실험적 녹농균성 각막염에 대한 점안 항생제의 치료 효과

녹농균성각막염은 각막의 정상적인 방어기전이 저하되어 있는 사람의 각막, 찰과상을

입었거나 이물제거 후에 상처를 입은 각막, 부신피질 홀몬제를 장기간 점안했던 눈의 각

막 등에 Pseudomonas aeruginosa에 오염된contact lens, 안용액, 안연고, 안과기구나 flu

orescein 용액을 사용한 경우에 발생하게 된다.

녹농균성각막염이 일단 발생한 경우에 적절한 치료를 하지 않으면 48시간내에 각막천공

을 유발하므로, 빠른 진단과 치료가 요구되는 질환이며 그 치료는 주로 항생제에 의존한

다.

이에 저자는 녹농균을 가토 각막에 접종하여, 녹농균성각막염을 유발시키고, 점안 항생

제인 Chloramphenicol hydrochloride(Chloramphenicol(R)), Polymyxin B sulfate, Neomyc

in sulfate 과 Gramicidin 복합체(Polyspectran(R)), Tobramycin(Tobra(R)), Gentamicin

sulfate( Gentamicin(R)), Ofloxacin(Tarivid(R))를 점안하여, 각 항생제의 임상적 치료

효과,세균학적 치료 효과, 점안시 문제되는 안 자극도를 비교하여 다음과 같은 결론을 얻

었다.

임상적인 치료효과와 세균학적인 치료 효과에서는 Polymyxin B sulfate, Neomycin sulf

ate 과 Gramicidin 복합체 (Polyspeotran(R)), Tobramycin(Tobra(R)), Gentamicin sulfat

e(Gentamicin(R)), Ofloxacin (Tarivid(R)) 이 녹농균성각막염의 치료 효과가 있었으며,

이들 약제간 치료 효과의 차이는 없었다.

안자극정도는, Tobramycin(Tobra(R))과 생리 식염수군이 가장 적었고,Gentamicin sulfa

te(Gentamicin(R))과 Ofloxacin(Tarivid(R))가 중등도, Chloramphenicol hydrochloride(

Chloramphenicol(R)), Polymyxin B sulfate, Neomycin sulfate 과 Gramicidin 복합체 ( P

olyspectran(R))은 가장 자극이 컸다.

Recently, acute pseudomonas keratitis is reported to occur more frequently. If

treatment is not enough after the invasion of Pseudomonas into the cornea, corneal

perforation endues within 48 hours. Hence the rapid diagnosis and treatment is

required, and the treatment largely depends on the antibiotics.

Pseudomonas Keratitis was induced in the rabbit cornea by inoculating Pseudomonas

aeruginosa( IID 1210). Then the keratitis was treated with topical

Chloramphenicol(R), Polyspectran(R), Tobra(R), Gentamicin(R), Tarivid(R). The

effect of the antibiotics on Pseudomonas keratitis was evaluated clinically as well

as bacteriologically and irritation of the antibiotics against the eye was also

observed.

1. Polyspectran(R), tobra(R), gentamicin(R), tarivid(R) treated experimental

Pseudomonas keratitis in the rabbit eyes but chloramphenicol(R) and saline did not

inhibit the bacterial growth of the rabbit corneas.

2. Against the rabbit eyes, chloramphenicol(R), polyspectran(R) were the most

irritable antibiotics, where as tobra(R) and saline were the least. Gentamicin(R)

and tarivid(R) were the moderate in the extent of irritation.
