복합경구 피임제 복용이 혈청 지질함량에 미치는 영향

dc.description.abstract[영문] [한글] 스테로이드성 경구피임제를 장기복용 함으로 인하여 심근경색증, 동맥경화증이나 고혈 압의 발생빈도가 증가한다는 것이 보고되어 왔으며 경구피임제를 복용한 여성에서 헐청 지질 농도가 증가되며 그중 cholesterol과 triglyceride가 증가된다는 보고들이 확인되어 왔다. 이러한 경구피임제 복용자의 혈청지질 농도에 관한 연구는 주로 구미인을 대상으 로 행하여져 왔다. 그러나 혈청지질의 농도는 식이양상, 영양상태, 기후, 종족 등 여러 요소에 따라 차이가 있는 것으로 보고되어 왔다, 본 연구는 복합 경구피임제를 복용하는 한국농촌가임 여성들의 혈청지질 농도와 양상을 알아보기 위하여 시도하였다. 대조군으로는 경구피임제를 사용한 경험이 없는 여성 10명을 선정하였으며 피임제 복용 자는 1년이하 복용자 14명, 1∼2년 복용자 12명, 2∼5년 복용자 29명, 5∼10년 복용자 32 명, 10년 이상 복용자 13명등 총 100명을 대상으로 하였다. Cholesterol은 Levinson의 효 소방법을 사용하여 측정하였으며 phospholipid는 Fiske-SubbaRow방법, triglyceride는 Su giura방법으로 측정하여 다음과 같은 결과를 얻었다. 1. 혈청 cholesterol농도는 대조군에 비해 피임제 복용자의 경우 4.8%가 증가하였으나 통계학적 유의성은 없었다. 2. 혈청 phospholipid농도는 대조군에 비해 피임제 복용자의 경우 14.3% 감소하였으며 통계적으로 유의하였다(P<0.05). 3. 혈청 triglyceride농도는 대조군에 비해 피임제 복용자의 경우 10.6%의 증가를 보였 으나 통계학적 유의성은 없었다. 4. 피임제 사용기간은 혈청 cholesterol, phospholipid와 triglyceride농도에 영향을 미치지 않았다. 이상의 결과로 보아 steroid성 복합경구 피임제는 혈청 phospholipid를 감소시켜 혈청 지질함량이 변동됨을 시사한다. THE EFFECT OF COMBINED ORAL CONTRACEPTIVE STEROIDS ON SERUM LEVELS OF LIPIDS Jung Hye Kim Department of Medical Science, The Graduate School, Yonsei University (Directed by Won Joon Kim, M.D.) Increased incidence of myocardial infarction, arteriosclerosis and hypertension in oral contraceptive steroids users has been recognized as serious problem. Evidences have been accumulated that oral contraceptive steroids cause an increase in serum lipid levels, mainly triglyceride as well as an increase in serum cholesterol concentration in some women. Effect of oral contraceptives on serum lipids have been predominantly confirmed in Europian and American women. It should be emphasized that effects of contraceptives may be influenced by difference in nutrition, climate, socioeconomic status and race. The present study was, therefore, attempted to determine the effect of oral contraceptive steroids on serum lipid levels in Korean women and also to demonstrate whether the duration of pill treatment may affect serum lipid levels. Ten women, who had never taken contraceptive steroids, served as a control. One hundred women on combined oral contraceptive steroids were grouped according to the duration of pill used: durations of less than one year(14 women), 1-2 years(12 women), 2-5 years(29 women), 5-10 years(33 women) and more than 10 years(13 women). Serum levels of cholesterol, phospholipid and triglyceride were measured by enzymatic method, Fiske-SubbaRow method and Sugiura method respectively. The results obtained are as follows; 1. Oral contraceptive steroids increased serum cholesterol concentration by 4.8% as compared with cholesterol value of control group. But this increase was not statistically significant. 2. As compared with control value, concentration of serum phospholipid in pill-treated women decreased by 14.3%, which was statistically significant.(p<0.05) 3. Concentration of serum triglyceride in pill-treated women increased, by 10.6% over control value, but this increase was not statistically significant. 4. Duration of pill-treatment did not affect serum concentrations of cholesterol, phospholipid and triglyceride. From the results mentioned above, it may be concluded that combined oral contraceptive steroids, at least used for subjects involved in this study, decreased serum phospholipid significantly, therepy influencing serum lipid concentration.-
