대장및 직장암에 대한 임상적 고찰

대장 및 직장암은 지역에 따라 발생빈도에 차이가 많은며 국내에서는 최근에 이르러 이의 발생빈도가 증가하는 추세이다.

대장 및 직장암은 치료후 결과가 비교적 양호한 암종의 하나이나 최근 20년간 그 생존율에 획기적인 향상이 없는 실정이다

이에 저자는 1972년 1월부터 1981년 12월까지 연세대학교 의과대학 외과학교실에서 수술 혹은 조직생검을 하여 대장 및 직장암으로 진단된 환자 330예에대한 임상적 고찰과 치료후 생존율을 조사하여 다음과 같은 결과를 얻었다.

1. 호발연령충은 50대와 60대로 전체의 49.0%였다.

2. 남녀의 비는 1.2대 1이었다.

3. 암의 위치는 직장이 65.7%로 가장 많았으며 상행결장이 18.8%, S자상결장이 11.5%였다.

4. Dukes씨 분류의 Astler-Coller modification에 따른 환자의 분포는 A가 1.0%, B가 45.0%, C가 36.2%, D가 17.8%였다.

5. 원발병소를 절제할 수 있었던 예는 257예로 절제율은 86.2%였다.

6. 전체대상에 대한 5년생존율은 30.2%였고 암의 진행정도에 따라 생존율에 현격한 차이가 있었다.

7. 암의 위치에 따른 5년생존율은 우측결장의 경우가 36.0%로 가장 양호하였으며 좌측결장이 33.3%, 직장의 경우 27.2%를 나타내었다.

[영문]

The incidence of carcinoma of the colon and rectum varies widely throughout the world, and is seemed to increase recently in Korea. Although this form of cancer is recegnized as one that can be treated with relatively favorable results, more critical review of the literature of the past 20 years reveals that these has been little improvement in the survival of patients with this disease.

In this regard, author have reviewed 330 cases of carcinoma of the colon and rectum which had been confirmed histologically and treated surgically from Jan. 1972 to Dec. L981 at Severance Hospital Yonsei. Medical Center. The objectives of this paper are to evaluate the practical situation of carcinoma of the colon and

rectum in Korea and to remind the new concepts on the management of the colorectal carcinoma.

The results are as follow :

1. The peak incidence occured in the sixth and seventh decades, with 142(49.0%) patients in these age group at the time of diagnosis.

2. The ratio between male and female is 1.2:1.

3. Location of tumor is most frequent in the rectum(65.7%) followed by ascending colon(18.8%) and sigmoid colon(11.5%).

4. The distrlbution according to Astler Coller modified Duke's Classification is as follow ; stage A ; 1.0%, STAGE b ; 45.0%, stage C; 36.2%, stage D ; 17.8%.

5. The resectability of primary lesion is 86.2%,

6. The overall 5 year survival rate is 30.2%.

5 year survival rate according to Dukes' stage Astier-Coller modification is 100% in stage A, 42.6% in stage B, 24.1% in stage C, and none in stage D.
