편마비환자의 견갑골 위치 측정에 대한 연구

dc.description.abstract[한글] 연세대학교 의과대학부속 세브란스병원 재활의학과에 1989년 5월 1일부터 1990년 1월 30일 까지 입원치료를 받고 있던 편마비 환자 중 본 연구의 필요조건을 충족 시키는 편마비 환자 35명을 대상으로 환측 견갑골과 건측 견갑골의 위치를 X-ray 촬영과 피부표면위 측정을 하여 환측 견갑골의 위치와 피부표면위 측정의 임상적 이용성을 알아보기 위하여 본 연구를 시행하였다. X-ray 측정결과와 피부표면위 측정 결과를 비교하기 위하여 짝 비교 t-검정을 시행하였다. 본 연구를 통해서 다음과 같은 결과를 얻었다. 1. 연구대상환자 35명을 흉추극돌기에서 견갑골극 내측연 삼각지점까지 거리를 X-ray 촬영 및 피부표면위 측정을 하여 확측과 건측을 비교한 결과 환측이 건측에 비해서 내전되어 있었다. 2. 연구대상환자를 흉추극돌기에서 환측과 건측견갑골의 견갑골극 내측연 삼각지점까지 거리를 X-ray 촬영과 피부표면위 측정을 하여 측정결과를 비교하였을 때 X-ray 측정치와 피부표면위 측정치는 유의한 차이가 없었다(P>0.01). 3. 연구대상환자를 견갑골극 내측연 삼각지점과 견갑골하각에서 환측과 건측견갑골의 높낮이를 X-ray 촬영과 피부표면위 측정을 하여 측정 결과를 비교하였을 때 X-ray 측정치와 피부표면위 측정치는 유의한 차이가 없었다(P>0.01). 이상의 결과로 보아 편마비 환자의 견갑골의 위치를 측정하고자 할 때는 X-ray 촬영을 하지 않더라도 측정자를 이용하여 피부 표면위에서 임상적으로 쉽게 측정 할 수 있으며 따라서 편마비 환자의 견갑골의 운동에 관련되는 근력과 생리학적 기능의 평가를 좀 더 쉽게 할 수 있고 상지 재활치료를 효과적으로 시행할 수 있게 된다고 생각된다. [영문] The purpose of this study was to determine if any significant difference existed in the results of measurements taken by X-ray and those carried out by simple measurement by a straight ruler and digital skin palpation of the scapulae in patients with hemiplegia. The subjects of the study were 35 patients, 22 male and 13 female, with an average age of 49.8 years (ranging from 24 to 75) with hemiplegia who received rehabilitation on an inpatient basis at Yonsei University Medical College Rehabilitation Center from May 1, 1989 to Jan 30, 1990. All measurements were carried out with the subjects in the sitting position. Three measurement criteria were used. The first was the distance between a point at the medial limit of the scapular spine and the adjacent spinous process which both manually palpable and visible on X-ray. The second was the relative levels of the inferior angle of the scapula and third, the scapular spine-points, with the levels on the unaffected side taken as the standard. All measurements were done on both the affected and non-affected sides. The results were as follows : 1. There was no statistically significant difference between the X-ray and digital/ruler measurements of the distance between the point at the medial end of the scapular spine and the adjacent spinous process(P>0.01). However, The average distance from scapula spine to adjacent spinous process was less on the affected side, indicating adduction of the scapula on the affected side. 2. There was no statistically significant difference between 1) the X-ray and digital/ruler measurements of the level of the point at the medial end of the scapular spine 2) the x-ray and digital/ruler measurements of the level of the inferior angle of the scapula(P>0.01). However, the scapula on the affected side was depressed in 29 cases (82.9%) and elevated in 6 cases(17.1%). It can be concluded that the use of an ordinary ruler plus palpation is a useful and inexpensive clinical method measuring scapular location in hemiplegia. This can be a helpful tool in the assessment of the hemiplegic shoulder and objective measurement of treatment progress.-
