욕창예방을 위한 의사결정 흐름도 개발

Abstract
[한글]

본 연구는 체계적인 욕창예방 간호의 지침을 마련할 목적으로 의사결정 흐름도를 개발하고자 시도된 방법론적 연구이다. 문헌 고찰을 통해 욕창발생 위험 요인을 도출하였고 의사결정 흐름도를 개발하고자하는 위험 요인을 선정하기 위해 전문가 집단을 이용한 타당도를 검증하였으며 각 항목은 CVI(Index of content validity)를 산출하여 83% 이상의 합의가 이루어진 항목을 선정하였다. 선정된 위험요인에 대한 예방적 간호의 세부 내용은 문헌고찰과 AHCPR의 ‘욕창예방 지침’, RCN의 ‘욕창 위험 요인 사정 및 예방 지침’을 기반으로 하여 본 연구자가 우리나라 실정에 맞게 구성하였으며, 작성된 세부 내용을 의사결정 틀에 맞추어 도식화하였다. 개발된

예비 의사결정 흐름도에 대한 내용 타당도는 전문가 집단을 이용하여 검증하였으며 전문가 타당도 검증 후 예비 흐름도의 임상 적용 가능성을 평가하기 위한 사용자 타당도는 임상 경력 2년 이상의 간호사를 대상으로 실시하였다. 전문가 타당도와 사용자 타당도를 거쳐서 수정, 보완된 흐름도를 욕창예방을 위한 최종 의사결정

흐름도로 확정하였다. 본 연구의 결과는 다음과 같다.1) 욕창발생 위험 요인 중 유의하다고 선정된 항목은 기동성 장애, 마찰력/응전력, 의식 상태 저하, 영양 상태 불량, 감각 장애, 습기(실금/실변), 활동성 장애, 말초 순환 장애, 간호제공자의 욕창과 관련된 전반적 인식 부족, 고연령으로 10개이다.2) 의사결정 흐름도 개발을 위한 일차적 모형은 압력에 영향을 미치는 위험 요인들에 대한 중재 방안을 먼저 수립하고 다음 단계에서 조직의 내구성을 저해하는 위험 요인을 사정한 후 각각의 위험 요인에 대한 흐름도를 개발하는 형식으로

구축하였다.3) 의식 상태 저하, 감각 장애, 기동성 장애 및 활동성 장애는 ‘압력’에 영향을 미치는 상태적 요소로써 압력의 강도와 기간을 감소시키는 간호중재 방안을 마련하였고 조직 내구성에 영향을 미치는 위험 요인인 마찰력/응전력, 실금/실변, 말초 순환 장애, 영양 부족에 대해서는 각각에 대한 중재 방안을 흐름도로 개발하였다.

‘간호제공자의 인식 부족’과 ‘고연령’의 항목은 흐름도 개발에서 제외하였다.4) 전문가 집단을 이용한 타당도 검증결과 흐름도의 구성, 용어, 내용에 있어서 합의도가 낮은 흐름도는 각각에 대한 전문가의 조언을 토대로 문헌고찰을 반복하여 내용 및 흐름 과정의 일부를 수정하였다. 5) 임상 적용 가능성을 평가하기 위한 사용자 타당도에서 의사결정 흐름도의 유용성으로는 일괄적이고 통일된 간호제공을 가능하게 하며, 흐름도를 통하여 욕창 유발 가능 요인들을 한눈에 확인할 수 있다는 점이 부각되었다. 또한, 흐름도의 사용이 예방적

욕창간호에 도움이 되며 안내 책자가 유용하다는 의견에는 대체적인 동의를 보였으나 사용의 편리성과 용어에 대한 이해의 용이함에는 70% 이하의 낮은 동의를 보였다. 그 원인을 분석한 결과 임상에서 흔히 접하지 않았던 용어들이 이해하기에 어려웠고 흐름도의 디자인이 단조로와 한눈에 들어오지 않는다는 의견에 따라 흐름도를 재구성하였고 추후 예방적 욕창 간호에 대한 지속적인 교육의 수립이 요구된다.6) 흐름도의 사용은 문제 해결이 용이할 뿐 아니라 지식습득을 할 수 있다는 장점을 가지고 있으므로 개발된 의사결정 흐름도는 지속적인 교육 도구로 활용될 수 있을 것이다. 결론적으로 본 연구에서 개발된 의사결정 흐름도는 구체적인 간호중재 방법의

제시로 예방적 욕창 간호에 있어서 일괄적이고 통일된 간호제공을 가능하게 하며 새로운 간호중재 방법을 학습하는 기회를 제공한다는 점에서 의의가 있다.

[영문]

The purpose of this study was to develop a decision-making tree to produce a comprehensive pressure ulcer prevention program. The risk factors for pressure ulcers were drawn from a review of the literature.

An expert team examined them in order to select the factors to be used in developing a decision-making tree. The final factors chosen were those with an index of validity above 83% agreement. The detailed composition for preventive care was based on a literature review, and guidelines from the AHCPR (Agency

for Health Care Policy and Research) and RCN (Royal College Nursing). These were used to form the decision-making tree. An expert team examined the content validity. User validity was tested by nurses who had more than 2 years clinical experience. The decision making tree was completed after modifications

from the expert and user validity testing.The results of this study are summarized as follows :1. The ten significant risk factors for pressure ulcer included; immobility, friction/shear, decreased mental status, nutritional deficit, sensory deficit, moisture (urinary/stool incontinence), impaired activity,

decreased peripheral circulation, knowledge deficit in the caregiver and old age. 2. The framework was composed of interventions for the effect of risk factors on pressure which were set up first, then the decision-making tree for each risk factor influencing tissue tolerance was developed. 3. Preventive

intervention were prepared to reduce intensity and duration of pressure for the conditional factors such as decreased mental status, sensory deficit, immobility and impaired activity. Each decision-making tree was developed with the factors influencing tissue tolerance such as friction/shear, urinary or stool incontinence, decreased peripheral circulation, and nutritional deficit. Knowledge deficit of the caregiver and old age were excluded from the decision-making tree.4. As the results of the expert validity test, the content and decision making process were partially modified.5. The utility of the decision-making tree was found to be its ability to provide standardized preventive care and to verify risk factors for pressure ulcers. Also the guidebook was evaluated as useful, but, there was less than 70% agreement on convenience of use and easiness of understanding. The reasons were analyzed and it was revealed that education about the preventive interventions of pressure ulcer are necessary.6. Because the decision-making tree has the benefit of giving information about up-to-date interventions as well as

usefulness in problem solving, it can be utilized as a tool for preventive pressure ulcer care education.

In conclusion, the decision-making tree which was developed in this study provides standardized preventive pressure ulcer care. Also it has significance in providing an opportunity to learn up-to-date preventive interventions.
