고혈압환자에서 가족기능지수(family APGAR)와 혈압강하정도와의 관계에 대한 연구

고혈압의 발병기전, 경과, 예후에 영향을 미치는 요인중 정신 사회적 스트레스가 중시되고 있는데, 정신 사회적 스트레스를 대변해주는 지수로서의 유용성과 신뢰성이 수차례 입증되어온 가족기능지수(Smilkstei, 1978)를 사용하여 고혈압 환자에서의 혈압 강하정도와 상관관계가 있는지를 보고자 하였다.

이에 1988년 5월부터 1988년 10월까지 오류동 시법진료소와 강화병원 가정의학과 외래로 내원하여 고혈압으로 진단받은 환자중 적어도 내원 6개월전까지는 고혈압에 대한 약물치료를 받지 않았으며, 초진시로부터 최소한 3개월은 추적진료를 받았으며, 3개월의 추적 진료 기간중의 총 투약일수가 최소한 60일이 넘는 환자들 103명을 대상으로 동량의 동일약물(Aldactazide 50mg)만을 투여하여 3개월간 추적 진료하여 만 3개월 후의 혈압 강하정도를 조사하여 다음과 같은 결과를 얻었다.

1. 조사 대상은 남자가 7명, 여자가 96명이었으며, 평균연령은 62.8세이었다. 비만형 체질을 가진 사람은 16명(15.5%)이었고, 흡연하는 예는 7예(6.8%)이었고, 흡연자의 평균 흡연량은 35 Pack. Year이었으며, 음주하는 예는 8예(7.8%)이었고, 음주자의 주당 평균

순수 알코올 섭취량은 371ml이었다. 고혈압의 이환 기간은 평균 4.6년으로 0년부터 15년까지 분포되어 있었으며, 합병증이 있는 경우는 10예(9.7%)로 심부전증이 8예, 뇌혈관 질환이 1예, 당뇨병이 1예가 있었다.

2. 가족기능지수는 0-3점(중증의 가족기증 장애군)이 14명(13.6%), 4-6점(중증도의 가족기능 장애군)이 39명(37.9%), 7-10점(건강한 가족기능군)이 50명(48.5%)이었다.

3. 치료전 수축기 혈압은 160.4±11.7mmHg, 이완기 혈압은 102.7±6.5mmHg, 평균 혈압은 121.9±7.4mmHg이었으며, 치료후 수축기 혈압은 137.6±7.4mmHg, 이완기 혈압은 90.6±6.5mmHg, 평균 혈압은 106.3±6.1mmHg으로, 치료전후로 혈압은 의의있게 강하하였다.

5. 치료후의 혈압 강하정도에 영향을 미칠 수 있는 변수들을 다변수 회귀분석을 사용하였을 때, 성별, 연령, 비만정도, 흡연여부, 음주여부, 고혈압의 이환기간, 합병증여부, 가족기능 장애정도로 수축기 혈압의 강하정도를 15.4%, 이완기 혈압의 강하정도를 37.5%, 평균 혈압의 강하정도를 38.4% 설명할 수 있었다. 이중에서 가족기능 장애정도만이 각각의 경우에서 통계학적인 의의가 있었다.

6. 가족기능 장애군별 수축기 혈압의 강하정도를 1요인 분산분서(one-way ANOVA)하였을 때, 각 가족기능 장애군 사이에서는 혈압 강하정도가 통계학적으로 유의한 차이가 있었다. 이완기 혈압 및 평균 혈압의 강하정도도 모든 군 사이에서 통계학적으로 유의한 차이가 있었다.

이상의 결과에서 고혈압 환자에서 동일 약물을 사용하였을 때의 혈압강하 정도에는 다른 변수는 별 영향을 미치지 못하였으나, 가족기능지수가 큰 영향을 미치는 것으로 나타났다. 그러나 이 연구는 농번기동안 외래로 내원하기 쉬운 여자와 노령층의 환자가 조사

대상자의 대부분을 차지하기 때문에, 흡연여부나 음주여부의 혈압 강하정도에 미치는 효과를 설명할 수 없어, 연구의 제한점을 갖고 있으며, 향후 이런 점이 보완된 연구가 뒤따라야 할 필요가 있다.

[영문]

The psychosocial stress has been considered as one of the effecting factors to the etiology, progress and prognosis of hypertension. It is true that the family APGAR score was proved as a predictor of psychosocial stress.

In order to find out the relationship between family APGAR and the degree of depression in blood pressure, a study was carried out at Oryu Health Service Clinic and Ganghwa Community Hospital from May 1988 to October 1988.

Total 103 patients had been on antihypertensive medications(Aldactazide 50mg/day) at least 60 days of 3 months follow-up. During that 3 months period, their blood pressures were checked. The systolic, diastolic, and mean blood pressures before treatment were compared to those after treatment.

Of the 103 patients, male patients were 7, and female patients were 96 respectively. The mean age was 62.8 years old, and obese patients were 16, smoking patients were 7, and drinking patients were 8, respectively, The average duration of hypertension was 4.6 years.

According to family APGAR, their family function were moderately dysfunctional showing the average score of 6.4 points.

The systolic, diastolic, and mean blood pressures, before and after treatment, were significantly depressed.

In relation to the systolic, diastolic, and mean blood pressures, among the variables that might influence the degree of depression in blood pressure after treatment, only the family APGAR showed statistically significant. On the other hand, sex, age, obesity, smoking, alcohol, the presence of complications, and the duration of hypertension were not statistically significant.

As a result, the degree of family dysfunction was observed as a significant impact on the degree of depression in blood pressure after treatment.

However, because this study included the old-aged, female, relatively high in compliance, the effect of smoking and alcohol to the depression of blood pressure were excluded. Therefore the study dealing with this points should be explored in the future.

The family physician who is at close contact with hypertensive patients, must be familiar with the family dysfunction which may disturb the curative activity of the antihypertensive medications, and make every effort to resolve family dysfuction.
