자궁내막증에 관한 임상적 고찰

자궁내막증은 수태 능력에 영향을 미칠 수 있기 때문에 조기 진단 및 치료가 매우 중요하나 임상적인 진단은 그리 용이하지 않다.

우리나라의 경우 자궁내막증은 그 빈도가 비교적 낮은 질환으로 인식되어 왔으나 최근에는 점차 증가하는 경향을 보이고 있다.

저자는 점증하는 자궁내막증에 대한 관심을 높이고 환자 치료 및 이에 관한 연구에 기초 자료를 얻고자 1971년 1월부터 1982년 12월까지 연세대학교 의과대학 부속 세브란스 병원 산부인과에서 자궁내막증으로 진단된 114예를 조사 분석하여 다음과 같은 결론을 얻었다.

1. 자궁내막증 환자의 빈도는 부인과 입원 환자의 1.0%, 개복 수술 환자의 2.2%를 차지하였으며 유병율이 점차적으로 증가하는 추세를 보였다.

2. 자궁내막증 환자의 연령 범위는 18∼57세였고 평균 연령은 36.0세였다.

3. 자궁내막증 환자의 평균 임신력과 평균 출산력은 대조군(무작위 추출한 부인과 환자)보다 의의있게 낮았다. (p<0.01)

4. 자궁내막증 환자의 주증상은 월경 곤난증, 골반 동통, 질 출혈, 불임증 등의 순이었으며 불임증을 호소하는 경우는 28.4%였다.

5. 수술 전에 자궁내막증을 의심할 수 있었던 경우는 22에로 19.3%에 불과했다.

6. American Fertility Society 분류법에 의하 28.1%가 제1기 자궁내막증, 27.2%가 제2기, 36.8%가 제3기, 7.9%가 제4기에 속하였다.

[영문]

This is a retrospective clinical study of endometriosis which was diagnosed at Department of Obstetrics and Gynecology, Yonsei University Medical Center from January 1, 1971 to December 31, 1982. The results were as follows.

1. The incidence of endometriosis was 1.0% among the patients who were admitted, and 2.2% among the laparotomy patients in gynecologic department, The incidence of endometriosis has been on rise.

2. The age of patients ranged from 18 to 57 years and the mean age was 36.0 years.

3. Compared with the control group (random sampled gynecology patient)both the average gravidity and the average parity of endometriosis patients were significantly low (p<0.01).

4. The main symptoms were dysmenorrhea, pelvic pain, vaginal bleeding and infertility in order of frequency. Notable was the complain of infertility by 28.4% of the patients.

5. Clinical suspect of endometriosis before laparotomy was observed in only 22 cases, representing a 19.3%.

6. According to the classification of American Fertility Society, 28.1% of patients were classified into stage Ⅰ, 27.2% into stage Ⅱ, 36.8% into stage Ⅲ, and 7.9% into stage Ⅳ.
