5. 증상과 증후는 시신경장애가 14례로서 가장 많았고 두통이 10례, 홀몬성 증상이 6례, 구토와 뇨붕증을 호소한 예가 각각 2례였으며 1례에서는 당뇨병의 증상을 나타내었다.
6. 뇌하수체 선종 16례의 방사선학적 분류는 터키안 상부로만재 연장되어 자란 선종이 6례였고, 터키안에 국한된 선종이 1례였으며 터키안 상부와 측부로 자란 선종이 9례로서 가장 많았다.
7. 수술 후 시신경 장애의 호전을 보인 경우는 생검만 실시한 미분화 악성종양 1례와 수술 전 시신경 장애가 전혀 없었던 2례를 제외한 16례 중 10례였다. 술후 악화된 예는 2례였으며 나머지 4례에서는 변화를 볼 수 없었다.
8. 수술 후 사망한 예는 1례였으며 술 후 3일째부터 뇌압상승의 증후를 보여 종양 또는 출혈로 인한 뇌압상승으로 보고 개두술을 계획하였던 거대한 종양으로 보호자의 수술 거부로 수술 시기를 놓쳤던 예이다.
9. 술 후 합병증은 6례에서 관찰되었는데 그 종류는 시신경 장애의 악화 2례, 일시적 뇨붕증 2례, 비중격 관통 2례, 비중격 만곡증 1례, 비강 폐쇄증과 비출혈이 각각 1례였다. 시신경 장애의 악화를 제외한 전 합병증은 적절히 치유되었다.
10. 19례에서 20번의 transsphenoidal approach를 시행하고 여러 지견을 얻게 되었으며 이러한 것은 금후 보다 완전한 수술을 시행하는 데 지침이 될 것으로 믿어진다.

From September, 1976, to June, 1978, 20 transsphenoidal operations were performed on 19 sella lesions (12 males, 7 females) with use of surgical microscope and radiofluoroscopic image intensification at the Department of Neurosurgery, Yonsei
University College of Medicine.
This series included 16 cases of pituitary adenoma, one case of
craniopharyngioma, one case of undifferentiated malignant tumor and one case of empty sella syndrome.
In a series of 16 pituitary adenoma, 10(62.5%) were nonsecreting and 6(37.5%) produced a hypersecretion syndrome: human growth hormone (HGH:4), prolactin(2).
Each patient was evaluated neuroradiologically and each pituitary tumor was classified as an intrasella, suprasella, parasella extension. anterior pituitary function was also evaluated for only some patient pre- and post-operatively.
During the transsphenoidal surgery, selective gross total removal of the adenomatous tissue was attempted but it was very difficult on our patients except one microadenomas. ten patients were considered cured, on the basis of relief of visual symptoms. Postoperative radiotherapy was employed in 12 cases of pituitary
adenomas.
In the entire series of 20 transsphenoidal surgery, minor complication occurred in 6(31.6%) patients in the form of transient diabetes incipidus (2), septal deviation (1), septal perforation(2), aggrevation of vision (2), nasal obstruction
(1) and epistaxis (1). In the entire series, there was one postoperative death.
The technical aspects of the transsphenoidal procedures were given with separate consideration of microadenoma and larger tumors. The simplicity of this technique, results of operation were described an discussed with review of literatures.
