중족골 단축증의 신연술 후 발생된 합병증

Abstract

[한글]목적: 중족골 단축증에 대하여 신연 골형성술 시행 후 발생한 합병증과 관련된 요인을 분석하여, 수술 후 만족도를 높이기 위한 방안을 제시하고자 하였다.재료 및 방법: 중족골 단축증으로 신연 골형성술을 시행한 14명(남2, 여12)의 환자, 22예의 족부를 대상으로 하였다. 후향적 연구를 통하여 합병증의 발생 및 임상 결과에 영향을 미칠 것으로 예상되는요인을 선정하고 그 관계를 분석하였다.결과: 신연 골형성술 결과 11예에서 중족 족지 관절의 강직, 10예에서 중족 족지 관절의 편향(제4 중족골의 경우), 5예에서 중족 족지 관절의 아탈구, 5예에서 근위 지절간 관절의 과신전, 6예에서 핀 고정 부위 감염이 발생하였다. 임상 결과는 중족골궁을 초과한 연장을 시행한 군, 최초 길이의 40%이상의 연장 및 원래의 해부학적 축에 평행하지 못한 연장을 시행한 군, 환자의 나이가 많은 군에서좋지 않은 것으로 나타났다.결론: 중족골 단축증으로 신연 골형성술을 시행하는 환자에 있어 합병증을 줄이고 만족스러운 결과를 얻기 위해서는 최초 길이의 40%이상 연장되지 않도록 하는 것 이외에 정상 중족골궁을 초과한연장이 되지 않도록 하는 것이 중요하며, 또한 원래의 해부학적 축에 평행하게 연장되도록 하여 중족 족지 관절의 합병증 발생을 최소화 하여야 할 것이다.

[영문]Purpose: To present methods for satisfying patients with

brachymetatarsia after distraction osteogenesis by analyzing

factors related with complications.

Material and Methods: 22 metatarsals in 14 patients with

brachymetatarsia were included in the study. Several factors

thought to be related with complications and analyzed their

relationships with outcome of the surgery were analyzed.

Results: Metatarsophalangeal joint stiffness occurred in 11

metatarsals, joint deviation in 10 metatarsals(in case of fourth

metatarsals) joint subluxation in 5 metatarsals, hyperextension of

proximal interphalangeal joint in 5 metatarsals, and pin site

infection in 6 metatarsals. Lengthening more than 40% of the

original length, lengthening beyond the parabolic arc of the

metatarsal head, not lengthening parallel to the original axis of

the metatarsal bone and old age were thought to be related with

poor outcome of the surgery.

Conclusion: To reduce complications and to achieve good

outcomes in patients with distraction osteogenesis, it is important to avoid lengthening more than 40% of the original length and beyond the parabolic arc and make efforts to lengthen parallel with the axis of the metatarsal bone.
