여러 가지 원인으로 인한 뇌손상환자들에게는 조기에 팀접근법에 의한 포괄적 재활치료를 시행하는 것이 중요하며, 또한 이들 환자의 기능회복의 정도를 예측하는 것이 필요하다. 이러한 기능회복의 예후와 관련되는 인자로 여러 가지 임상적 인자, 뇌전산화단층촬영, 뇌압측정 및 유발전위를 이용한 연구가 많이 보고되었으며, 특히 최근에는 유발전위의 유용성이 점차 강조되고 있는 실정이다. 우리나라에서도 뇌손상환자들에 대하여 여러 유발전위검사를 시행한 보고들이 있지만, 아직 뇌간청각유발전위와 Middle Latency Respo
nse을 시행한 연구보고가 드문 실정이다. 그러므로 뇌손상환자들에 대한 기능회복의 정도를 예측하는데 있어서, 뇌간청각유발전위와 Middle Latency Response의 유용성을 알아 보고자 본 연구를 시행하였다.
저자는 정상성인 63명을 대상으로 Middle Latency Response의 정상치와 자극부위, 성별 및 연령의 영향을 살펴 보았으며, 1988년 3월 1일부터 1989년 1월 31일까지 연세대학교 의과대학 재활의학과에, 뇌졸중이나 외상으로 인하여 뇌손상을 받고 입원하여 치료를 받았던 30명을 대상으로 입원당시에 뇌간청각유발전위와 Middle Latency Response을 시행하여, 뇌손상환자의 기능회복에 관한 예후인자로서의 유용성을 조사한바, 그 결과를 요약하면 다음과 같다.
이상의 결과에서 포괄적 재활치료를 시작하는 뇌손상환자에게 기능회복에 관한 예후예측을 위하여서는, 입원당시에 뇌간청각유발전위와 Middle latency Response을 동시에 시행하는 것이 도움이 된다고 생각한다.
[영문]
The purpose of this study is to demonstrate normal values and age effect on the Middle Latency Response, and to evaluate the prognostic value of brain-stem auditory evoked potentials and Middle Latency Response in brain injured patients.
The subjects of this study were sixty-three adult volunteers and thirty brain injured patients who were admitted to the Department of Rehabilitation Medicine, Yonsei University College of Medicine, and managed with a comprehensive rehabilitative treatment between March 1, 1988 and Jan. 1, 1989.
The main results are as follows:
1. In the Middle Latency Response, the mean latencies of wave Na and Pa were 21.7±2.50 msec and 32.5±2.14 msec, respectively; and the mean amplitude of Na-Pa was 1.03±0.383 uV.
2. Although no significant effect of sex and site was noted, the mean latencies of wave Na and Pa became significantly prolonged and the mean amplitude of Na-Pa larger with increments of age(p<0.05).
3. Among the 30 brain injured patients, there were 21 males and 9 females. The mean age was 48.3 years.
4. All of the brain injured patients showed normal brain-stem auditory evoked potentials.
5. At discharge, the brain injured patients who showed normal Middle Latency Response at admission had achieved significantly greater improvement in their functional status compared with those who showed abnormal Middle Latency Response(p<0.01).
According to the above results, the Middle Latency Response study in the early stage of rehabilitation is helpful in predicting the prognosis of brain injured patients.
