[한글]

전이성 뇌 종양에 대한 신경외과영역에서의 치료 방법으로는 방사선 치료, 수술적 제거술, 방사선수술, 항암치료 및 각각의 조합이 있으며, 병의 진행 정도, 병변의 크기, 환자의 임상양상, 나이, 원발 병소 암의 특성, 타 장기로의 전이여부, 여명 등 여러 가지 요인으로 인해 치료의 방침이 아직까지는 확립되어 있지 못한 부분들이 있다. 이중 방사선 수술은 현재까지는 모든 전이성 뇌 병변이 대상이 되고 있지는 않으며, 일반적으로 병변의 직경이 3cm미만으로 크기가 작거나 병변의 갯수가 3개 이하의 경우, 분할 방사선 치료에 치료가 잘되지 않는 종양, 그리고 Karnofsky performance scale이 70 이상인 환자에게 적용되고 있다.저자는 전이성 뇌 종양에 있어 지름이 3cm이상이었던, 병변의 크기가 큰 환자들에게도 방사선 수술이 유용한지, 또한 일차적 치료 방법선택에 있어 방사선 수술이 가능할지에 대하여 알아보고자 하였다.연구대상은 전이성 뇌 종양에 대하여 감마나이프 방사선 수술을 시행 받은 304명중 종양의 최대 직경 크기가 3cm이상인 환자이며, 수술 후 남은 병소 또는 이전에 전 뇌 방사선 치료를 받은 환자는 제외 하였고, 최소 한 번 이상의 추적 진단방사선 검사가 시행되었던 환자 20명을 대상으로 종양의 반응, 크기의 변화, 종양 주변 부종의 변화, 종양의 조절과 종양 반응에 미치는 여러 인자들을 조사하였다.전이성 뇌 종양의 평균 크기는 3.7cm(범위; 3.1~4.4cm)였고, 경계선량은 평균 14.67Gy(범위; 10~19Gy)였으며, 등선량 선은 50%였다. 감마나이프 방사선 수술에 반응을 보인 전이성 뇌 종양은 40%였고, 감소는 55%, 종양의 조절은 65%, 부종의 감소는 70%였다. 종양 반응에 영향을 미치는 인자 중 직경(p=0.083)과 두개강외 전이성 병변의 유무(p=0.103)가 낮게 나왔으나 통계적 유의성은 없었다. 방사선 수술 이후 생존 분석은 12개월째 65%의 생존을 나타내었다.크기가 3cm이상인 전이성 뇌 종양의 경우 일차적으로 개두술을 통한 종양제거술을 시행하여야 하나, 그렇지 못하는 선택적인 경우에 한하여 방사선 수술을 일차적인 치료 방법으로 고려할 수 있을 것이다.



[영문]The metastatic brain tumors are treated with whole brain radiation therapy, operation, radiosurgery, chemotherapy and its combination in the neurosurgical area. Current management of brain metastases focuses on individualizing patient care on the basis of known prognostic factors; status of systemic disease, the patient performance status and age, as well as the tumor number, location, size, and histology. Among these therapeutic modalities, radiosurgery is often used for the size of mass less than 3cm, number of mass less than 3, KPS more than 70, and poor response tumor to fractionated radiation therapy, such as melanoma, renal cell cancer.We discuss about the effectiveness of gamma knife radiosurgery (GKS) on metastatic brain which is more then 3cm in diameter, and the possibility of selecting radiosurgery as a first treatment of these cases.A total of 20 patients with metastatic brain tumor were included in this study, and we excluded surgical remnant or previous irradiated lesions. Mean diameter of the tumor was 3.7cm (range; 3.1~4.4cm), mean marginal dose 14.67Gy (range; 10~19Gy) with 50% isodose line. The tumor response to GKS was 40%, decrease in size 55%, tumor control rate 65%, and the reduction of peritumoral edema 70%. But there were no significant variables between tumor response and others. The overall survival rate was 65%/12months.In fact, the large size metastatic brain tumor must be treated with surgical way, but in selected case, GKS could take place as a first line of treatment modality.
