아디포넥틴은 지방세포에서만 생산되는 혈장단백질로 chromosome 3q27에 위치하는 adipose most abundant gene transcript-1 유전자에 의하여 encoding된다. 아디포넥틴의 혈중농도는 비만환자, 관동맥질환자, 제 2형 당뇨병 환자에서 감소되어 있으며, 인슐린저항성 정도와 반비례함이 알려져 있다. PPAR-γ 효현제는 PPAR-γ를 활성화시켜 아디포넥틴 유전자에 결합하게 하여 아디포넥틴 유전자의 발현을 증가시키는데, PPAR-γ 효현제를 투여함에도 혈중 아디포넥틴 농도가 증가하지 않는 환자들이 있다. 이러한 환자들은 아디포넥틴 유전자의 다형성에 의하여 PPAR-γ 효현제에 반응하지 않는 것으로 사료되어 본 연구에서는 제 2형 당뇨병 환자에게 PPAR-γ 효현제인 rosiglitazone을 투여한 후 환자들의 아디포넥틴 유전자의 다형성에 따라 혈당조절 및 아디포넥틴 농도의 변화와의 관계를 분석하고자 하였다. 총 109명의 제 2형 당뇨병 환자에서 3개월간 기존의 약제는 변화시키지 않으면서 rosiglitazone을 추가하여 투여하고, 혈중 아디포넥틴 농도를 rosiglitazone 투여 전 후로 측정하고 아디포넥틴 유전자의 프로모터 부위의 PPAR responsive element(PPRE)부위와 liver receptor homolog-1 responsive element(LRH-1 RE)부위 및 SNP45, SNP276 부위의 유전자 분석을 실시하여 유전자 다형성과 혈당반응 및 혈중 아디포넥틴 변화와의 관계를 분석하여 다음과 같은 결과를 얻었다.



1. 아디포넥틴 유전자의 PPRE부위와 LRH-1 RE부위에서는 유전자 돌연변이가 관찰되지 않았다.

2. SNP45부위의 유전자형에 따른 기저 공복혈당, 당화혈색소, 아디포넥틴의 농도에는 차이가 없었다. G

G 유전자형을 갖는 환자에서 다른 유전자형을 갖는 환자보다 rosiglitazone 투여에 의한 당화혈색소의 감소정도가 유의하게 적었고(0.0±1.1 vs. 0.7±1.1% reduction, p=0.015) 아디포넥틴의 증가정도도 유의하게 적었다(1.1±1.9 vs. 4.2±5.2㎍/㎖, p=0.021).

3. SNP276 부위의 G/G 유전자형을 갖는 환자에서 다른 유전자형을 갖는 환자에서보다 아디포넥틴의 증가정도가 유의하게 적었다(2.5±4.5 vs. 4.9±5.1㎍/㎖, p=0.012).

4. Haplotype 조합을 이루어 보았을 때 GG/GG 유전자형조합이 TT/TT 유전자형조합보다 아디포넥틴 증가정도가 유의하게 적었다(12.5±3.0㎍/㎖ vs. 1.21.5㎍/㎖, p=0.001).

5. 아디포넥틴 유전자의 SNP45부위의 G allele과 SNP276부위의 G allele을 갖는 당뇨병 환자에서는 rosiglitazone 투여 시 아디포넥틴의 증가가 다른 유전자형에 비해서 적었으며, SNP45부위의 G allele을 갖는 환자에서 rosiglitazone에 의한 혈당강하 효과도 적었다.



결론적으로 아디포넥틴 유전자의 다형성은 rosiglitazone의 혈당강하효과 및 혈중 아디포넥틴 증가효과에 영향을 줄 수 있으며, 향후 아디포넥틴 유전자 다형성이 PPAR-γ 효현제의 효과에 주는 영향에 대한 기능적인 연구가 필요할 것으로 사료된다.





[영문]Adiponectin gene polymorphisms have been reported to be associated with type 2 diabetes (T2DM) and the circulating adiponectin levels. The aim of this study was to examine the effects of the rosiglitazone on adiponectin and plasma glucose levels according to the common adiponectin gene polymorphisms.

Material and Methods: A total of 109 patients with T2DM were treated with rosiglitazone(4mg/day) for 12 weeks without changing any previous medications. Peroxisome proliferator-activated receptor responsive element(PPRE), single nucleotide polymorphism(SNP) 45 and SNP276 of the adiponectin gene were examined. Serum adiponectin levels were measured before and after rosiglitazone treatment.

Results: No mutation at PPRE in adiponectin gene promoter was observed. Regarding SNP45, the reduction in the HbA1c value was lower for the G/G homozygote genotype than for the other genotype (0.0± vs. 0.7±% reduction; p=0.015). The increment in the plasma adiponectin concentration was lower for the G/G homozygote genotype than for the other genotype (1.1±1.9 vs. 4.2±5.2㎍/㎖; p=0.021). Regarding SNP276, the increment in the plasma adiponectin concentration was lower for the G/G homozygote genotype than for the other genotype (2.5±4.5 vs. 4.9±5.1㎍/㎖; p=0.012). It was observed that the increase in the plasma adiponectin concentration was lower for the GG/GG haplotype than for the TT/TT haplotype (1.2±1.5, 12.5±3.0㎍/㎖; p= 0.001).

Conclusion: Present study results suggest that patients with G allele at locus 45 and G allele at locus 276 are likely to response poorly to rosiglitazone treatment. Genetic variations in the adiponectin gene may affect the rosiglitazone treatment response of the circulating adiponectin level and the blood glucose control in patients with T2DM.
