구강 편평세포암종의 증식과 침습을 유도하는 간세포성장인자에 대한 녹차추출물의 효과

dc.description.abstract[한글] 간세포성장인자(Hepatocyte growth factor, 이하 HGF)는 다양한 종양에서 증식, 침습, 전이 및 혈관형성에 관여하는 중요한 예후인자로서 최근 항암표적인자의 하나로서 다양한 연구가 진행되고 있다. 본 연구는 구강암에서의 HGF/c-Met의 과발현에 따른 임상적 의의를 확인하고, 현재 항암제로서 임상연구가 활발히 진행되고 있는 녹차추출물의 주성분인 (-)-epigallo catechin-3-gallate(EGCG)의 구강암세포사멸효과와 HGF/c-Met knock-down시에 EGCG의 효과에 대해 비교분석하고자 하였으며 in vivo 실험으로 동계 마우스(syngenic mouse, C3H)을 이용하여 EGCG의 항암효과를 분석하였다. (대상 및 방법) 구강 편평세포암 61명에 대해 HGF와 c-Met에 대한 면역조직화학염색을 시행하고 3명 구강설 편평세포암 환자의 정상조직과 암조직에서 HGF와 c-Met의 발현을 RT-PCR과 Western blot으로 확인하였다. In vitro 실험으로 구강 편평세포암 세포주(KB cell line)에 EGCG을 처리한 뒤 MTT assay을 시행하였고, EGCG의 HGF 억제효과를 보기위해 wound healing assay와 invasion assay을 시행하였다. 또한 EGCG의 세포사멸효과를 알아보기 위해 Tunnel assay, DNA fragmentation analysis, FACS with Annexin V staining and PI, cell cycle분석, mitochondrial membrane potential변화, ROS의 형성에 대한 분석을 시행하였다. EGCG에 의한 HGF/c-Met signal 억제기전과 세포사멸 신호전달기전 분석을 위해 다양한 유전자에 대해 Western blot을 새행하였다. 또한 c-Met knock-down시의 EGCG의 효과를 알아보기위해 siRNA를 이용하여 구강암 세포주(KB cell line)에서 c-Met을 knock down 시킨후 위의 in vitro 실험을 시행하였다. In vivo 실험을 위해 마우스의 구강암세포주인 SCC7와 동계 마우스 C3H을 이용하였다. 5×105 개의 SCC7세포를 8주된 동계마우스 피하로 주입 후 2일째부터 녹차추출물인 EGCG을 매일 0, 0.5mg, 1mg, 1.5mg 투여군으로 나누어 각 마우스에 매일 투여하였다. 이틀마다 종양의 부피를 측정하여, 3주째 종양을 절개하여 크기와 무게를 측정하였다. 얻어진 조직에서 TUNEL 염색을 통해 EGCG의 세포사멸을 확인하였고 HGF/c-Met의 발현정도를 확인하였다. (결과) 면역조직화학적 검사상 c-Met의 발현은 54.1%에서 양성이었으며 림프절전이(p=0.005), 병기(p=0.004) 및 재발(p=0.018) 등과 통계학적으로 유의하게 연관성이 있었고, HGF와는 통계학적 유의성이 없었다. 생존율분석에서 c-Met의 발현이 양성인 경우 음성에 비해 생존율이 낮았다.(p=0.0032) 구강암 세포주(KB)에서 HGF에 의해 유도되는 암증식과 침습이 EGCG에 의해 억제되고 Tunnel assay, FACS with Annexin V-FITC and PI, DNA fragmentation analysis 등에서도 세포사멸이 HGF 존재시에도 효과적으로 증가됨이 확인되었고, Flow cytometry상 subG1의 증가를 확인할 수 있었다. HGF에 의해 유도되는 c-Met의 발현은 0.15μM의 EGCG에서도 효과적으로 억제되었고 phosph. AKT와 phosph. ERK의 발현이 감소됨을 확인할 수 있었다. 한편, c-Met을 knock down시킨 경우 wild type에 비해 EGCG에 의한 항암효과가 감소되어 나타나는 것을 확인할 수 있었다. C3H 동계마우스에서 EGCG의 용량이 증가함에 따라 종양의 부피와 무게가 통계학적으로 유의하게 감소하는 것으로 나타났으나 1mg 혹은 1.5mg투여군의 일부에서 EGCG에 의한 약물부작용으로 식욕부진, 무기력, 체중감소 및 사망 등이 관찰되었다. 3주 뒤 마우스를 희생하여 얻은 조직에서 시행한 TUNEL염색상 EGCG의 투여량에 비례하여 세포사멸이 증가되는 것으로 확인되었다. 종양의 진행에 관여하는 HGF와 c-Met의 발현도 EGCG처리시 감소되는 것으로 확인되었다. (결론) 본 연구를 통하여 HGF의 수용체인 c-Met이 구강암진행에 중요한 예후인자이며, EGCG는 구강암세포주에서 HGF/c-Met signaling을 억제하고 이를 통해 강력한 항암작용이 있음을 확인할 수 있었다. 또한 EGCG는 구강암 모델인 동계 마우스(C3H)에서 강력한 항암효과가 확인되었고 HGF/c-Met의 발현을 억제하는 것으로 나타나 향후 구강 편평세포암의 치료에 있어서 항암치료물질로서의 가능성이 있음을 확인할 수 있었다. [영문] Hepatocyte growth factor (HGF), a potent stimulator of hepatocyte growth, stimulates motility, invasiveness, proliferation, and morphogenesis of epithelium, and it may be involved in physiologic and pathologic processes such as embryogenesis, wound healing, organ regeneration, inflammation, and tumor invasion. (-)-epigallocatechin-3-gallate (EGCG) can