우리나라 기혼여성의 제왕절개분만 결정요인에 관한 연구

Abstract

[한글]제왕절개분만은 산모나 태아의 생명에 치명적 위험을 초래하여 정상분만이 어렵다고 판단될 경우에 사용되는 방법으로 널리 이용되어 왔고 20년전에 비해 제왕절개분만율은 세계적으로 증가 추세를 보이고 있다. 특히 1990년대 이후 우리나라에서는 제왕절개분만율이 급격히 증가하면서 이와 관련된 영아 및 산모의 건강상 위험 및 부정적인 사회·경제적 영향에 대한 우려의 목소리가 높아지게 되었고 이에 따라 우리나라 기혼 여성의 제왕절개분만 결정요인에 대한 구체적인 조사가 필요하게 되었다.본 연구는 제왕절개분만 결정에 영향을 미치는 다양한 관계를 사회·경제적 요인, 인구적 요인과 임신·출산에 관련된 요인을 통하여 우리나라 기혼여성의 제왕절개분만의 결정요인을 파악하여 제왕절개분만 감소를 위한 기초자료를 마련한 목적으로 시도되었다.연구대상은 한국보건사회연구원이 조사한 2003년 전국 출산력 및 가족보건·복지 실태조사자료를 이용하여 2001년 1월 1일 이후부터 2003년 4월까지의 최종 출생아 및 산모를 대상으로 하였으며, 총 분만건수 1,111명 중에서 자료에 무응답한 사람을 제외한 1,101명을 포함하여 분석을 진행하였다. 자료 분석은 크게 사회·경제적 특성, 인구적 특성, 임신·출산 관련 특성을 독립변수로 분만방법을 종속변수로 하여 기술적 통계 분석 및 카이스퀘어 검정을 통해 각 변수들과 제왕절개분만간의 관계를 분석하고 비교하였으며, 로짓분석을 통해 제왕절개분만 결정에 영향을 미치는 요인을 다각적으로 분석하고 각 독립변수와 제왕절개분만간의 통계적인 유의성을 검증하였다.연구결과는 다음과 같았다.제왕절개분만 결정요인을 로짓분석을 통해 분석한 결과, 남편 교육수준, 남편 연령, 산전관리회수가 제왕절개분만에 통계적으로 유의한 영향을 미치는 것으로 나타났다. 단변량 분석에서는 남편 교육수준, 남편 연령, 산전관리회수가 제왕절개분만과 통계적인 유의성을 가지고 있었고, 독립변수간에 다중공선성(multicollinearity)을 검증한 stepwise 방법이 사용된 다변량 분석에서도 남편 교육수준, 남편 연령, 산전관리회수가 제왕절개분만과 통계적으로 유의한 관련성을 가지고 있는 것으로 분석되었다.남편 교육수준이 고등학교 이하인 경우보다 대학교 이상일 경우 제왕절개분만을 할 확률이 높았는데 이는 남편 교육수준이 제왕절개분만을 결정하는 주요한 요인으로 작용하고 있음을 보여주는 것이다.또한 남편 연령이 29세 이하인 경우보다 35~39세인 경우 제왕절개분만을 할 확률이 낮은 경향을 볼 수 있었고 이는 남편 연령이 증가할수록 제왕절개분만을 하게 될 확률이 높아진다고 한 기존의 연구결과와 차이가 있는 것이다. 따라서 남편의 특성이 제왕절개분만과 어떠한 관련성을 갖는지에 관해 알아보기 위하여 남편 관련 특성 변수를 다양하게 포함하여 제왕절개분만과의 관계를 분석하는 연구가 필요할 것으로 생각된다.그리고 산전진찰회수가 많아질수록 제왕절개분만을 할 확률이 더 높아지는 것으로 나타났고 이는 기존의 연구결과와 일치하는 것이다. 이것은 산과적 특성이 제왕절개분만에 중요한 결정요인으로 작용했음을 고려하지만, 과다한 의료 공급이 오히려 수요를 창출해낸 측면이 있으리라 생각되며 의료공급을 적정화하여 제왕절개분만을 감소시키는 정책이 필요할 것으로 본다.본 연구를 통하여 불필요한 제왕절개분만을 줄이고 자연분만을 증가시키고자 하는 움직임이 필요하다는 생각에서 제왕절개분만을 결정하는 요인이 무엇인지 파악하고자 하였다. 사회·경제적, 인구적, 임신·출산 관련 측면에서 제왕절개분만 결정요인에 대해 알아보았는데, 향후 분만에 대한 신념이나 태도와 같은 인지적(cognitive) 측면에 대한 연구를 통해 자연분만을 증가시키는 방안을 모색하는 것도 필요하리라 생각되며, 남편 특성 변수들과 제왕절개분만의 인과관계를 잘 파악하여 설명할 수 있는 체계적인 추후연구가 필요하다고 생각한다.

[영문]Cesarean section rate is increasing tendency in the world for 20 years. Especially this current is showing no sign of abating in korea from the 1990s, so that health problems and socioeconomic effect has become social issues. And it is needed to investigate more concrete and integrated study on the factors determining cesarean delivery.The purpose of this study was to examine the factors of determining cesarean delivery concerning the impact of socioeconomic and demographic status, obstetrical characteristics by investigating the previous article and testing the major hypothesis.This Study is based on data from Year 2003 Korea National Fertility Survey carried by Korea Institute of Health and Social Affairs. In particular, the last babies born from January, 2001 to April, 2003 and the mothers were analysed as the main data for this study.Descriptive analysis showed general characteristics of mothers and babies. Hypotheses were tested by the statistic significance using logit analysis for the factors determining the cesarean delivery.The results of this study can be summarized as follows;Husband's education level, husband's age, number of preterm care were significant by univariate analysis. And all of this variables were significant in stepwise method by multivariate analysis in case controlled to the other factors.In case husband's education level is above university, the probability of cesarean delivery is high compared to under high school in socioeconomic characteristics. And in case husband's age is 35~39, it has little probability compared to under 29 in demographic characteristics. Finally the higher number of preterm care in obstetrical characteristics, the higher the cesarean delivery.We tried to research what are the determinants on the cesarean section delivery. We analysed in aspects of the socioeconomic characteristics, demographic characteristics, obstetrical characteristics, so we need the further research in cognitive characteristics such as an attitude and belief to find a strategies for encouraging mothers to delivery naturally. And this analysis suggests the necessities of systemic research to explain relation of cause and effect among husband's characteristics explaining the factors of determining cesarean delivery.
