관절경을 이용한 견관절 충돌 증후군의 치료

1990년 7월부터 1994년 6월까지 연세대학교 의과대학 정형외과에서 견관절 충돌 증후군으로 진단받고 관절경을 이용하여 치료받은 48명의 환자 50례를 대상으로 평균 2년 11개월간 (1년 6개월부터 4년 9개월까지) 추시 관찰 후 치료 결과를 분석하여 다음과 같은 결론을 얻었다.

1. 환자의 성별로는 남자가 33명 (35례), 여자가 15명 (15례)이었고 평균 연령은 남자가 37세, 여자가 47세였다.

2. Neer의 분류법에 따라 분류한 결과 21례가 견관절 회전 근개의 파열이 없는 제 2기로 분류되었고 29례가 견관절 회전 근개의 파열 소견이 있는 제 3기로 분류되었다.

3. 제 2기 병변 21례를 1군으로 하고 제 3기 병변중 회전 근개의 부분파열이 있었던 15례를 제 2군으로, 제 3기 병변중 회전 근개의 전충을 침범하는 완전파열의 14례를 제 3군으로 분류하여 치료결과를 분석한 결과 관절경적 감압술 및 변연절제술은 회전근개의 파열 여부및 그 정도에 관계없이 견관절충돌 증후군의 치료에 효과적이었다.

4. 관절경을 이용한 치료의 만족도는 각 군별로 통계학적으로 의의 있는 차이가 나타나, 회전근개의 파열이 없거나 정도가 경미한 경우가 만족도가 높았다.

From July 1990 to June 1994, the arthroscopic decompression and cuff debridement were performed on 50 cases in 48 consecutive patients with either stage Ⅱ or stage Ⅲ impingement syndrome, and the patients were followed up for an average 2 years and Ⅱ months( from 1 year 6 months to 4 years 9 Months). The purpose of this stuty was to evaluate the effectiveness of arthroscopic subacromial decompression and debridement in the treatment of shoulder impingement syndrome. The results are as follows.

1. There were 33 males(35 cases) and 15 females (15 cases) and the mean age was 37 years for man and 47 years for woman.

2. According to Neer classification, 21 cages were classified as stage Ⅱ and 29 cases were classified as stage Ⅲ.

3. Among 50 cases, 21 cases who had no cuff tear were categorized as group 1, and 15 cases who had partial thickness cuff tear were categorized as group 2, and 14 cases who were found to have complete tear on cuff were categorized as group 3. The effectiveness of the treatment and the difference of the satisfactory results were evaluated with statistical method among the groups.

4. The arthroscopic subacromial decompression and rotator cuff debridement were a good treatment method in all the three groups, however the satisfactory results were less likely in patients with cuff tears.
