성인 아토피 피부염 환자에서 Narrow-band Ultraviolet B 치료 효과

아토피 피부염은 유병율이 증가 추세에 있는 염증성 피부 질환의 하나로서, 면역 억제제나 부신 피질 호르몬제, 그리고 보습제의 사용이 현재로서는 치료의 주류를 이룬다. 기존의 국소 부신 피질 호르몬제 치료는 장기간 적용 시 피부 위축과 혈관 확장, 호르몬 불균형, 성장 저해 등 다양한 부작용을 동반할 수 있어 대체할만한 치료법이 필요하다. 건선과 백반증 치료에 최근 사용되고 있는 narrow-band ultraviolet B (NBUVB)는 311 nm 전후의 좁은 파장대의 자외선으로 별도의 광과민성 유도제의 복용 없이 치료에 이용되며, 최근 아토피 피부염에서도 그 효과가 보고되고 있다. 본 연구에서는 중등도 이상의 15세 이상 성인 아토피 피부염 환자에서 12주간 NBUVB 광선 치료를 시행하고 효과를 관찰하였다. 임상적인 효과는 환자 자신의 평가와 Eczema Area and Severity Index (EASI)를 통한 의사의 평가, 그리고 육안 사진 촬영으로 확인하였다. 검사실 소견은 치료 전후의 혈청 내 eosinophilic cationic protein (ECP)과 E-selectin의 농도, 그리고 호산구 분획 등을 비교하였다. 치료 후 대부분의 환자에서 유의하게 임상적 호전이 있었으며, 검사실 소견도 개선되었고 심각한 부작용은 없었다. 특히 E-selectin의 혈중 농도와 호산구 분획에서 유의하게 감소하였다. 본 연구결과 성인 아토피 피부염 환자에서 NBUVB치료가 효과적인 것으로 생각된다.

[영문]The increasing incidence of atopic dermatitis calls for attention on new therapeutic modalities such as phototherapy. Adverse outcomes following the continuous uses of corticosteroid and immunosuppressive agents could be avoided with the narrow-band ultraviolet B phototherapy. We have conducted an uncontrolled study on 24 adult atopic dermatitis patients with moderate to severe disease. Laboratory data including eosinophil fraction, serum levels of eosinophilic cationic protein and soluble E-selectin that support the atopic inflammation were compared before and after the 12 week treatment session, as well as the clinical parameters assessed by eczema area severity index and patient questionnaires. Patients showed clinical improvement and serum eosinophilic fraction and E-selectin level showed significant decrements following the phototherapy. The improvements evidenced by clinical and laboratory data seem to be promising.
