내이 Mondini 기형 환자의 인공와우 이식술 후 청력 결과 분석

Abstract

[한글]내이Mondini 기형을 가진 난청 환자의 인공 와우 이식 후 결과는 다양하게 보고되고 있으며, 수술 술기가 어려우며, 청력 결과에 대한 예측이 힘들어 술 후 관리가 힘들다. 본 연구에서는 Mondini 기형을 가진 인공와우 이식 환자군의 수술소견을 후향적으로 조사하고, 순음 청력검사 및 개방형 단음절 한국어 어음검사를 시행하여, 술 후 청력결과를 분석하였다.신촌 세브란스 병원 이비인후과서 2000년1월부터 2005년7월까지 인공와우 이식술을 시행한 181명의 환자 중, 방사선적으로 확인된Mondini 기형이 있는 8명을 대상으로 선정하였다.후향적인 수술소견 조사에서, 외림프액의 분출이 2예 있었으나 수술 중 근막이나 근육조각을 이용하여 적절히 처리되었다. 전극을 완전히 넣지 못한 경우가 2예 있었으며, 수술 후 안면신경마비나 뇌막염 등의 합병증은 없었다.순음 청력검사에서, 모든 환자의 순음청력이 향상되어31데시벨 이상 43데시벨 이하의 순음역치를 보였고, 개방형 단음절 한국어 어음 검사상, 언어습득전 환자 1명을 제외한 모든 환자에서30 에서 40%의 어음변별력을 얻었다.내이 Mondini 기형을 지닌 인공와우 이식 환자군에서도 단순한 전정수도관 확장이나 불완전 구획만을 지닌 환자군처럼 좋은 순음 청력과 어음변별력의 향상을 보였으므로, 이들 환자군에서 인공와우 삽입술은 만족할 만한 효과를 기대할 수 있어, 인공와우를 적극적으로 시행하는 치료전략이 요구되겠다.

[영문]Cochlear implantation is not readily recommended to patients with Mondini malformation because postoperative hearing results are variable and operative techniques are difficult. To assess the audiologic and surgical outcomes for cochlear implant patients with Mondini malformation, we retrospectively reviewed the medical charts of eight patients who were determined as having Mondini malformation on computed tomography or temporal magnetic resonance imaging and underwent cochlear implantation at Severance Hospital of Yonsei University from 2000 to 2005.Perilymph gush-out through the cochleostomy was encountered in 2 of 8 patients. All leaks were controlled intraoperatively with temporalis muscle plugs or fascia with fibrin glue. There were no cases of postoperative facial nerve palsy, meningitis or revision surgery. Incomplete electrode insertion occurred in 2 patients. Comparison of pre- and postoperative pure tone audiograms showed that the hearing threshold was improved to 31 dB to 43dB in all patients who had profound hearing loss before cochlear implantation. In open-set speech perception tests, all patients received a speech disrimination score of 30 to 40% except one prelingual deaf patient. Considering these results, cochlear implantation can be more widely recommended in patients with Mondini malformation.
