mm**2) 및 PGP9.5에 의하여 염색된 신경섬유의 수를 측정하였다. 결과적으로 혈중 norepinephrine은 adriamycin투여에 따라 유의한 변화가 없었으며, adriamycin에 의하여 유발된 심장독성은 심초음파보다 autoradiography에 의하여 조기에 발견되었다. 즉 좌심실박출계수는 8주에서 감소하는데 반하여(p=0.0001) MIBG섭취 및 PGP9.5 염색상에서 계수된 신경섬유

의 수는 6주부터 유의하게 감소하기 시작하였다(p=0.001). 대조군에서 교감신경의 분포는 subendocardium보다 subepicardium에 풍부하였으며(p=0.001) subendocardium은subepicardium보다 adriamycin에 의하여 쉽게 손상됨이 MIBG autoradiography영상과 면역조직학적 분석 결과상 관찰되었다(p=0.005). 반면에 thallium-201의 섭취는 adriamycin의 투여 기간에 따른 유의한 변화를 보이지 않았다. 결론적으로 I-131-MIBG 스캔은 adriamycin에 의한 심장독성을 민감하게 반영하는 생화학적 표식자일 뿐만 아니라 PGP9.5에 의한 면역조직학적 소견과도 잘 일치함을 확인할 수 있었다.

Adriamycin is one of the most useful antitumor agents in current oncologic practice, however, therapeutic potential of this agent has been limited by a frequently irreversible and often fatal drug induced congestive heart failure. A number of approaches have been used for early detection of adriamycin induced cardiomyopathy. Measurement of left ventricular ejection fraction (LVEF) is one of the standard clinical approaches to monitoring adriamycin cardiotoxicity. This study was to evaluate cardiac sympathetic nerve damage as a biological marker of cardiac toxicity in adriamycin induced cardiomyopathic rats. Wistar rats(average weight 200g) were treated by injection of adriamycin(2mg/kg) via tail vein once a week for 4(n=10), 6(n=11) and 8 weeks(n=11). Serum norepinephrine level was evaluated by high performance liquid chromatography(HPLC) technique. LVEF

calculated by M-mode echocardiography using 12MHz transducer for small animal was used as an indicator of cardiac function. Pathologic specimens were obtained 4 hours after intravenous injection of I-131-MIBG and thallium-201(74MBq/kg) for digital autoradiography, and for histologic examinations including protein gene product (PGP) 9.5 immunostain. Short axis images of the autoradiography at left ventricular mid body level was divided into 8 segments, and each segment was divided into subepicardial, midmyocardial and subendocardial layers (24 ROls).

Integrated intensity(PSL/mm**2) within the ROls was measured by autoradiography and the number of nerve fibers was counted using a morphometric grid under light microscope. Serum norepinephrine level did not reveal significant difference between control and adriarmycin treated groups. LVEF decreased in 8-week group(p=0.0001) but MIBG uptake and nerve fibers on PGP9.5 stain significantly decreased in 6week group(p=0.001). The distribution of the adrenergic nerve was more abundant in subepicardial layer than subendocardial layer(p=0.001).

Subendocardial layer appeared to be more vulnerable than subepicardial layer on both MIBG autoradiographic images and immunohistochemical analysis (p=0.005). Thallium-201 shows no significant difference in uptake. In conclusion, I-131-MIBG

is a sensitive biochemical marker of adriamycin induced cardiomyopathy which was well correlated with immunohistochemistry using PGP9.5.
