직장암에서 수술전 경직장 초음파의 정확도에 관한 연구

Abstract

[한글]직장암 환자에서 수술 전 정확한 병기 설정을 위해 여러가지 검사를 시행하게 된다. 이 중에서 경직장 초음파는 종양의 국소 침윤 깊이 및 주위 조직의 침윤 여부, 직장 주위 림프절의 비대를 평가함에 있어 다른 방사선학적 검사들에 비해 높은 정확성을 나타내고 있다. 본 연구는 1996년 1월부터 2005년 5월까지 직장암으로 수술받았던 환자 374명 중 충분한 정보를 얻을 수 있는 57명을 대상으로 수술전 시행한 경직장 초음파의 정확도를 살펴보고, 이의 임상적 의의에 대해 살펴보고자 하였다.대상환자 57명은 남자 36명, 여자 21명이었고, 이들의 평균나이는 58.5세였다. 수술법에 따라 저위전방절제술을 시행받은 환자가 14명, 저위전방절제술 및 대장항문문합술 22명, 복회음절제술 6명, 경직장절제술 14명, 하트만식 수술 1명이었고, 전체 환자 중 수술전 항암방사선치료를 받은 환자가 23 명, 바로 수술을 시행받은 환자가 34 명이었다. 이들에서 수술법에 따른 수술전 경직장 초음파의 정확성을 살펴보면 저위전방절제술을 시행받은 환자에서 T stage 85.7%,N stage 78.5%의 정확성을 보였고, 저위전방절제술 및 대장항문 문합술을 시행받은 환자에서는 T stage, N stage모두 68.2%의 정확성을 보였으며, 경직장 절제술을 시행받은 환자에서는 T stage 64.2%, N stage 100%의 정확성을 알 수 있었다.이전 문헌들과 비교하여 살펴보았을 때, 본 연구기관에서 시행한 직장암 환자에서의 수술전 경직장 초음파의 정확성은 다른 보고들과 큰 차이가 없었다. 또한, 초음파 검사를 통하여 국소침윤 및 항문주위 림프절 전이여부에 대한 정확한 평가를 통해 수술전 항암 방사선 치료여부를 결정하고, 합리적인 수술방법을 결정하는데 도움이 될 수 있다.

[영문]In the rectal cancer, various examinations might be performed for the correct staging. Transrectal ultrasonography is highly accurate for the evaluation of local invasion depth, invasion to the adjacent organ and enlargement of perirectal lymph node. Of the 374 patients with rectal cancer examined by TRUS(Transrectal ultrasonography), 57 that had sufficient informations were included. In these, the accuracy of preoperative transrectal ultrasonography and clinical meanings were inspected.In 57 patients, low anterior resection was done in 14 patients, low anterior resection with coloanal anastomosis in 22, abdominoperineal resection in 6, transanal excision in 14, Hartman’s operation in 1. And preoperative neoadjuvant chemoradiotherapy was done in 23 patients.In patient with low anterior resection, accuracy of T stage was 85.7%, N stage was 78.5%. In low anterior resection with coloanal anastomosis, accuracy of T stage and N stage was 68.2% each and in group with transanal excision accuracy of T stage was 64.2%, N stage was 100%. This result in my institute was similar to the other articles.So through TRUS, more informations were provided about the invasion of depth and enlargement of perirectal lymph node. These would be helpful for decision of preoperative chemotherapy and appropriate operative plan.
