본 연구는 내분비계장애물질과 관련이 의심되는 비뇨생식기계질환 중 대표적인 질환인 정규고환과 요도하열을 중심으로 각 질환의 변화추이를 보고, 일상생활 속에서 내분비계 장애물질 노출 가능 요소들과 각 질환과의 관련성을 밝혀 이를 내분비계장애물질과 비뇨생식기계질환 관련성에 대한 기초적인 자료로 활용하고자 하였다. 각 질환의 변화추이는 대한비뇨기과 학회의 입원환자 통계자료와 통계청의 출생자수 자료를 이용하였고 일상생활 속에서의 내분비계 장애물질 노출 가능 요소들과 각 질환과의 관련성에 대해서는 정류고환 환자 42명과 요도하열 환자 50명, 소아과 방문 환자 96명의 부모들을 대상으로 구조화된 설문지를 이용하였다.

1996년에서 2002년까지 정류고환과 요도하열의 임상적 변화추이를 조사한 결과 전체 비뇨기과 입원환자 중 각각의 질환이 차지하는 비율은 약간의 감소추세를 보였으나 동일 해의 출생자수가 감소하고 있는 추세를 감안한다면 오히려 증가하고 있다는 해석도 할 수 있었다.

일상생활 속에서의 내분비계장애물질 노출 가능 요소들과 정류고환, 요도하열과의 관련성에 있어서 부모의 거주지역에서의 노출을 임신 전과 임신 중으로 나누어 조사한 결과 부모의 지역적 노출이 비뇨생식기계질환 특히 요도하열과 관련성이 있음을 알 수 있었고 부모의 직업에서의 노출과 부모의 식생활습관을 통한 노출은 다변량 분석 결과 통계학적으로 유의하지는 않았지만 비뇨생식기계질환과 어느 정도의 관련성이 있음을 의심할 수 있었다. 어머니의 내분비계장애물질 노출 가능 약물복용은 비뇨생식기계질환과 관련성이 있었는데 특히 경구피임약에 있어 관련성이 의심되었다. 부모의 흡연여부에 있어 아버지의 흡연이 비뇨생식기계질환과 관련이 있고 흡연기간이 길어지거나 흡연량이 많아질수록 비뇨생식기계질환을 가진 아들을 낳을 위험이 높아지는 것을 의심할 수 있었고, 어머니의 흡연은 본 연구 대상 가운데 흡연을 하는 어머니가 없는 것으로 조사되어 그 관련성을 알 수 없었다. 내분비계장애물질 노출의 간접적 증거로서 어머니의 유산경험 여부는 다변량 분석 결과 통계학적으로 유의하지는 않았지만 비뇨기계 질환과 어느 정도 관련이 있는 것을 알 수 있었고, 비뇨기계 질환의 가족력은 정류고환의 경우에서 관련성이 있는 것을 알 수 있었으며, 가족 중 암 발생 여부는 비뇨기계 질환과 관련성이 있는 것으로 나타났으나 암 발생의 경우 개인적 기질이나 생활 습관 등 많은 관련요소들이 작용하므로 관련성이 있다고 하기에 무리가 있는 것으로 생각된다.

본 연구는 일상생활 속에서의 내분비계장애물질 노출 가능 요소들을 규명하고 이를 내분비계장애물질과 비뇨생식기계질환과의 관련성에 대한 기초적 자료로 활용 할 수 있다는 데 의의를 가질 수 있고, 앞으로 이 결과를 바탕으로 내분비계장애물질 노출 가능 요소들을 보다 구체화하고 객관화 한 추후연구가 필요하다고 생각된다.





[영문]Endocrine disruptor is suspected to be related to the urogenital disease. This study observes the trend of cryptorchidism and hypospadias, the most common types of urogenital disease, and find out how we are exposed to endocrine disruptor in our daily life and how it is related to the diseases. This study will be a basic reference to the relation of endocrine disruptor and urogenital disease. For the investigation of the trend of the diseases, statistic datas of the patients of The Korean Urology Association and the demographic statistics of the National Statistical Office were used as a reference, and for the investigation of the how our exposure to endocrine disruptor is related to the diseases, questionnaire surveys were conducted to 42 cryptorchidism patients, 50 hypospadias patients, and 96 parents of pediatric outpatients.

Examining the clinical trend of cryptorchidism and hypospadias based on the datas from year 1996 to 2002, out of all urology patients, the portion of each disease seems to be decreasing slightly, however considering the decreasing birth rate, it can be interpreted that the portion is actually increasing.

Investigation of how we are exposed to endocrine disruptor and its relation to the diseases was analyzed depending on the parents'' exposure in their residents before pregnancy and during pregnancy. The outcome of the analysis was that the parents exposure to endocrine disruptor was related to urogenital diseases, especially hypospadias and also according to the multivariate analysis, patents'' exposure in their workplaces and in their dietary habits was relatively related to urogenital diseases. Maternal use of drugs which could be an exposure to endocrine disruptor is related to urogenital diseases; especially taking birth pills has a great influence. In case of smoking parents, father''s smoking is related to urogenital diseases and if the period of smoking gets longer, and amount of cigarettes gets larger, the risk of bearing a son with urogenital disease is bigger. In case of mother''s smoking, analysis was excluded in this study because not a single mother under investigation smoked thus making it impossible to find out the relation. Mother''s experience of abortion, which is an indirect evidence of endocrine disruptor exposure, was according to multivariate analysis, related to urogenital diseases. Family history of urogenital disease is related with cryptorchidism, and a family history of cancer is also shown as being related to urogenital disease. However, taking into account that cancer is affected by personal constitution, and personal habitude, it is difficult to conclude as such.

This study is meaningful in a way that it thoroughly investigates our exposure to endocrine disruptor in our daily life and that it can be used as a basic reference to the relation of endocrine disruptor and urogenital disease. With these basic findings, further studies on the exposure to endocrine disruptor with more detailed and objective surveys will be needed.
