임플란트의 디자인과 표면처리는 주위 골조직 치유과 개조에 많은 영향을 준다. 이에 본 연구에서는 치과용 임플란트의 나사산 거리(pitch)와 표면처리가 골유도 재생에 미치는 영향을 알아보고자 하였다. 길이 10mm, 직경 3.5mm의 타이타늄 임플란트를 장축을 중심으로 반쪽은 0.6mm pitch, 나머지 반쪽은 0.4mm pitch를 가지는 형태로 제작하였다. 이렇게 제작된 임플란트를 turned surface(TS), blasting & etching surface(BES), anodic oxidation surface(AOS)로 표면처리하여 가토 경골에 식립하고 높이 4mm 타이타늄 원통형 차폐막과 골이식재로 골조직의 재생을 유도하였다. 12주 후 가토를 희생하여 나사산 거리가 0.6mm와 0.4mm을 가지는 TS, BES, AOS가 골유도 재생에 미치는 영향을 조직형태학적으로 비교, 분석하여 다음과 같은 결과를 얻었다.1. 재생된 골조직의 수직높이는 BES와 AOS가 TS보다 유의성 있게 큰 값을 나타냈고, BES와 AOS의 0.4mm pitch가 0.6mm pitch보다 유의성 있게 큰 값을 나타냈다(p<0.05).2. 재생된 골조직의 폭경은 BES와 AOS가 TS보다 유의성 있게 큰 값을 나타냈고(p<0.05), 0.6mm pitch와 0.4mm pitch 사이에서는 유의차가 없었다.3. 재생된 골조직의 골접촉률은 BES와 AOS가 TS보다 유의성 있게 큰 값을 나타냈고(p<0.05), 0.6mm pitch와 0.4mm pitch 사이에서는 유의차가 없었다.4. 재생된 골조직의 나사산내 골양은 BES와 AOS가 TS보다 유의성 있게 큰 값을 나타냈고(p<0.05), 0.6mm pitch와 0.4mm pitch 사이에서는 유의차가 없었다.이상의 결과에서 BES나 AOS가 TS보다 골유도 재생에 유리하며, BES와 AOS의 0.4mm pitch가 0.6mm pitch보다 수직적 골유도 재생에 있어서 더 유리한 디자인이라고 여겨진다.



[영문]Design and surface feature have shown an influence on the bone healing and remodeling around implants. The purpose of this investigation was to study the response of different implant designs and surfaces influencing the healing of guided bone regeneration around implants.All the implants were custom-manufactured with turned surface(TS), blasting & etching surface(BES) and anodic oxidation surface(AOS) had 0.6mm pitch of half part and 0.4mm pitch of other half part. They were augmented with titanium chamber and particulated bone(autogenous & bovine bone). After 12 weeks healing, the response of guided bone regeneration around implants was evaluated with histomorphometric analysis. The following results were observed.1. Vertical height of regenerated bone at BES and AOS was significantly higher than TS and vertical height of regenerated bone at 0.4mm pitch with BES or AOS was significant higher than the 0.6mm pitch(p<0.05).2. Width of regenerated bone at BES and AOS was significantly higher than TS(p<0.05), but no significant difference was obtained between 0.6mm pitch and 0.4mm pitch.3. Bone to implant contact at BES and AOS was significantly higher than TS(p<0.05), but no significant difference was obtained between 0.6mm pitch and 0.4mm pitch.4. Bone volume in threads at BES and AOS was significantly higher than TS(p<0.05), but no significant difference was obtained between 0.6mm pitch and 0.4mm pitch.BES and AOS showed a higher degree of vertical height of regenerated bone, width of regenerated bone, bone to implant contact of regenerated bone, bone volume in threads of regenerated bone compared with the turned surface. The findings of this study imply that 0.4mm pitch design with BES and AOS resulted in a higher degree at vertical ingrowth of regenerated bone compared with 0.6mm pitch design.
