항경련제의 장기투여가 갑상선 기능에 미치는 영향

여러종류의 항경련제를 장기간 단독 혹은 병용 투여하였을 때 갑상선 기능의 변화에 관한 연구는 1961년 Oppenheimer 등이 phenytoin의 장기투여에 따른 Protein-bound iodine외 감소를 보고한 이래 다각적이고 지속적으로 행하여져 왔다. Carbamazepine, sodium va

lporate, phenobarbital등도 갑상선 기능의 변화와 관계가 있다고 알려지고 있으며 드물게는 이들 약물에 의한 갑상선 기능 저하증이 보고된 예도 있다.

본 연구는 장기간 항경련제를 투여하는 환아에서 투여기간 동안 갑상선기능의 변화를 관찰하고 투여 기간과의 상관관계를 조사하여 갑상선 기능저하의 예방을 목적으로 시작하였다.

여러종류의 항경련제를 투여받는 소아 간질 환아 108명을 대상으로 T3, T4 및 TSH를 측정하여 같은 연령의 대조군과 비교한 결과 장기간 항경련제를 투여한 군에서 갑상선 기능의 변화가 관찰되었는데 주로 T4 및 TSH가 감소하는 것을 관찰할 수 있으며 이러한 변화는 특히 carbamazepine과 phenttoin을 병용투여할 때와 3가지 이상의 항경련제를 동시에 투여할 때 더욱 현저하였으며 투여기간이 5년 이상인 경우 가장 현저하였다.

그러므로 calbamazepine과 phenytoin을 포함한 여러 종류의 항경련계를 동시에 투여할 때 그리고 5년 이상 장기투여할 때 갑상선 기능에 특히 유의하여야 하며. 이러한 경우 증상이 없더라도 갑상선의 기능을 정기적으로 평가하여 보아야 할 것이다.

Serum concentrations of total triiodothyronine(73) and thyroxine(74) as well as serum thyroid-stimulating hormone(TSH) were measured in 108 patients with childhood epilepsy taking anticonvulsants and in 54 normal healthy children as control group.

Of these 108 patients. 23 were treated with carbamazepine alone. 17 were phenytoin alone and the rest 68 were treated with combination of two or more drugs involving carbamazepine and phenytoin.

The 74 and the TSH concentrations were depressed in anticonvulsant treated group, and the degree of the decrease was more severe in a group who had combination therapy with carbamazepine and phenytoin. And as for the treatment duration, the degrees of the decrease of the 74 and the TSH were most markedly observed in more than 5 year treatment group. There was no significant decrease in serum 73 concentration in any treatment group. The possible mechanisms of the influence of

anticonvulsants on thyroid function were discussed.

It is thought that thyroid function test would be needed during long- term anticonvulsant therapy.
