dc.description.abstract[한글] Prostaglandin(PG)은 위장관계통을 비롯한 체내 여러조직에 널리 분포되어 있으며 위내홀몬과 상호작용을 하는 것으로 알려져 있다. PG의 위산분비 억제효과는 Robert등(1967)이 개에서 처음 보고하였으며 PG중 E 및 A계통 그리고 prostacyclin은 위산 기초분비 및 자극분비를 억제한다고 보고되어 있으며 이중 PGE^^2는 사람에서 뚜렷한 효과가 있음이 확인되었다. 또한 Robert등(1971)은 PG가 동물실험에서 인위적으로 형성된 궤양을 예방할 수 있다고 보고하고 이 작용을 세포보호 기능(cytoprotection)이라고 하였으며 비 스테로이드성 항염증제제나 알콜, 화상 및 고농도의 염산등에 의하여 유발된 위궤양이 PG중 A, E, F계통 제제 혹은 그의 methyl유도체 등의 전처치로 억제됨을 여러 실험에서 보고하고 있다. 그러나 Svanes등(1982)은 내인성 이든 외인성 이든 PG가 이러한 방어효과가 없다는 반론을 제기하고 있다. 한편 근래에 실험상 알콜을 쥐의 위내에 관류시 산분비가 억제된다는 보고들이 있는데 저농도인 10%이하에서는 아마도 PG가 관여되는것 같으며, 20%이상의 고농도에서는 H** +의 역확산으로 인한다는 주장들이 있다. 따라서 본 실험에서는 PG의 위산분비에 대한 영향과 나아가 세포보호기능 여부를 검토하고져 흰쥐에서 indomethacin 및 PGE^^2반응을 관찰하였으며 또한 알콜과의 상호관계를 검색하여 다음의 성적을 얻었다. 1. 생리적 식염수(pH 6.0)를 위내 관류시 위산분비는 3.6±0.16μEq/15min으로 일정하였으며 indomethacin투여로 위산분비 증가를, PGE^^2(200μg/kg/hr) 주입으로 현저한 위산분비 억압을 나타냈으나 소량(20μg/kg/hr)의 PGE^^2는 별영향이 없었다. 2. 알콜은 10%농도에서 위산분비를 심하게 억제하였으며 연속적으로 관류할 경우 계속적인 위산분비 억제를 볼 수 있었으며 알콜농도 20%이상에서는 H**+의 역확산을 나타냈다. 3. PGE**2 소량(20μg/kg/hr)을 전처치한 군의 알콜관류중 위산분비는 대조군보다 의의있게 억제되었으며 indomethacin 전처치군의 분비억제는 미약하고 산분비치는 대조군보다 높았다. 4. 알콜에 의한 위산분비 저하반응이 회복된 후 알콜을 재관류 하면 PGE^^2나 indometh acin 전처치와 상관없이 그 저하반응은 초기반응보다 약화되었다. 5. 알콜은 흰쥐에서 높은 위궤양 발생율을 나타냈으나 PGE^^2 전처치로 위궤양 발생은 현저히 억압되었고 indomethacin 전처치로는 위궤양 발생이 증가되며 위출혈을 나타내었다. 이상의 성적으로 PGE^^2는 알콜에 의한 위산분비 억제효과 내지 위산의 역확산을 증가시키고, 알콜에 의한 위궤양 발생을 억압하는 세포보호효과가 현저함을 알 수 있었다. [영문] Prostaglandins (PG's) are widely distributed throughout the gastrointestinal tract and have been known to inhibit gastric secretion. Furthermore, PG's are capable to protect the cells of gastrointestinal epithelium against a variety of potentially noxious agents apart from their antisecretory effect and it is termed as cytoprotection. Recently, it iS reported that ethanol, a possible ulcerogenic agents, inhibits the gastric acid secretion, and moreover, ethanol, higher than 20%, causes back diffusion of hydrogen ions into the gastric mucosa. Present study was undertaken to investigate the effects of prostaglandin on the gastric acid secretion and gastric ulceration in perfused rat stomach with or without alcohol and pylorus ligated rats. The albino rats, weighing about 180 g of either sex, were starved for 72 hours. Under urethane (1.5g/kg) anesthesia polyethylene tube was inserted into esophagus and pylorus for gastric perfusion. The physiologic saline, adjusted to pH 6.0, was used as standard perfusate and ethanol was diluted with saline. The 10% ethanol perfusate was adjust to pH 6.0, and hydrochloric acid was added to the concentration of 2 mM for 20% and 30% ethanol perfusate. The stomach was perfused with 4 ml oft he perfusate every 15 min and the acid content was titrated to pH 6.0 with N/20 NaOH by means of titrator TTT 2b. For ulceration study the pylorus was ligated after instillation of 20% ethanol(1.0 ml) or saline(1.0 ml) under light ether anesthesia, and 4 hours later the gastric secretion was measured and gastric lumen was examined for ulceration. The results obtained are as follows: 1. The gastric perfusion with physiologic saline (pH 6.0) in rat showed relatively constant acid secretion, 3.6±0.16 μEq/15 min, and indomethacin increased the acid secretion. The acid secretion was markedly decreased by PGE^^2 200 μg/kg/hr but 20 μg/kg/hr caused little change. 2. Gastric perfusion with 10% ethanol caused marked decrease of acid output and perfusion with alcohol in the concentration of 20% or higher evoked less of acid in the gastric lumen. 3. Prostaglandin E^^2 (20 μg/kg/hr) augmented the inhibition of acid secretion induced by ethanol but the acid output in indomethacin treated group was less inhibited by ethanol and showed higher acidity than control. 4. The inhibitory response of gastric acid secretion was less marked when ethanol perfusion was repeated after recovery of the first response of ethanol regardless of PGE^^2 or indomethacin treatments. 5. Ethanol caused the increase of ulceration but the acid output was decreased in pylorusligated rats. The ulcer formation was prevented by PGE^^2 and aggrevated by indomethacin pretreatment. The results show that prostaglandin inhibits acid secretion and promotes alcohol-induced back diffusion of gastric acid, and that it has protective effects on alcohol-induced gastric ulceration.-
