한 종합병원 입원환자의 재원기간에 관련되는 요인분석

Abstract

[한글]1977년부터 시작된 의료보험으로 미충족 의료수요(unmet health need)가 유효수요로 전

환되면서 필요한 날에, 원하는 병원의 병실에 입원하기가 어려워졌다. 이러한 현상에는

여러가지 원인이 있겠지만 환자의 재원기간과 관계가 있는 바 본 연구에서는 재원기간과

관계되는 여러 변수들을 찾아 분석함으로써 그 요인들을 찾아 보고자 하였다.

1965년 1월부터 1980년 12월까지, 매 5년 간격으로 4개년, 즉1965년, 1970년, 1975년,

1980년을 선택하여 서울시 내에 위치한 한 종합병원에서 충수돌기 절제술, 담낭절제술 및

재왕절개술을 받은 환자 1,003명을 대상으로 하였다. 연도별로 여러 요인들의 평균 재원

일수를 보기 위해서 회귀분석 및 분산분석, X**2-test를 이용 분석하였다.

조사 결과는 다음과 같다.

1. 연도별 평균 재원일수는 충수돌기 절제술을 받은 환자에서는 변화가 없었으며 담낭

절제술과 제왕절개술 환자에서는 감소하는 경향을 보였다.

2. 충수돌기 절제술 환자의 재원기간에 영향을 미치는 변수는 합병증 여부, 연령, 수술

요일, 타질병 동반여부, 의료보험 가입여부의 순서였다.

3. 담낭절제술 환자의 재원기간에 영향을 미치는 변수는 합병증 여부, 수술요일, 연령,

병실등급, 입원경로의 순서였다.

4 .제왕절개술 환자의 재원기간에 영향을 미치는 변수는 합병증 여부, 병실등급, 타질

병 동반여부, 거주지역, 수술요일의 순서였다.

[영문]An Analysis on length of stag and its related factors in a University Hospital

In order to determine the factors which affect the length of stay, a total of

1,003 in-patients medical records for appendectomy, cholecystectomy, and Cesarean

section discharged in 1965, 1970, 1975 and 1980 from a university hospital were

studied.

The major findings were as follows;

The average length of stay showed no statistically significant difference by year

among the cholecystectomy and Cesarean section patients.

Complication after surgical operation explained the length of stay mostly in all

the diagnoses; next factors were age, operation day of the week, multiple

diagnosis, and health insurance for appendectomy; operation day of the week, age,

type of accommodation, admission route for cholecystectomy; and type of

accommodation, multiple diagnosis, place of residence, operation day of the week

for Cesarean section respectively.
