근거리검영법 조절마비후검영법 및 자각적 굴절검사로 측정한 굴절이상의 비교관찰

시력장애를 호소하는 환자중 안과학적 검사를 실시하여 시력장애를 가져올만한 안질환이 없으면 굴절이상을 의심해야한다. 굴절이상을 측정하는 방법에는 여러가지가 있다. 이에 저자는 1986년 5월 1일부터 1986년 9월 30일까지 연세대학교 의과대학 부속 세브란스병원 안과 외래에 내원한 환자중 굴절이상이 있는 1세부터 10세이하(Ⅰ군) 40명(80안), 11세부터 20세이하(Ⅱ군) 48명(96안), 21세부터 30세이하(Ⅲ군) 34명(68안)을 대상으로 동일인에서 근거리검영법 조절마비후검영법 및 자각적굴절검사를 시행하여 측정한 구면굴절

력(spherical power)과 원주굴절력(cylindrical power)를 분석하여 다음과 같은 결과를 얻었다.

1. 조절마비후검영법과 자각적굴절검사에 의하여 측정한 구면굴절력의 차이는 0.59D.였으며 근시안군에서는 0.49D., 원시안군에서는 0.76D.였으며 근시안군과 원시안군간의 차이는 통계학적으로 유의하였다(P<0.05). 연령군별로 보면 Ⅰ군은 0.59D., Ⅱ군은 0.64D., Ⅲ군은 0.45D.였으며 연령군간의 차이는 통계학적인 유의성이 없었다(P>0.05).

2. 조절마비후검영법과 자각적굴절검사에 의하여 측정한 원주굴절력의 차이는 0.07D.였으며 연령군별로는 Ⅰ군은 0.03D., Ⅱ군은 0.12D., Ⅲ군은 0.07D.였으며 연령군간의 차이는 통계학적인 유의성이 없었다(P>0.05).

3. 근거리검영법과 자각적굴절검사에 의하여 측정한 구면굴절력의 차이는 0.97D.였으며 근시안군에서는 0.92D., 원시안군에서는 1.05D.였으며 근시안군과 원시안군간의 차이는 통계학적인 유의성이 없었다(P>0.05). 연령군별로 보면 Ⅰ군은 1.03D., Ⅱ군은 0.94D., Ⅲ군은 0.92D.였으며 연령군간의 차이는 통계학적인 유의성이 없었다(P>0.05).

4. 근거리검영법과 자각적굴절검사에 의하여 측정한 원주굴절력의 차이는 0.11D.였으며 연령군별로는 Ⅰ군은 0.18D., Ⅱ군은 0.11D., Ⅲ군은 0.04D.였으며 연령군간의 차이는 통계학적인 유의성이 없었다(P>0.05).

5. 근거리검영법에 의하여 측정한 구면굴절력과 조절마비후검영법에 의하여 측정한 구면굴절력과의 상관계수 r=+0.82였으며 회귀방정식은 Y=0.877 + 0.724 X(X : 근거리검영법에 의하여 측정한 구면굴절력에 조정요인을 합친 값 Y : 조절마비후검영법에 의하여 측정한 구면굴절력)이였다.

The object of this study was to observe the refractive errors obtained by near retinoscopy, retinoscopy under cycloplegia and subjective refraction on 244 Korean eyes with refractive error below the age of 30.

The results were as follows

A. Comparision between the refractive errors obtained by retinoscopy under cycloplegia and subjective refraction

1. The difference of spherical powers was 0.59D.

2. The difference of spherical powers was 0.49D in myopia and 0.76D in hyperopia.

3. The difference of spherical powers was 0.59D in Group Ⅰ(under age 10), 0.64D in Group Ⅱ(age 11-20) and 0.45D in Group Ⅲ(age 21-30).

4. There was not statistically significant difference of spherical powers among the age groups but between myopia and hyperopia.

5. The difference of cylindrical powers was 0.07D.

6. The difference of cylindrical powers was 0.03D in Group Ⅰ, 0.12D in Group Ⅱ and 0.07D in Group Ⅲ.

7. There was no statistically significant difference of cylindrical powers among the age groups.

B. Comparison between the refractive errors obtained by near retinoscopy and subjective refraction

1. The difference of spherical powers was 0.97D.

2. The difference of spherical powers was 0.92D in myopia, and 1.05D in hyperopia.

3. The difference of spherical power was 1.03D in Group Ⅰ, 0.94D in Group Ⅱ and 0.92D in Group Ⅲ.

4. There was neither statistically significant difference of spherical powers among the age groups nor between myopia and hyperopia.

5. The difference of cylindrical powers was 0.11D

6. The difference of cylindrical powers was 0.18D in Group Ⅰ, 0.11D in Group Ⅱ and 0.04D in Group Ⅲ.

7. There was no statistically significant difference of cylindrical powers among the groups.

C. In this study, regression formula was Y=0.877 + 0.724 X(X: adjusted spherical power obtained by near retinoscopy Y: spherical power obtained by retinoscopy under cycloplegia)
