시립병원 위탁운영의 경영성과 분석

시립병원을 제3차 진료병원에 위탁운영한후 경영성과를 평가하기 위하여 1987년말 부터 서울대학교병원이 서울시와의 위탁계약하에 운영중인 서울특별시립 영등포병원과 서울특별시가 계속 운영하고 있는 서울특별시립 동부병원을 비교병원으로 선정하여 위탁전, 후의 3년간의 경영성과를 비교, 분석하여 다음과 같은 결과를 얻었다.

첫째, 재무상태에 있어서는

가. 당기순이익은 아직도 적자를 기록하고 있으나, 위탁운영후 재정자립도가 꾸준한 향상을 보이고 있다. 또한 수익성도 비교병원에 비해 위탁운영후 향상되고 있다.

나. 위탁운영후 총자본회전율이 상승하여 자본의 효용성이 재고되었다.

다. 성장성지표인 의료수익증가율, 총자본 증가율 및 환자수 증가율이 모두 상승하다가 감소추세를 보이고 있어 병원의 규모가 커졌음을 나타내고 위탁운영에 의한 개선효과가 3년내에 거의 다 이루어진 것으로 보인다.

둘째, 환자진료측면에서

가. 위탁운영후 환자의 진료실적이 증가하였으며, 특히 입원환자 증가에 따라 병상이동율은 급등하였다.

나. 환자구성비는 일반환자는 감소한 반면, 의료보험환자 및 의료보호환자는 증가하였다. 전체환자수의 증가에도 불구하고 의료보호환자 구성비가 증가 함으로서 위탁운영을 해도 공공기능이 감소하지 않았다.

셋째, 생산성, 의사1인당 환자수, 직원1인당 환자수가 모두 증가되어 노동생산성이 향상되었다.

넷째, 의료서비스의 질은 제3차 진료병원의 수준으로 향상된 것으로 나타났으나 조직의 비대 및 인건비부담의 증가등 위탁운영의 문제점도 노출되었다.

결국, 제3차 진료병원이 시립병원을 위탁운영함으로써 재무상태의 개선과 환자진료실적 및 노동생산성의 향상이라는 긍정적인 결과를 가져왔고, 공공기능도 위축되지 않아 성공적이라 평가할 수 있다.

Seoul National University Hospital has been entrusted with full responsibility for running the Youngdeungpo city Hospital since 1987.

This study was designed to evaluate the effectiveness of entrusting the city hospitar management to the tertiary care hospital using a quasi experimental design. Dongbu City Hospital was chosen as a comparison group because it is the only city hospital in Seoul which is run by its own management group.

Results were as for lows:

1. With respect to the financial condition;

a. Both the profitability and the self supporting rate have contineously improved after the consignment although the net income still showed a deficit.

b. The overall capital turnover has improved with the more total asset turnover rate.

c. Indicators such as total revenue, total asset and patient number were also showed remarkable increase during the first two years after the consignment.

2. With respect to the performance of patient care:

a. A total number of patient treated increased after the intervention. Especially an increase in the number of admissions was outstanding. As a result, a bed utilization rate also increased rapidly.

b. The proportion of the Medical Aid Patients among the total patients treated increased. It reflects that the consignment did not curl up the public function of the city hospital.

c. Productivities in terms of a number of patients per physician and a number of patients per employee improved after the commission.

d. The quality of medical care improved up to the similar level of the tertiary care hospital.

These results provided the evidence that entrusting a city hospital management to a tertiary care hospital has positive effects not only on financial status but also on patient cares.
