성선기능 부전증 환자에서 배란유도시 에스트로겐 증가가 칼슘대사에 미치는 영향

폐경이후 여성에서 에스트로겐의 칼슘대사에 대한 작용은 잘 알려져 있으나, 폐경기 이전 연령층의 성선부전증 환자에서의 역할은 잘 알려져 있지 않다. 이에 본 저자는 연세대학교 의과대학 세브란스병원 산부인과에서 성선부전증으로 진단된 폐경이전 연령층의 환자로서 입원 또는 내원하여 배란유도를 받은 11명을 실험군으로, premarine을 4주간 투여받은 15명을 대조군으로 하였으며, 혈청 E^^2, LH, FSH,칼슘, 인산 및 i-PTH농도를 뇨중 칼슘 대 크레아틴 비, 인산 대 크레아틴 비를 측정 비교하여 다음과 같은 결론을 얻었다.

1. 실험군 및 대조군 공히 실험전 혈청 E^^2 평균농도는 각각 39.6pg/ml(Ⅰ군), 25.8pg/ml(Ⅱa군) 및 32.4pg/ml(Ⅱa군)으로 감소되어 있었으며, 1군 11명중 9명, 2군 15명중 11명에서 골밀도 검사상 골 조송증을 보였다.

2. 실험군에서 뇨중 칼슘 대 크레아틴 비는 배란유도전 0.200 0.134, 배란유도후 0.095 0.056으로 매우 의의있는 감소를 보였다.

3. 대조군에서 뇨증 칼슘 대 크레아틴 비는 premarine투여 전 0.208 0.217, 투여후 0.135 0.127로 의의있는 감소를 보였다.

4. 뇨증 칼슘 대 크레아틴 비의 배란유도 전후 mean difference는 premarine투여 전후의 것에 비해 높은 경향을 보였다.

5. 실험군에서 뇨증 인산 대 크레아틴 비는 배란유도 전의 0.34 0.15, 배란유도 후 0.41 0.13으로 증가하였으나 통계적 의의는 없었다.

6. 실험군에서 E^^2의 평균농도는 배란유도 전 39.6pg/ml, 배란유도 후 426pg/ml로 현저히 증가하였으나. 이에 따른 혈청 칼슘, 인산 및 i-PTH의 의의있는 변화를 관찰할 수 없었다.

[영문]

The role of estrogen in calcium metabolism in postmenopausal women has been well known, but in premenopausal women with hypogonadism its role is not well established. In this study, the effect of estrogen on calcium metabolism was investigated in 26 patients. 11 patients who underwent ovulation induction served

as a study group and 15 patients who were treated with premarine only served as a control group. Serum concentrations of E^^2, LH, FSH, Ca, P and i-PTH were measured by specific radioimmunoassay and urine Ca/Cr and phosphate/Cr were measured.

The result obtained were as following.

1. The basal serum E^^2 levels before the study in both study and control group were as low as 39.6 pg/ml(Group Ⅰ), 25.8 pg/ml(Group Ⅱa), and 32.4 pg/ml(Group Ⅱb) respectively.

2. In study group, the Ca/Cr in urine before and after the ovulation induction were 0.200±0.134 and 0.095±0.056 respectively and it was significantly decreased after ovulation induction.

3. In control group, the Ca/Cr in urine before and after premarine therapy were 0.208±0.217 and 0.135±0.127 respectively but it was significantly decreased after premarine therapy.

4. The mean difference of Ca/Cr in urine in study group showed higher tendency to increase.

5. In study group, the phosphate/Cr in urine before and after ovulation induction were 0.34±0.15 and 0.41±0.13 respectively and it was not stastistically significant.

6. Mean serum E^^2 level in study group significantly increased after ovulation induction from 39.6 pg/ml to 426 pg/ml, but significant differences were not noted in mean serum concentration of Ca, phosphate and i-PTH.
