최근까지 들어와서 환경오염물질 중 중금속 오염에 의한 영향은 과거부터 지속적으로 고려되어 왔다. 저농도 중금속의 지속적인 노출은 생체 내에서 농축이 되고 이로 인해서 여러 가지 건강 위해로 나타나게 된다. 특히 수은(Hg)은 환경 중 미량으로 존재하는 원소이지만, 산업과 농업 활동의 결과로 배출되어 환경 상 문제가 발생하고 있다. 본 연구에서는 도시에 거주하는 일반 성인의 혈중 수은 농도분포를 확인하고, 개인 생활 요인과 신장 기능과 산화성 손상에 관한 지표와의 상관성 평가를 통해 일반 성인의 혈중 수은농도에 있어서 관련된 생활 요인을 평가하고, 실제 나타난 건강 요인과의 관련성을 살펴보고자 하였다.

연구대상자의 임상검사 및 중금속분석을 위한 채혈은 2002년 5월부터 9월까지 실시되었다. 총 235명의 남녀를 대상으로 채혈 및 임상검사를 실시하였으며, 설문조사를 통해 성별, 연령, 흡연유무 등의 기본 조사를 실시하였으며, 식습관에 관한 설문도 함께 실시하였다. 혈중 수은(Hg) 농도 측정을 위해서 흡수분광광도계에 수소화물 발생장치(hydride generator, Shimadzue HVG-1, Japan)를 장착하여 분석하였다. 자료의 통계분석은 SAS 프로그램을 이용하여 로지스틱(logistic regression) 및 다중회귀분석(multiple regression)을 실시하였다.

혈중 수은의 농도는 전체 평균 3.19㎍/ℓ(불검출~8.64㎍/ℓ)이었으며, 대체로 정규분포를 보이고 있었다. 총 230명의 연구 대상자 중 35명(15.2%)에서 검출한계 이하의 농도를 보였으며, WHO(2001)에서 제시하고 있는 일반인 수은 노출 수준에 따른 혈중 농도(5㎍/ℓ)를 초과하는 연구 대상자는 16명(7.0%)로 나타났다.

연구 대상자의 혈중 수은 농도에 영향을 미치는 개인 생활특성 및 식습관을 알아보기 위하여 다중회귀분석을 실시한 결과, 일반 개인특성 중 흡연이 통계적으로 유의한 것(p<0.05)으로 관찰되었으며, 통계적으로 유의하지는 않았지만 채소 및 과일 섭취 빈도가 높으면 높을수록 혈중 수은 농도가 낮아지는 것으로 나타났다. 또한 음주 습관에서 거의 마시지 않는 사람에 비해 매일 마시는 사람에서 통계적으로 유의성(p<0.05)을 관찰할 수 있었다.

연구 대상자를 신장질환 그룹과 정상 그룹으로 나누어서 혈중 수은의 농도를 관찰한 결과 남자의 경우 정상 그룹은 평균 3.35㎍/ℓ, 신장질환 그룹은 평균 5.30㎍/ℓ으로 통계적으로 유의한(p<0.05) 차이를 나타냈으나 질환 그룹의 대상자 수가 너무 적었기 때문에 해석상 제한이 있었으며, 혈중 수은 농도를 세 농도(Hg-B<2, 2≤Hg-B<5, 5≤Hg-B)범위로 분류하여 신장 기능 지표들의 차이를 관찰 한 결과, 크레아티닌과 β2 마이크로글로블린 항목에서 혈중 수은의 농도 분류에 따라 통계적으로 유의한 차이(p<0.05)가 관찰되었다.

산화성 손상 지표인 MDA(Malonydialdehyde)와 8-OHdG (8-Hydroxydeoxyguanosine)의 분석을 통하여, 혈중 수은의 농도 분류에 따른 각각의 농도를 평가한 결과 혈중 수은이 높은 그룹일수록 MDA와 8-OHdG의 수치가 증가함을 보였으나 통계적으로 유의하지는 않았으며, 흡연 습관 및 식습관의 조사로는 구운고기의 섭취 빈도에서 유의한 관련성을 관찰할 수 있었다.

혈중 수은에 영향을 많이 주는 인자를 고려하여 연구 대상자를 고위험군(흡연, 육류 주섭취 그룹)과 저위험군(비흡연, 채소류 주섭취 그룹)으로 나누어 혈중 수은 농도를 분석한 결과 고위험군에서 4.27㎍/ℓ(ND~7.84㎍/ℓ), 저위험군에서 3.30㎍/ℓ(ND~8.64㎍/ℓ)으로 고위험군이 저위험군에 비해 높았으며 통계적으로 유의한(p<0.05) 차이가 있는 것으로 나타났으며, 건강 영향 지표에서 고위험군에 미치는 인자와의 로지스틱 회귀분석 결과, 산화성 손상지표인 8-OHdG가 저위험군에 비해 고위험군이 5.4배 높은 것으로 관찰되었으며, creatinine항목의 odds ratio가 3.6으로서 고위험군에 관련성 있는 인자로 나타났다.

