B형간염 바이러스 감염에 의한 만성 간질환에 있어서 HBxAg의 발현

B형간염 바이러스 감염에 의한 만성 간질환 및 간암에서 HBxAg의 발현과 그 의의를 알아보기 위해 HBx 항체를 이용하여 면역조직화학적 염색을 시행하고 이를 p53 및 Fas의 발현과 비교하여 보았다. HExAg에 대한 면역조직화학적 염색 결과 무증상 보유자의 18.2%,만성 간염의 17.9%, 간암 주변조직의 29.6%,그리고 간암의 3.7%에서 양성반응을 보였으며, 특히 문맥주위 활성도가 높고 간경변증으로 진행된 만성 간염과 간암 주변조직에서 높은 발현율을 보였다. HBxAg을 혈청 HBeAg의 유무와 비교하여 볼 때 혈청 HBeAg이 양성인 경우 HBxAg의 발현율이 높아 B형간염 바이러스가 증식하는 시기에 HBxAg이 많이 발현됨을 알 수 있었고, 이는 HepG 2215세포주와 Huh-1 세포주를 대상으로 northern blot을 시행하여 HBx RNA를 검출하였을 때의 결과와 일치하였다. HBxAg의 발현을 Fas의 발현과 비교한 결과 Fas는 HBxAg이 발현된 예에서 더 많이 발현하였고 HBxAg과 Fas가 모두 주로 문맥주변의 조각괴사를 보이는 간세포에 발현되는 것으로 보아 HBxAg의 발현이 만성 간질환에서 간세포의 고사에 관여한다는 것을 알 수 있었다. 간암조직의HBxAg발현과 p53 및 p21 발현사이에는 명확한 연관이 없었으며, 간암 주변조직에서 HBxAg이 발현된 경우에도 p53 및 p21의 발현과의 연관은 찾을 수 없었다.

We examined HBxAg expression by immunohistochemical method and compared with the expression of Pas or p53 in order to know the significance of HBxAg expression in the chronic B-viral liver diseases and hepatocellular carcinoma. HBxAg was expressed in 18.2% among the cases of inactive carrier, 17.9% among the cases of chronic hepatitis, and 3.7% among the cases of hepatocellular carcinoma. However, HBxAg was expressed in 29.6% among the nonneoplastic tissue of cases of hepatocellular carcinoma. HBxAg showed higher expression rate in the cases of chronic hepatitis with higher portal and periportal activities, and with cirrhotic

change. Comparing the expression of HBxAg with serum HBeAg, the expression rate of HBxAg was higher in the state of viral replication and this result was consistent with the expression rates of mRNA in the HepG 2215 and Huh-1 cell lines. Comparing

the expression of HBxAg with fas, there was a positive correlation between HBxAg and fas expression. The HBxAg and fas were expressed mostly in the cytoplasm of hepatocytes showing piecemeal necrosis. These findings suggest that the expression

of HBxAg is related to the apoptosis of hepatocytes. Using immunohistochemical stains of p53 and p21, there was no correlation between HBxAg expression and p53 or p21.
