선천성 비후성 유문협착증에 대한 임상적 관찰

선천성 유문 협착증은 생후 2개월내에 발생하여 비교적 발생 빈도가 높은 질환으로서 진단만 신속히 되면 간단한 치료로 완치를 기할 수 있다.

연세대학교 의과대학 부속 세브란스 병원에 1959년 1월부터 1978년 8월까지 약 20년 동안에 걸쳐 선천성 유문 협착증으로 진단된 75예를 분석 관찰하여 다음과 같은 결과를 얻었다.

남녀비는 6.5:1로 남아에서 월등히 많은 발생비율을 보였고, 5년 간격으로 본 발생빈도는 1974년에서 1978년 사이에 38예로 가장 많이 관찰되었고, 출생 순위별 발생율은 75예중 첫째 아이가 37예로 과반수를 차지하였다.

증상발생시부터 입원까지의 기간은 1주 말부터 3주 말사이가 41예로 가장 많았다. 임원시 임상증상 및 이학적 소견은 구토가 전예에서 있었고, 우측 상복부에 종괴를 촉지 할 수 있었던 경우가 67예(90퍼센트), 가시성 유동파를 볼 수 있었던 경우가 25예(33퍼센트), 장기 황달이 6예(8퍼센트)에서 있었다. 대사성 알칼리 혈중을 나타내는 경우는 25예(33퍼센트), 혈중 Na농도는 정상 범위인 130∼150mEq/1인 경우가 59예(80퍼센트), K농도에서도 3∼5mEq/1인 경우가 65예(87퍼센트)로 대부분 정상 범위에 있었다. 치료는 70예에서 외과적 치료를 하였던 바 69예에서 완치가 되었고, 1예는 수술후 흡인성 폐염으로 사망하였다. 5예에서 내과적 치료를 시행하여 4예에서 호전을 보였다.

During last 20 years from 1959 to 1978, 75 cases of the congenital pyloric stenosis were admitted to department of pediatrics of Yonsei University College of Medicine, and evaluated clinically.

The results obtained are as follows:

1. Vomiting is usually developed during 2 to 4 weeks of life(38 cases, 50%) and the ratio of male to female was 6.5:1.

2. The incidence among siblings were 37 cases in 1st born, 20 cases in 2nds, and 9 cases in 3rds.

3. The duration from onset to admission showed a peak incidence between one to two weeks(41 cases among 75 cases)

4. In the symptoms and physical findings;

Non-bile strained vomiting was developed in all of the cases, abdominal masses palpated in 67 cases(90%), and visible peristaltic waves were observed in 25 cases(33%).

5. Metabolic alkaglosis was developed in 25 cases(33%), serum Na concentration was ranged between 130-150 mEq/1 in 59 cases and serum K concentration between 3-5 mEq/1 in 65 cases.

6. 70 cases received surgical treatment. 69 cases were improved, and one case was expired due to aspiration pneumonia. 4 cases were improved with medical treatment including anticholinergics.