수은은 다른 중금속과 마찬가지로 여러 가지 경로로 인체 내로 유입하게 된다. 그 중 가장 큰 부분은 일반인에 있어서 식생활에 의하여 그 축적량의 차이가 나타나며, 이에 따른 인체 영향이 다양하게 발현하는 경향이 있다. 최근에 와서 인체에 안전하다는 농도는 현재 새로운 연구에 의해 바뀌고 있으며, 이러한 농도에 있어서의 확장된 연구와 재평가가 필요한 실정이다. 또한 국제기구에서는 수은의 노출에서 보호하기 위한 어류의 수은 농도 제한치(0.5㎍/g)를 사용하고 있으나, 인간이 자주 섭취하게 되는 어류에 있어서 이러한 제한치는 수은에 의한 위해로부터 인체를 보호하기 위한 안전 장치로서 충분하지 않을 것이며, 이에 대한 지속적인 관리, 관찰이 필요할 것으로 사료된다.

Heavy metal pollution has become a serious health concern in recent years. Continuous exposure to low levels of heavy metals may result in bioaccumulation and adverse health consequences in human. Although, mercury(Hg) is not a naturally abundant element in the environment, residues frequently occur in many environmental compartments because of widespread contamination from industrial and agricultural practices. This research evaluated Hg-B concentrations of general population who was not occupationally exposed. And also evaluates the association between life factor and health effect with Hg-B concentration of general adult through interrelationship estimation and index about kidney function and oxidative damage that appeared by questionnaire survey and medical examination.

Blood sampling and clinical examination for research target person and heavy metals analysis were taken in May and September, 2002. Then I conducted basis investigation of sex, age, smoking etc., and questionnaire survey about eating habits to 235 participants at the same time. Hg-B analysis used AAS attached by hydride generator(Shimadzue HVG-1, Japan) for measuring. I performed logistic regression and multiple regression analysis using SAS program for statistical analysis of data.

Hg-B concentration was total average 3.19㎍/ℓ(ND~8.64㎍/ℓ), and showed normal distribution on the whole. 35(15.2%) of 230 participant were below detection limit, and persons who exceed mercury exposure level(5㎍/ℓ) presented in WHO(2001) appeared by 16(7.0%).

According to multiple regression analysis to search individual life factor and eating habit that influence to Hg-B concentration of participants, it observed statistically significant(p<0.05, regression coefficient 0.768) for smoking of general individual life factor, and Hg-B concentration becomes lower as vegetables and fruit intake frequency increase, but it was not statistically significant. Also, person who drinks alcohol every day than person who do not drink hardly in drinking habit could observe higher Hg-B concentration(p<0.05) as regression coefficient 1.518.

Participants divided to nephropathy group and normal group, and then observed Hg-B concentration. Nephropathy group expressed average 5.30㎍/ℓ and normal group was 3.35㎍/ℓ(p<0.05), but there was analysis limitation by different classification number between groups. Hg-B concentration classified to three range(Hg-B<2, 2≤Hg-B<5, 5≤Hg-B) and evaluated differences for kidney function index. According to the result, difference as creatinine and β2 microglobulin observed significantly(p<0.05).

As Hg-B concentration classification, MDA (Malonydialdehyde) and 8-Hydroxydeoxyguanosine(8-OHdG)(oxidative damage index) increased but it is not statistically significant, and MDA was higher in smoker than non-smoker(p<0.01). Also, the intake amount of roast meat is lead to increasing 8-OHdG value significantly(p<0.05). It is considered that oxidative damage influenced in eating habit. Considering factors that influence to Hg-B concentration, divided participant by high-risk group(smoking and meat main intake group) and low-risk group(non-smoking and vegetable diet main intake group). High-risk group had significantly higher Hg-B concentration than low-risk group(p<0.05, low-risk group:3.30㎍/ℓ(ND~8.64㎍/ℓ), high-risk group:4.27㎍/ℓ(ND~7.84㎍/ℓ)). As a result with the logistic regression analysis on factor which kidney function and oxidative damage index influenced to high-risk group, observed 8-OHdG''s OR(odds ratio) as 5.393, and creatinine(kidney function index)''s OR was 3.592 as relative factor.

Mercury is influenced in human body through various kinds of path as other heavy metals. Because the biggest part of them is associated with dietary life in ordinary people, of accumulation amount and effect on human body tends to appear variously. In recent years, level that is safe in human body is changing according to new research, and these level is necessary to extended research and re-evaluation. Also, International organization recommend limit value(0.5㎍/g:WHO/ICPS, 1990) that was protect in dangerous exposure by mercury. But it is not enough to protect human health as safety device in finny tribe that human often ingests, so we need to conduct continuous management and research.
