상악골 급속확장에 의한 Angle씨 제 III급 부정교합 환자의 안모형태 변화에 관한 두부방사선 계측학적 연구

저자는 상악골 발옥부전을 동반한 Angle씨 제Ⅲ급 부정교합 환자들 중에서 상악골이 양

측성으로 협소하고 전치부에 심한 반대교합이 있으며 안모의 불균형이 있는 환자 25명(남

:6, 여:19)을 대상으로 정중구개봉합선 급속확장 장치를 사용한 치료전·후의 경·연조직

을 두부방사선 계측학적으로 분석 비교한 결과 다음과 같은 결론을 얻었다.

1. ANS는 하방이동을 하였으며 Point A는 전하방으로 이동되면서SNA가 증가하였고 Poin

t B는 후하방으로 전위되면서 SNB가 감소하였다.

2. 상악골의 전하방 이동과 함께 많은 양의 하악골 후하방전위로 교합이 이개되면서 전

치부의 반대교합이 개선되었다.

3. 하악 경조직의 후하방 전위로 연조직의 facial convexity angle감소, Point B', Pog

'의 후하방 전위가 일어나면서 연조직도 같은 변화를 보였다.

4. 상순의 변화는 전방이동 보다는 하방이동이 유의성이 있었으며, 상악전치, 하악골

전위와 상관성이 높았다.

5. 하순의 변화는 후방이동 보다는 하방이동이 유의성이 있었으며 하악골 전위와 상관

성이 높았다.

6. 수평관계에서 경조직 변화와 연조직 변화가 비교력 강한 상관성을 보여주는 항목은

1:LS, 1:POg', Pog:LS, Pog: LI, Pog:Pog'이며 수직관계에서는 1:Pog', Pog:Ll, Pog: Pog

'만이 상관성이 높았다.

A CEPHALOMETRIC STUDY ON THE HARD AND SOFT TISSUE CHANGES BY THE RAPID PALATAL

EXPANSION IN ANGLE'S CLASS Ⅲ MALOCCLUSION

This study was undertaken to evaluate the cephalometric changes of the soft

tissue and skeletal profile subsequent to the rapid palatal expansion in 25 Angle's

Class Ⅲ cases, ranging in age from six to fifteen years, with cross-bite of the

anterior teeth , underdevelopment of maxilla and facial disharmony.

Following results were obtained :

1. ANS moved downward, Point A presented forward & downward movement increasing

SNA and Point B presented backward & downward movement decreasing SNB.

2. Mandible was rotated to backward & forward and maxilla moved forward &

downward with the bite opening and improvement of anterior teeth cross-bite.

3. Soft tissue on mandible was rotated to backward & forward following hard

tissue changes causing the decrease of facial convexity angle and backward &

downward rotation of Point B', Pog'.

4. Response of the upper lip was more significant in downward than forward

direction, and correlated with the upper central incisor and mandible rotation.

5. Response of the lower lip was more significant in downward than backward

direction, and correlated with the mandible 1 rotation.

6. There was a rather high degree of correlation between skeletal profile and

soft-tissue profile, 1:LS, 1 : Pog', Pog:LS, Pog:L1, Pog:Pog' in horizontal

measurements and 1:Pog', Pog:L1, Pog:Pog' in vertical measurements.
