사람 정상 코점막 및 비용 상피세포에서 점액과 리소자임의 발현 양상

dc.description.abstract[한글] 사람 정상 코점막 및 비용 상피세포에서 점액과 리소자임의 발현 양상 사람의 정상 기도에는 호홉시 유입되는 먼지나 병적인 균들로부터 기도를 보호하는 점액섬모계가 있다. 그러나 천식, 만성 기관지염, 급·만성 비염 또는 낭성섬유증 등과 같은 질환에서는 배세포의 수가 증가하고, 점막하선내의 점액세포들이 비후되어 점액의 과다분비에 의한 점액섬모계 장애 때문에 유해한 세균들의 증식에 의한 조직의 손상이 발생한다. 이러한 기도내 점액과다분비의 조절기전을 밝히기 위하여 많은 연구가 진행되었고, 최근 분자생물학적 기법의 발달에 힘입어 사람에서 9가지의 점액유전자가 알려졌으나 아직도 정확한 조절기전은 밝혀지지 않았으며, 더욱이 하기도에서 발현되는 점액에 대한 연구에 비하여, 비강과 부비동을 포함한 상기도에 대한 연구는 매우 미흡한 실정이다. 이에 연구자는 본 연구를 통하여 점액유전자 및 리소자임이 사람의 정상 하비갑개조직과 비용의 상피에서 어떠한 발현 양상을 보이는가를 reverse transcription poly-merase chain reaction을 이용하며 알아보고, 비용상피세포 자체가 만성 부비동염에서 분비물의 증가에 어떠한 영향이 있는가를 알아보고자 하였다. 정상 상피세포는 정상 성인 10명의 하비갑개 점막에서 상피세포를 긁어 채취하고, 비용은 만성 부비동염으로 비용적출술을 시행한 환자에서 채취하였다. 비용에서 분리한 상피세포와 정상 상피세포를, 일정수의 세포당 세포내 점액과 리소자임 양을 immunoblot으로 정량하고, RT-PCR로 점액과 리소자임 mRNA의 발현 양상을 비교 분석 하였다. 점액성 분비물인 점액과 장액성 분비물인 리소자임의 세포내 양을 정량한 결과 사람의 정상 하비갑개 상피세포에 비하여 세포내 점액의 양이 2.9배의 뚜렷한 증가를 보였으나 장액성 분비물인 리소자임은 의미있는 차이가 없었다. 이는 염증성 매개체에 의하여 점액성 분비물은 증가하나 장액성 분비물은 변화가 없다는 것을 의미하며, 비용상피세포 자체가 만성 부비동염에서 점액의 분비증가에 일부 기여한다는 것을 의미한다. 점액과 리소자임 mRNA의 발현 양상을 비교 분석한 결과에 따르면 MUC2와 MUC8이 대조군에 비하여 비용상피세포에서 강하게 발현되어, 비용 상피세포에서 점액의 증가는 MUC2와 MUC8이 관여하고 있을 가능성을 의미한다. 비용 상피세포에서 MUC2 mRNA가 정상조직에 비하여 강하게 발현되는 것은, 비용 상피세포가 다양한 염증성 매개체에 노출되기 때문으로 생각하며, 또한 MUC8 mRNA의 증가는 임상질환에서 MUC8 mRNA가 증가될 수 있다는 매우 의미있는 결과로 사료된다. 그러나 아직 MUC8-특이적 항체가 없고 유전자 배열도 다 밝혀져 있지 않아 MUC8 mRNA의 정확한 기능은 알 수 없으나 향후 MUC8 mRNA의 조절기전과 MUC8 cDNA의 유전자배열을 밝히는 것이 중요할 것으로 생각한다. [영문] In normal human airway, the mucociliary system exist, which protects airways from inhaled unfavorable foreign materials and pathogenic microorganism. However, in such diseases as asthma, chronic bronchitis, acute/chronic rhinitis or cystic fibrosis, a number of goblet cells increase along with hypertrophy of mucous cells in submucosal gland, which makes mucociliary system ineffective of clearing out the pathogens. There has been continuous effort to clarify the regulation mechanism of airway mucous secretion, and with rapid development of advanced molecular biologic techniques, 9 human mucin genes has been identified. However, we still don't fully understand what kind of role each gene has or how they interact in producing mucous secretion which is prerequisite to understand the nasal physiology as well as expression patterns of them in upper airway system. In this study, author aimed to find out which mucin gene is expressed in the epithelium of normal human nasal mucosa and nasal polyps, and also to verify whether the epithelium of nasal polyp itself contribute to the increased nasal secretion as in chronic sinusitis accompanied by nasal polyp using immunoblot and RT-PCR. The level of intracellular mucin and Iysozyme in the epithelium of inferior turbinate and nasal polyp revealed that the level of mucin was 2.9 times higher in nasal polyp which was statistically significant. And, seven mucin genes (MUC1, 2, 4, 5AC, 5B, 7, 8) were expressed in the epithelium of normal inferior turbinate and polyps, but MUC2 arid MUC8 were more strongly expressed in the epithelium of nasal polyp than that of normal inferior turbinate. Therefore, the results suggests that the polyp epithelium itself is contributing to increased secretion in chronic sinusitis and MUC2 and MUC8 is thought to be responsible for this change. However, since there is no MUC8 mRNA-specific antibody available with fully identified genetic sequence, further study is required to uncover the full sequence of MUC8 mRNA and its exact function.-
