비소세포성폐암에서 Cyclin D1,p21 및 p53의 발현과 예후와의 관계

[한글]

최근 폐암의 예후인자의 후보로 세포주기 조절인자가 대두되고 있다. 종양형성과 관련이 높은 세포주기 조절인자로 cyclin Dl단백, p2l단백과 p53단백이 보고되고 있다. 본 연구에서는 수술절제한 비소세포성 폐암 164예에 대하여, 면역조직화학염색을 이용하여, cyclin Dl, p21, p53 발현과 환자 병기, 조직학적 유형과 조직학적 분화도간 관계를 X**2-검정을 이용하여 연구하였다. Cyclin Dl, p21과 p53간의 상호작용이 X**2-검정 분석에 미치는 혼란요인을 제거하기위하여, 만텔-한젤(Mantel-Haenzel test)를 이용하였다. 비소세포성 폐암의 조직학적 유형은 편평상피암이 98예, 선세포암은 66예이었다. 면역조직화학 염색에서,비소세포성 폐암 164예중, cyclin D1은 83예(50.6%), p21은 71예(43.4%), p53은 97 예(59.1%)에서 양성 반응 보였다. p53 발현은 조기 병기(stage I, II, IIIa)와 통계학적으로 유의한 관련성을 보였고, 병기가 낮을수록 발현 빈도가 높았다(p<0.05). 그러나, cyclin D1과 p21 발현은 병기와 통계학적으로 유의한 관련성이 없었다(p>0.05). 조직학적 유형과의 관계에서, p21과 p53 발현은 편평상피암에서 선세포암보다 통계학적으로 유의한 더 높은 발현 빈도를 보였다(p<0.05). 조직학적 분화도에서, p21과 p53 발현은 조직학적 분화도가 저분화도일수록 발현빈도가 높았고, 통계학적으로 유의한 관련성을 보였다(p<0.05).

Cyclin Dl, p21과 p53 발현과의 상호작용에 관한 분석에서는 p21 발현과 p53 발현간에는 통계학적으로 유의한 관련성이 있었다(p<0.05). 만텔-한젤 통계기법 분석상에서, p53 발현은 p21 발현과 조직학적 유형간의 관계에 변화를 주었고, p21 발현은 p53 발현과 조기 병기간의 관계에 변화를 주었다. 그 외에는 cyclin Dl, p21과 p53 발현간의 상호작용이 이들 각각과 환자의 병기, 조직학적 유형과 조직학적 유형과 조직학적 분화도 간의 관계에는 변화를 주지 않았다.

결론적으로, 이 연구에서, cyclin Dl, p21과 p53은 비소세포성 폐암에서 종양형성에 관련있다고 생각된다. Cyclin Dl과 p21은 예후와 관련이 없다고 생각되며, p53 발현은 조기병기에서 유용한 예후인자라 생각된다. p21과 p53 발현은 선세포암보다 편평상피암 형성과 관련이 있다고 생각된다. p2l과 p53 발현은 조직학적 분화가 저분화를 일으키는 데 관련이 있다고 생각되며, 이에 대한 향후 연구가 필요하리라 본다. 마지막으로, p53 발현은 p21 발현과 조직학적 유형간의 관련성에 영향을 주며, p21 발현은 p53 발현과 조기 병기간의 관련성에 영향을 주는 것으로 생각한다. 이에 대한 향후 연구가 필요하리라 본다.

[영문]

Recently, cell cycle regulators have been suggested as new prognostic factors of the lung cancer. In this study, we evaluated the expression of the immunohistochemical markers for cyclin Dl, p21, and p53 using the X**2 test, with regard to the stage of the patients, histologic type, and histologic differentiation in the 164 cases of non-small cell lung cancer(NSCLC). To evaluate the confounding effects among cyclin Dl, p21, and p53 on X**2-test analysis, we used the Mantel-Haenzel test. The NSCLC in this study included 98 cases of squamous cell carcinoma and 66 cases of adenocarcinoma. Each nuclear staining of cyclic Dl, p21, and p53 was observed in 83 cases (50.6%), in 71 cases(43.4%), and in 97 cases(59.1%) of NSCLCs, respectively The immunohistochemical expression of the p53 was significantly associated with the early stage(stage I, II, IIIa)(p<0.05). But, the expression of cyclin Dl and p2l was not correlated with the stage of the NSCLC(p>0.05). Both p21 and p53 expession showed a more significant association with squamous cell carcinoma than adenocarcinoma(p<0.05). In addition, both p21 and p53 expression were significantly associated with poor histologic differentiation of NSCLC. The confounding effectcs among cyclin Dl, p21, and p53 did not influence on the relationship between the stage, histologic type and histologic grade of the NSCLC, except that p53 expression changed the relationship between p21 expression and histologic type, and p21 expression changed the relationship between p53 and early stage.

In conclusion, this study indicates that cyclin Dl, p21, and p53 may be implicated in the oncogenesis of the NSCLC and the p53 protein expression is a useful prognostic factor in the early stage, but cyclin Dl and p21 have no prognostic importance. It seems to be that both p21 and p53 may play a role more in the carcinogenesis of squamous cell carcinoma than in that of adenocarcimona, p53 expression may influence on the relationship between p21 and histologic type, and p21 expression may influence on the relationship between p53 and early stage, also. In this regard, further study is needed.
