혈청내 뇨산치는 성별, 연령, 인종, 환경외에도 음식물, 약물 및 정신적 육체적 자극

등에도 영향을 받으며 류마치스성 관절염환자에서도 과뇨산혈증이 적지 않음이 보고되었

다.

비화농성인 이른바 "관절염" 환자에서 뇨산치를 측정, 과뇨산혈증을 검색해 냄으로써

통풍(痛風)환자를 확인할 수 있고 또한 통풍의 장래 발생을 예견하는데도 도움이 된다.

저자는 1974년 3월부터 1975년 5월까지 만 1년 3개월동안 연세대학교 의과대학 세브란

스 병원에서 가료받은 관절질환환자 100명을 대상으로 하여 혈청내 뇨산치를 측정하고 그

분포를 분석 검토한 결과 다음과 같은 결론을 얻었다.

1. 총 100명중 남자가 45명, 여자가 55명으로서 여자가 약간 더 많았다.

2. 관절질환 환자에서 뇨산치가 더 높다는 증거는 없으나 남자가 여자보다 더 높은 수

치를 보여 주었다. 즉 전체환자의 뇨산치의 평균 4.54 mg% 이었으며 남자가 5.70 mg%, 여

자가 3.60 mg% 였다.

3. 남자가 6 mg% 이상의 과뇨산혈증을 나타낸 환자의 수는 37.8 %의 높은 비율을 보였

고, 여자에서 5 mg% 이상을 나타낸 환자의 수는 12.7 %를 차지하였다.

4. 질환별로 보면 류마치스성 관절염환자에서는 10.7 %가 요통환자에서는 9.5 %가 6 mg

% 이상의 과뇨산혈증을 보였다.









[영문]

Galen still states that gout is rare amongst Asians and that it is more common

amongst beer drinkers and people whose living standards are high.

It is interesting to note, the recent estimate by Grayzel, that 17 % of normal

male subjects would be expected to have values above 6.0 mg% and 16 % of normal

female subjects values above 5.0 mg%.

In a recent study 30 % of 737 patients seen at a rheumatic-disease hospital were

found to have serum urate levels greater than 6 mg%.

There is very great confusion about normal values of serum uric acid. There is no

standardisation on an international basis. It is frequently stated that serum uric

acid values greater than 6.0 mg% in male subjects, or greater than 5.0 mg% in

female subjects, constitute hyperuricemia.

100 cases of joint disease, admitted to the Orthopedic Department of Severance

Hospital during the period from March 1974 to May 1975, were studied.

The serum uric acid determinations were undertaken in order to define the

distribution characteristics of serum uric acid in joint disease in Korea, and to

evaluate the importance of age and sex.

The results are summerized as follows:

1. Sex incidence showed male 45% and female 55%.

2. The prevalent age groups in joint disease were 40 to 49 years of age in male

patients and 50 to 59 years of age in female patients.

3. Serum uric acid levels in joint disease was not so high as compared with the

normal subjects in the foreign countries.

4. The mean concentration for 100 subjects of both sexes was 4.54±0.185 S.E.

mg%.

5. Male subjects, of whom there were 45, had serum uric acid values ranging from

3.2 to 11.9 mg% with an arithmetic mean of 5.70 mg% and a standard error of 0.27

mg%. Female subjects, 55 in all, had serum uric acid values ranging from 1.5 to 7.3

mg% with an arithmetic mean of 3.60 mg% and a standard error of 0.17 mg%.

6. The mean difference between mean values in male and female subjects was about

2.1 mg%.

7. 37.8% of all male subjects were found to have values of 6.0 mg% or greater.

12.7% of all female subjects were found to have values of 5.0 mg% or greater.
