CQI 활동 후 사후관리 체계 조사연구

이 연구는 병원에서 CQI 활동 후 사후관리가 지속적으로 진행되고 있는지 사후관리가 잘되고 안 되는 요인이 무엇인지를 분석하기 위하여 1994년부터 1999년까지 한국의료QA학회에 발표된 의료기관의 CQI 담당자 및 과제 발표자들을 대상으로 각 병원의 사후관리 실태

를 조사, 분석하였다. 연구의 주요 결과는 다음과 같다. 1. 학회에 발표된 연제는 매년 증가하여 현재는 모든 의료기관에 확대되었으며, 의료기관은 연간 11∼20건의 CQI 주제를 운영하는 것이 가장 많았지만 31건 이상을 운영하는 경우도 많았다. 또한 800병상 보다 적은 의료기관이 더 많은 과제를 추진하므로서 인당 4건 이상 참여하는 경우가 23.3%나 되어 특정인에 의한 활동으로 업무부담이 증가되고 실질적인 활동보다 형식에 치우칠 가능성을 배제할 수 없었다.

2. 학회에 발표된 과제 중 51.7%가 사후관리가 되는 것으로 조사되었다. 특히 서울의 48.8%보다 지방이 55.3%로 높았으며, 600∼799병상이 80.0%로 사후관리가 가장 잘되는 것으로 나타났다. 발표 년도가 오래된 과제일수록 사후관리가 안 되는 것으로 조사되었다.

3. 사후관리가 잘된다고 응답하였지만 점검계획 및 도구 개발도 안된 상태에서 사후관리가 되고 있었으며, 그 이유는 활동 중 업무에 대한 표준화가 안된 상태로 종료되고, 팀원들도 생각하지 않는 것이 주 요인이었다.

4. 사후관리가 잘되기 위해서는 병원장과 활동 부서 및 팀원들의 관심, 주관 부서의 역할, 목표관리를 통한 사후관리, 업무 표준화, 상하 좌우간의 의사소통이 가장 중요한 요인으로 응답하였으며, 시행한 과제의 누적으로 인한 업무 과중 외에 사후관리에 대한 인식도 없는 것이 가장 큰 문제로 인식되었다.

5. CQI 활동은 전직원의 개선하고자 하는 마음과 경영에 도움이 되기 때문에 바람직하다는 의견이었으며, 사후관리를 성공하기 위해서는 리더쉽, 정보분석 및 활용, 사후관리 및 기획, 질 관리에 대해서는 매우 중요하다고 인식한 반면 성과에 대해서는 많은 차이가 있었다.

향후 CQI 활동이 활동으로 끝나지 않게 하기 위해서는 금번 연구결과에서 도출된 문제점을 토대로 근본적인 시스템을 재 구축하여 보완되어야 할 것으로 생각되며, 이번 연구가 각 병원에서 CQI 활동 후 사후관리를 하는 기본적인 틀을 잡기 위한 참고 자료가 되기를

기대한다. 그리고 앞으로도 CQI 활동에 대한 다양한 사후관리 연구가 진행되어야 할 것으로 생각한다.

[영문]

This study was conducted to determine whether follow-up management is carried out continuously following CQI activity and to analyze the factors behind the success and failure of follow-up management. Past presentations from 1994-1999 of CQI coordinators and lecturers from various institutions who presented at The Korean Society of Quality Assurance in Health Care(KoSQA) on the conditions of follow-up management in each institution were analyzed. The results of this study were as follows;

Since the number of subjects on CQI increased each year at symposiums, this has expanded to all medical institutions. Although medical institutions usually conduct 11-20 subjects on CQI per year, there were many such occasions where more than 31

subjects were conducted. Moreover, institutions with less than 800 beds have come up with more projects than those with more than 800 beds, thus 23.3% of these institutions had at least 1 person involved in 4 projects. This had created an overload of responsibilities for specific persons'' involvement, prompting them to incline toward formalities in their work rather than substantial activities.

Among the projects presented at the symposiums, 51.7% demonstrated that follow-up management could be carried out. In particular, 55.3% of the projects from provincial regions could carry out follow-up management compared to 48.8% in Seoul.

Moreover, it was demonstrated that 80% of the projects from institutions with 600-799 beds carried out follow-up management most effectively. With regards to previous presentations, the older they were, it was found that follow-up management could not be effectively carried out.

Some institutions that responded that follow-up management was carried out effectively in their institutions were found to have conducted follow-up management without any inspection strategies or the appropriate tools. CQI activities were executed and terminated with no consistency and team members had no real concern for it.

The most important factors that contribute to an effective follow-up management are the need for concern and interest from the directors of the hospitals, from the relevant departments and team members in addition to the role of the supervising department, follow-up management through management of target goals, consistency in tasks along with communication between all team members. The biggest problems were perceived to be overload of work due to accumulation of proposed projects in addition to lack of awareness pertaining to follow-up management.

CQI is beneficial for all staff for the improvement of the mind and business administration and thus it is believed to be desirable. To carry out follow-up management effectively, leadership, analysis and application of information,

follow-up management and planning, as well as quality management are perceived to be essential, on the other hand, the results showed a significant difference. Henceforth, to prevent CQI activities from becoming just an activity, the basic

system should be reconstructed and augmented based on the problems derived from the results of this study. Moreover, we hope this study will be used as reference material that would encourage the administration of follow-up management after CQI

activities in most hospitals. Furthermore, various studies on follow-up management should be conducted for CQI activities in the future.
