본 연구는 삼음교 지압이 여대생의 월경통에 미치는 효과를 규명하기 위하여 한의학의 경락학설(經絡學說)을 기반으로 스트레스, 월경태도, 월경통 강도, 곡골혈 피부온도, 혈청 코티졸 호르몬을 연구변수로 도출하였으며, 비동등성 대조군 전후 설계(nonequivalent control group pretest-posttest design)를 이용하였다.연구기간은 2002년 5월 1일부터 8월 31일까지였으며 서울과 경기도 소재 2개 대학교를 선정하여 월경통을 호소하는 여학생을 실험군과 대조군에 배정하였다. 연구대상자는 실험군 30명과 대조군 28명으로 총 58명이었다. 삼음교 지압은 문헌고찰과 전문가 집단의 타당도 조사, 연구자 훈련, 예비조사를 거쳐 수정·보완하였다. 자료수집은 실험군과 대조군 모두 사전에 제 변수를 측정하고 난 후 실험군에게는 20분간 삼음교 지압을 제공하였으며 효과를 측정하기 위해 처치 직후, 처치 30분 후, 1시간 후, 2시간 후, 3시간 후, 4시간 후, 5시간 후, 6시간 후, 7시간 후, 8시간 후에 제 변수를 측정하였다. 연구도구는 Johnson(1974)의 Visual Analogue Scale(VAS)을 이용하여 월경 통의 강도를 측정하였고 대상자의 곡골(曲骨)혈 피부온도는 피부온도계(Simson electric co., USA)를 사용하였으며 혈청 코티졸 호르몬도 분석하였다. 또한 Brooks-Gunn과 Ruble(1980)의 월경태도(Menstrual Attitudes Questionnaire) 측정도구, 전겸구와 김교헌(1990)의 스트레스 도구를 사용하였다. 수집된 자료는 SAS program을 이용하여 - test, t-test, ANCOVA, Bonferroni method로 비교 분석하였다.연구결과는 다음과 같다. 가설 1 : 삼음교(SP-6) 지압을 받은 여대생과 삼음교 부위에 엄지손가락을 접촉한 여대생의 월경통의 강도는 차이가 있을 것이라는 가설은 일부 지지되었다. 지압 직후(F=18.50, p=0.001), 30분 후(F=12.29, p=0.009), 1시간 후(F=12.37, p=0.009)에서 두 군 간에 유의한 차이가 나타났으며, 2시간 후(F=8.04, p=0.064)에서는 실험군이 대조군에 비하여 월경통의 강도가 감소되는 경향이 나타났다. 가설 2 : 삼음교(SP-6) 지압을 받은 여대생과 삼음교 부위에 엄지손가락을 접촉한 여대생의 곡골(曲骨)혈의 피부온도는 차이가 있을 것이라는 가설은 기각되었다(지압 30분 후; F=4.87, p=0.06). 그러나 지압 30분 후에 실험군이 대조군에 비하여 곡골혈의 피부온도가 증가하는 경향이 나타났다.가설 3 : 삼음교(SP-6) 지압을 받은 여대생과 삼음교 부위에 엄지손가락을 접촉한 여대생의 혈청 코티졸 호르몬은 차이가 있을 것이라는 가설은 기각되었다(F=2.01, p=0.1621).이상의 연구결과를 종합해 보면 삼음교 지압은 여대생의 월경통의 강도를 감소시키는데 효과적이며 자궁의 반응점인 곡골혈의 피부온도를 증가시키는 경향이 나타났다.

The purpose of this study was to identify effects of the SP-6 acupressure on dysmenorrhea, skin temperature on CV2 acupoint, and serum cortisol level. This study was a pretest-posttest study design with a nonequivalent control group. Data were collected from May 1 to August 31, 2002. A total of 58 female students from two universities participated in the study. Among them, 30 female students were assigned as an experimental group and the other 28 students were assigned as a control group. The pretest was performed before the intervention to measure three variables such as the degree of dysmenorrhea, skin temperature on CV2 acupoint, and serum cortisol level for both groups. Then, the SP-6 acupressure was provided for 20 minutes for students in the experimental group. The instruments used in this study included Visual Analogue Scale developed by Johnson(1974), skin thermometer by Simson Eectric CO., serum cortisol level, Menstrual Attitudes Questionnaire scale developed by Brooks-Gunn & Ruble(1980), and Stress scale developed by Chun and Kim (1990).The data were analyzed using SAS program. Chi-square test, t-test, ANCOVA and Bonferroni method were used to determine significant differences between the two groups.The results of this study are as follows; Hypothesis I : There is a statistically significant difference in the intensity of dysmenorrhea after the intervention between the experimental group and the control group. This hypothesis is accepted. There were significant differences in the intensity of dysmenorrhea immediately after the intervention(F=18.50, p=0.001), 30 minutes(F=12.29, p=0.009), one hour(F=12.37, p=0.009) after the intervention between the two groups. Hypothesis II : There is a statistically significant difference in skin temperature on CV2 acupoint after the intervention between the two groups./This hypothesis is rejected. There were no statistically significant difference in skin temperature on CV2 acupoint immediately after the intervention(F=1.47, p=0.46), 30 minutes(F=4.87, p=0.06) after the intervention between the two groups. Hypothesis III : There is a statistically significant difference in serum cortisol level after the intervention was given between the two groups.This hypothesis is rejected as well. There was no statistically significant difference between the two groups(F=2.01, p=0.1621). In conclusion, the SP-6 acupressure was proved as an effective nursing intervention to reduce dysmenorrhea. Therefore, it is recommended to use the SP-6 acupressure for women suffering from dysmenorrhea.
