퇴행성 요추질환에서 Graf연성고정장치의 치료 효과

dc.description.abstract[한글] 척추는 안정성과 운동성의 적절한 조화를 통하여 정상적인 기능유지가 가능하다. 척추의 안정성을 이루는 구조인 척추주위 근육, 척추, 인대, 추간반, 관절낭의 퇴행성 변화는 척추의 안정성을 파괴하여 과운동성을 일으키고 척추불안정성을 초래한다. 퇴행성 척추질환의 수술치료의 원칙은 광범위한 감압술과 감압술후 초래되는 불안정성의 예방으로 이를 위해 최근까지 척추경고정장치가 보편적으로 이용되어왔다. 그러나, 고정부 상하분절의 불안정성, 나사못골절등과 같은 많은 합병증의 발생으로 보다 생리적인 개념인 연성고정장치의 필요성이 증가되었다. 본 연구는 척추연고정장치인 Graf연성고정장치의 임상적 유용성을 알아보고자 1993년 9월부터 1996년 3월까지 연세대학교 의과대학 영동세브란스병원 척추센터에 입원하여 퇴행성 요추질환으로 Graf연성고정장치를 시행받은 106명의 환자를 대상으로 이들의 임상기록 및 치료성적을 분석하여 다음과 같은 결과를 얻었다. 1. Graf연성고정후 추간반간격이 L3/4에서 8.84mm에서 9.84mm로, L4/5는 9.28min에서 10.19mm로, L5/S1은 9.44mm에서 10.43mm로 통계적으로 유의하게 증가하였다. 2. Graf연성고정후 굴전불안정성이 L3/4에서 -6.9°에서 5.5°로, L4/5에서는 -7.45°에서 5.04°로, L5/S1에서는 -2.09°에서 10.81°로 통계적으로 유의하게 불안정성의 소실을 관찰하였다. 3. Graf연성고정후 전위불안정성이 L3/4에서는 16.8%에서 14.9%로, L4/5에서 19.9% 에서 12.4%로, L5/S1에서는 27.1%에서 20.1%로 호전되는 것을 관찰할 수 있었다. 4. 수술성공율은 Excellent 56%, Good 27%, fair 9%, poor 8%로 83%의 수술성공율을 얻었다. 5. 수술후 합병증은 3.7%로 비교적 안전하게 수술 시행할 수 있었다. 이러한 결과를 종합해 볼 때 퇴행성 요추질환, 특히 요추불안정성이 있는 경우 및 수술후 요추 불안정성이 예견되는경우 Graf연성고정장치는 치료에 유용하게 사용될 수 있을 것으로 판단된다. [영문] To maintain the normal function of the spine, stability and mobility should be in proper balance. Degenerative involution of the spine causes destruction of spinal stabilizer which consists of bone, ligament, joint capsule, and disc, which substantially leads to hypermobility and instability of the spine. A wide spinal canal decompression and prevention of the spinal instability are considered the major purposes of the operation in degenerative lumbar disease. Generally the hard fixation system has been used far the treatment of lumbar instability. However, hard fixation system has many complications, which include screw loosening, screw fracture, and instability on, above, and below the fusion segment. These complications of the hard fixation system bring out the invention of a more physiologic fixation device, the soft fixation system. A soft fixation system has many advantages over a hard fixation system which includes enough stability without arthrodesis after extensive surgical decompression, early rehabilitation, less operative risk and good preservation of spinal motion, We have used the Graf soft fixation system as an instrument for degenerative lumbar disease, performing a total of 106 cases using the Graf system between August, 1993 and March 1996. The clinical assessments, radiologic findings, and operative results were analyzed. 1. The height of disc space significantly increased from 8.84mm to 9.84mm on L3/4, 9.28mm to 10.13mm on L4/5, and 9.44mm to 10.47mm on L5/S1 after Graf soft fixation. 2. Flexion instability changed from -6.9° to 5.5° on L3/4, -7.45° to 5.04°on L4/5,-2.09°to 10.81°on L5/S1 after Graf soft fixation. 3. Translation instability was corrected from 16.8% to 14.9% on L3/4,19.9% to 12.4% on L4/5, 27.1% to 20.1% on 1.L5/S1 after Graf soft fixation. 4. The clinical result is as follows: 56% in degrees of excellent, 27% good, 9% fair, and 8% poor. 5. The operation was performed safely with minimal complications (wound infection 2, transient neurogenic bladder 1, and screw loosening 1) and total complication rate was 3.7%. These results suggest that Graf soft fixation system far degenerative lumbar disease would not only be useful and effective, but also avoid unwanted complications of the hard fixation system.-
