Percutaneous Transhepatic Biliary Drainage (PTBD) is an effective new nonsurgical method for combined external and internal catheter decompression of obstructed extrahepatic bile ducts.
PTBD was performed 40 times in 37 patients with obstructive jaundice, from May, 1981, to August 1983, at Department of Radiology Severance Hospital, Yonsei University, College of Medicine.
The causes of obstructive Jaundice were bile duct cancer in 17 cases, pancreas head cancer in 9 cases, periampullary cancer in 5 cases, stone in common bile duct in 4 cases and metastasis to porta hepatis in 2 cases.
Internal drainage was performed in 8 cases and external drainage in 29 cases. Daily drainage amount was correlated with decline of serum bilirubin and alkaline phosphatase level in blood.
Six patients in sepsis were all improved after PTBD with decline of WBC level and fever.
This relatively simple procedure effectively decompresses the severely obstructed biliary tree prior to sugery, palliates the clinical problems in patient with unresectable malignant obstruction and controls sepsis effectively.
