Rapid palatal expansion appliance를 이용한 Angle씨 제Ⅲ급 부정교합 환자의 상하악골의 변화에 대한 두부 방사선 계측학적 연구

연세대학교 치과대학 부속병원 교정과에 내원한 Angle씨 제Ⅲ급 부정교합 환자중 상악골이 양측성으로 협소하고 절치부에 반대교합이 있는 환자에서, Rapid palatal expansion appliance를 이용하여 정중 구개봉합을 급속 확장시켜 치료한 환자 18명에서 Rapid palatal expansion appliance의 장착 전과 장착 후에 상하악골의 위치적인 변화를 두부방사선 계측학적으로 분석하기 위하여 28개의 계측 항목을 선정하여 이 항목별 측정치의 변화값을 구하고, 이 변화값이 평균치, 표준오차, 표준편차를 구하고 이 변화값을 T 검정하여 각 항목별의 유의성 여부를 p<0.05와 p<0.01 수준에서 조사하여 다음과 같은 결론을 얻었다.

1. point A가 전하방으로 이동하였다.

2. palatal plane의 ANS는 하방으로 이동하였지만, PNS의 이동은 변화가 다양하였다.

3. 상악 전치와 SN plane이 이루는 각과 상악 전치에서 Facial plane까지의 거리가 증가하였다.

4. 대부분의 환자에서 SN-MP각이 증가하였다.

5. 하악골은 후하방으로 전위 되었다.

The purpose of this investigation was to study the change of maxillary and mandibular position after mid palatal suture opening.

The 18 patients selected for study were taken from the orthodontic department of the Infirmary in Dental College of Yon-sei University.

The patients had bilateral maxillary constriction, anterior cross-bite and Angle's class Ⅲ malocclusion and treated by rapid palatal expansion.

Lateral cephalograms were taken before treatment and at the completion of maxillary suture opening.

Cephalometric analysis and comparision were done in 28 measurements.

The variations, means, standard errors and standard eviations were obtained and T-tested were done at p<0.05 and p<0.01 level in 28 miasurements.

Average response to midpalatal Suture opening revealed the following results in this study;

1. Maxillary expansion was affective in all patilnts.

2. The point A moved downward and forward after Completion of suture opening.

3. The ANS in palatal plane moved downward but the PNS was variable.

4. 1 to SN angle and 1 to Facial plane distance were increased.

5. In most of all patients, SN-MP angle was incrased.

6. Mandible was rotated to downward and backward.
