본 연구는 교정치료의 결과에 중요한 영향을 미치는 교정치료 시작시기에 관련된 요인들을 밝혀내기 위한 것이다. 서울지역의 Y대학병원(56.1%)과 교정전문 치과의원(43.9%) 3군데에서 교정치료중인 환자 및 보호자를 대상으로 설문 조사하였으며, 조사기간은 1999년 10월 11일 - 11월 6일까지였다.

인구사회학적인 특성, 부정교합 상태와 인식도, 심리적 요인이 교정치료의 시작시기에 미치는 영향을 분석해 봄으로써, 적절한 치료시기의 선택을 방해하는 요인들을 미리 제거할 수 있으며, 심리상태의 파악으로 환자 협조도를 증진시킬 수 있고, 조기치료를 위한 동기유발, 환자 및 보호자를 위한 계몽용 프로그램 개발에 도움이 될 수 있을 것으로 생각된다.

분석에 사용된 자료는 모두 554부였고 이들 자료는 SAS통계 package를 이용하여 빈도분석, 교차분석(Chi-square test), 다중회귀분석(multiple regression)을 사용하여 통계 처리한 결과 다음과 같은 결론을 얻었다.



교정치료의 시작시기는 남자의 경우 7세 - 12세(혼합치열기)가 가장 많았고, 13세 - 18세(영구치열기)가 다음으로 많았으나, 여자는 7세 - 12세(혼합치열기)가 가장 많았고, 19세 이상(성인)이 13세 - 18세(영구치열기)보다 더 많았다.



부모의 학력수준이 높을수록 조기에 교정치료를 시작하였고, 가족 중 어머니의 치아배열이 나쁜 경우 조기에 치료를 시작하였으며, 아버지의 직업과 가족의 월 평균 소득에 따라 치료시기에 유의한 차이가 있었다.

치아배열의 인식에서, "입이 너무 튀어 나와 보인다" 와 "턱에서 소리가 난다"는 경우에 19세 후에 치료를 시작한 경우가 가장 많았으나, 나머지 인식 군은 7세 - 12세(혼합치열기)에 치료를 시작한 경우가 많았다. 교합상태에 따라서는 III급 부정교합 환자들은 조기에 교정치료를 시작하였으며, I급 부정교합 환자들은 다소 늦게 시작하였다. 치과의원 환자는 I급 부정교합이 많았으나, 대학병원 환자들은 III급 부정교합이 가장 많았다.



치료를 받는 동기는 외모의 개선이 현저히 많았으며, 치료의 필요성을 크게 느낄수록 조기에 치료를 시작하였다. 교정치료의 시작시기에 대해서는 영구치 교환 후에 치료를 시작하는 것이 좋다고 생각하는 경우가 가장 많았고, 언제 치료를 시작해야 될지를 모르는 경우도 많았으며, 이에 관한 인식에 따라 치료 시작시기와의 관계가 유의한 차이를 보였다.

또한 교정치료에 대하여 치과의사나 가족, 친지가 권유한 경우 비교적 조기에 시작하였으며, 본인 스스로나 친구 동료에 의한 경우에는 시작시기가 늦었다.



심리적인 요인은 치아배열이 고르지 않아 놀림을 받은 경험이나 놀림의 형태에 따라 유의한 차이를 보였으며, 놀림 받은 후의 성격이나 사회생활에는 유의한 차이가 없었다.



이상의 결과를 정리해 보면 부모의 학력이 높을수록 , 어머니의 치아배열 상태가 나쁜 경우, 치료의 필요성을 더 느낄수록, III급 부정교합일 경우, 개인의원보다 대학병원에서 교정치료를 조기에 시작하고 있었다. 반면에 많은 환자나 보호자는 교정치료의 최적시기

를 영구치열기로 알고 있거나 전혀 모르고 있었다. 교정치료의 적절한 시작시기에 대한 홍보 및 교육이 더 필요할 것으로 생각된다.

[영문]

The purpose of this study is to investigate the effects of various factors on orthodontic treatment timing.

A survey was conducted on patients and their parents who were being treated for malocclusion in Y University Dental Hospital and three private orthodontic clinics in Seoul.

The relationship between sociodemographic factors, malocclusion, perception, and psychologic factors to the timing of the first orthodontic treatment was constructed using multiple regression analysis.

The results will be utilized to promote cooperation by understanding of patient's psychologic condition, to induce the motivation of early treatment, and to develope the educating programs for patients and parents by removing the disturbing factors to select optimal treatment timing.

The 554 samples were obtained. The statistical methods used in the analysis were frequency tables, Chi-square test and multiple regression analysis.

The results are as follows;



1. The frequency of the time of the first orthodontic treatment was most frequent in 7 - 12 ages(mixed dentition) in both males and females, but adult patients were more than in permanent dentition(13 - 18 ages) in females. Also the first treatment time in males was earlier than that in females.



2. The education level of parents was higher, the time of the first orthodontic treatment was earlier. The treatment time was early in mother's poor occlusion. And the treatment time was different according to father's job and family income.



3. The first treatment time in "protrusive lip" and "clicking sound in TMJ" case was mainly adult stage(after 19 years old), but remaining of tooth arrangements was started in 7 - 12 ages. According to malocclusion types, Class III malocclusion was

treated earlier and Class I malocclusion was treated more later.



4. The main motivation of treatment was improvement of facial esthetics. The necessity of treatment was higher, the treatment was started earlier. The patient and parent's perception of optimal treatment timing was permanent dentition, and many patients and parents didn't know the optimal timing, but beginning time was not different.

The treatment by the urging of family and dentist was started earlier, but treatment decided by patient was started later.



5. The rate of teasing experiences and types due to malocclusion were different, but the first treatment time was not different.



This study suggests that more investigation must be carried in education programs about the first orthodontic treatment time for patients and parents.