modulate the cell signaling associated with angiogenesis, metastasis, and migration of cancer cells. In the present study, we have investigated the effects of HGF on invasion and metastasis of oral cavity cancer and the effect of EGCG on blocking HGF-induced invasion and metastasis in vitro and in vivo. We examined the role of the HGF/c-Met on invasion and metastasis of oral squamous cell carcinoma(SCC). We performed immunohistochemical stains on 61 normal mucosas and 61 SCCs of oral cavity with HGF antibody and c-Met antibody. For Western blot, fresh normal tissues and cancer tissues in oral cavity obtained from three patients and oral cavity cell line(KB) were used. The changes of tumor with HGF and EGCG treatment in KB cells were investigated by MTT assay, would healing assay, and invasion assay. The effect of EGCG on apoptosis of KB cells were investigated by TUNEL assay, DNA fragmentation analysis, FACScan with annexin V and PI, measurement of ROS generation, measurement of mitochondrial membrane potential transition, and cell cycle analysis. To survey effect of c-Met expression, c-Met knockdown by siRNA was performed. The effect of EGCG was tested in oral cavity cancer model used with C3H mouse and SCC7 cellline. The positive rates of HGF and c-Met expression in oral SCC were 57.3% and 54.1%, respectively. c-Met staining was significantly correlated with lymph node metastasis (p=0.005), stage (p=0.004), and recurrence (p=0.018) The increased expresssion of c-Met mRNA(RT-PCR) and protein (Western Blot) were detected in oral SCC tissue. EGCG showed dose-dependent delays in wound healing. HGF stimulated cell invasion, and this effect of HGF was inhibited by EGCG. c-Met knockdown inhibited KB cell invasion in Transwell chamber but the effect of EGCG on invasion was decreased in c-Met knockdown KB cells. EGCG inhibited significantly decreased MMP-2, 9 mRNA expression for 24 hours in a dose-dependent manner, revealed by RT-PCR. EGCG induced apoptosis, altered mitochondrial membrane potential and intracellular reactive oxygen species (ROS) generation in KB cells. But c-Met knockdown by siRNA partially inhibited EGCG-induced apoptosis, intracellular ROS generation, and mitochondrial membrane potential transition. The phosphorylation of Met, Akt, and Erk was decreased or disappeared at concentrations of 0.15, 30, and 5μM EGCG, respectively. EGCG activated p38, pJNK and increased cleaved form of PARP and caspase-3 in KB. In C3H mouse model, EGCG repressed mouse oral SCC(SCC7 cellline) tumor growth and inhibited HGF and c-Met expression of mouse oral SCC in Western Blot. These results suggest that HGF may play an important role in the progression of oral cavity cancer and EGCG may serveas a therapeutic agent to inhibit HGF-induced invasion in oral SCC patients.-
